Mohammad Hoesin Palembang. Lumbar disk herniation (LDH) is the most common disease that becaused low backpain and functional disability. Some studies mentioned that hydrotherapy is an effective treatment for low back pain.Therefore, this study was conducted to determine the effect of hydrotherapy on reducing pain intensity and improvingfunctional ability in LDH patients. This study was a pre-experimental study with one group pretest-posttest design. Datawas collected by direct interviews to the patients using visual analogue scale (VAS) to measure pain intensity andModified Oswestry Low Back Pain Disability Questionnaire to assess functional ability before and after underwenthydrotherapy once a week for 4 weeks. The data then undergo Shapiro-Wilk normality test and continue with Paired t-Test or Wilcoxon test. From 30 subjects, it was found that there are effects of hydrotherapy on reducing pain intensity(p<0,001) and improving functional ability (p<0,001) in LDH patients that undergo non-operative procedure at theMedical Rehabilitation Installation of RSUP Dr. Mohammad Hoesin Palembang. There are effects of hydrotherapy onpain intensity and functional ability in LDH patients that undergo non-operative procedure.

Hernia nukleus pulposus (HNP) lumbal merupakan penyakit yang paling sering menjadi penyebab nyeri punggung bawah dan disabilitas fungsional. Beberapa studi menyebutkan bahwa hidroterapi merupakan terapi efektif untuk pasien dengan nyeri punggung bawah. Oleh karena itu, penelitian ini dilakukan untuk mengetahui pengaruh hidroterapi terhadap penurunan intensitas nyeri dan perbaikan kemampuan fungsional pasien HNP lumbal. Jenis penelitian ini adalah praeksperimental dengan desain one group pretest-posttest. Pengambilan data dilakukan melalui wawancara langsung mengenai intensitas nyeri yang diukur menggunakan visual analogue scale (VAS) dan kemampuan fungsional yang diukur menggunakan Modified Oswestry Low Back Pain Disability Questionnaire sebelum dan sesudah hidroterapi selama 4 minggu dengan durasi satu kali seminggu. Uji normalitas data dilakukan dengan metode Shapiro-Wilk dan selanjutnya dianalisa dengan Paired t-Test atau Wilcoxon. Dari 30 subjek penelitian, didapatkan bahwa hidroterapi berpengaruh terhadap penurunan intensitas nyeri (p<0,001) dan perbaikan kemampuan fungsional (p<0,001) pasien HNP lumbal yang tidak menjalani tindakan operatif di Instalasi Rehabilitasi Medik RSUP dr. Mohammad Hoesin Palembang. Terdapat pengaruh hidroterapi terhadap intensitas nyeri dan kemampuan fungsional pasien HNP lumbal yang tidak menjalani tindakan operatif.

Abstract ObjectivePercutaneous transforaminal endoscopic discectomy (PTED) is minimally invasive and has been widely used to treat patients with lumbar disk herniation(LDH) due to its safety and efficiency. However, due to the unique anatomy of the L5-S1 level, the PTED procedure is often difficult to perform in the region. ZESSYS, a targeted and quantificational foraminoplasty device, may help to overcome these anatomical limitations. In this study, we assessed the efficiency and the short-term effects of PTED with ZESSY at the L5-S1 level.MethodsBetween January to August of 2018, Fifty-six patients with lumbar disk herniation at the single level of L5–S1 and underwent percutaneous transforaminal endoscopic discectomy were enrolled in this retrospective cohort study. They were segregated into the the TESSYS group and the ZESSYS group. The puncture time, foraminoplasty time, decompression time and fluoroscopy time evaluated for the operation efficiency. Clinical outcomes were assessed by visual analog scale(VAS) score and Oswestry Disability Index(ODI) score. The MacNab criteria were used to evaluate patient subjective satisfaction at 12-month follow-up postoperatively.ResultsThe average puncture time (5.29±2.05 minutes), foraminoplasty time (12.82±2.52 minutes) and fluoroscopy time (26.29±5.96 seconds) were all significantly shorter in the ZESSYS group than in the TESSYS group (average puncture time 8.07±3.13 minutes, p<0.01; foraminoplasty time, 17.18±2.92 minutes, p<0.01; fluoroscopy time, 34.73±6.86 seconds; p<0.01). No significant differences were observed between the 2 groups in the decompression time (p=0.057). The VAS score of low back pain and leg pain, as well as the ODI score, improved at all time points postoperatively compared with preoprative , in both the TESSYS group and the ZESSYS group (P < 0.05). There were no significant differences in the VAS score of low back pain, VAS score of leg pain, and ODI score between the TESSYS group and the ZESSYS group at the same time points (P > 0.05). According to the MacNab criteria, the excellent and good rate at 12-month follow-up postoperatively was 85.7% in the TESSYS group and 89.3% in the ZESSYS group (P > 0.05).ConclusionThe targeted and quantificational foraminoplasty device named ZESSYS was more efficient in the puncture and foraminoplasty procedures, effectively protecting the exiting nerve and minimizing the level of radiation exposure. The device is efficient and safe for PTED in treating lumbar disk herniation at the L5-S1 level.

OBJECTIVE: The objective of the study was to compare between the conventional discectomy and the interbody fusion with bilateral pedicular screw fixation in the management of single level lumbar disk herniation. METHODS: This is a prospective study done on 50 patients (25 in each group) at Cairo University Hospitals in the period between October 2018 and June 2019 fulfilling the inclusion criteria. Diagnosis was made clinically with history (low back pain and sciatica),examination (motor power assessment, straight leg raising test), and radiological findings (MRI, CT and X-ray lumbar spine). The pain status (VAS) was pre and postoperatively evaluated and followed up every 3 months for 1 year. RESULTS: The study included 50 patients (25 in each group) (mean age 40.4 years old) with slight female predominance. The most common clinical findings presented at diagnosis were low back pain followed by lower limb pain in the form of claudication and sciatica. L5-S1 disk prolapse was the most common level affected in both groups followed by L4-5 level. Regarding the clinical outcome, there was statistical significance in the VAS of low back pain at 9 months and 1-year follow-up favoring the fusion group with mean VAS 0.40 and 1.32 (p < 0.05). CONCLUSION: Clinical outcome was excellent in both groups. However, the reduction in low back pain after surgery was greater in the fusion group (p < 0.05). The rate of recurrent disk herniation at the surgical level in the nonfusion group was higher, but intraoperative blood loss, operation time, length of hospital stay, and total cost of procedure were all less in the patients undergoing discectomy alone. Although there is still controversy regarding the pros and cons of fusion in association with disk excision without instability, fusion results were highly favorable.

INTRODUCTION: Ozonetherapy is a therapeutic tool used in the treatment of low back pain associated to herniation of lumbar disks. OBJECTIVE: The objective of this systematic review was to ratify the relevance of this treatment in clinical practice, besides emphasizing its possible utilization on physiotherapy. MATERIALS AND METHODS: PRISMA and PICOS were used to analyze the manuscripts design. Manuscripts selection was made by a research in the PUBMED, Periódicos CAPES and Scielo databases. Four clinical trials were selected according to the inclusion criterias designer for the study. RESULTS: All the authors confirmed the efficiency of ozonetherapy as a therapeutic method in reversing the algic symptomatology of pacients whit lumbar disc herniation. Ozonetherapy associated to the physiotherapeutic treatment can contribute pain relief related to low back pain by influencing the improvement in patients quality of life. CONCLUSION: Ozonetherapy is an effective therapeutic option for patients whit low back pain associated whit lumbar disk herniation.
