The Hemicorporectomy in 2020: A Case Report and Recent Advances

Chronic decubitus ulcers are a common problem in spinal cord injury (SCI) patients. Late-stage ulcers often lead to chronic osteomyelitis, which carries high rates of morbidity and mortality, as well as significant healthcare costs. Refractory decubitus ulcers are most commonly seen in the pelvis and pose a clinical challenge to provider teams caring for SCI patients. With long-term disease, surgical and antibiotic therapies may fail, which places patients at risk for entering a terminal disease state. A radical treatment for this condition was described in the early 20th century and involves amputation of the lower half of the body. This procedure, the hemicorporectomy, continues to be performed today in a very small, select group of patients. Here, a case report is described of a 39-year-old female T12 paraplegic with refractory decubitus ulcers and terminal pelvic osteomyelitis that underwent hemicorporectomy as definitive treatment. The procedure was successful and the patient was safely discharged to a long-term rehabilitation facility.

ABSTRACT All of us as clinicians at one time or another have to attend to dental emergencies involving severly fractured or even lost or avulsed anterior teeth. Treating such cases is often difficult and there is always a time factor involved. Loss of anterior teeth can affect a patient psychologically and socially. This can be minimised by an immediate replacement or replantation of the lost or avulsed teeth. Definitive treatment planning and consultation with specialists is seldom possible at the time of emergency treatment. Replantation of the avulsed tooth can restore esthetic appearance and occlusal function shortly after the injury. This article describes the management of a young female with an avulsed maxillary permanent incisor that had been air-dried for about 72 hours. The replanted incisor retained its esthetic appearance and functionality 8 months after replantation, yet the long-term prognosis remains questionable.

Background: Malignant melanoma (MM) usually present with metastases to unexpected regions of the body. Metastatic MM is a highly lethal condition, and the median survival in this setting is 6 to 7.5 months; however, few reports rarely describe long-term after chemotherapy.Case Report: We describe a 31-year-old man with MM, which got metastatic (to paranasal sinuses) after local and systemic therapy showed complete responses with long-term survival after endonasal endoscopic metastasectomy and radiotherapy of the nasal cavity, paranasal sinuses, and base of the skull.Conclusion: Although long-term survival is rare, few reports describe cases after chemotherapy. MM could be associated with metastasis to any regions and clinicians should be aware of its behavior and perform complete investigation in the presence of any suspicious symptoms, and this should be reinforced periodically. However, the survival is poor in the metastatic setting, and the treatment of choice is debatable, some patients may benefit from metastasectomy and local radiotherapy. [GMJ.2018;7:e860]

Introduction The insertion of nasogastric (NG) feeding tube or Ryle’s tube is a common procedure for treating patients in different medical or surgical conditions. One of its indications is in patients who can’t eat or swallow due to obstruction in upper digestive tract. Case Report We encountered a 71 year old female patient with stricture in mid to low esophagus, who presented with a retained NG tube in situ for more than 16 years. Post admission, an NCCT scan of neck, thorax and upper abdomen showed about 30cm long retained tube with its lower end in the body of stomach. Upper gastro-intestinal endoscopy was subsequently performed and the retained tube was carefully removed in toto. Discussion Insertion of nasogastric tube is a frequent and well tolerated day to day procedure though it can produce unexpected complications like stricture, perforation or haemorrhage and even spontaneous transection in a few patients with prolonged indwelling Ryle’s tube. Long term placement of nasogastric tube is thus not recommended to avoid complications.

A journey of a young man started from day one in the hospital. When he came with the traumatic paraplegia, no doubt a paraplegic has to face a lot of challenges in his life when he came to know that his both legs are not working. He became bedridden and his moral is also become down for his future planning. But we accept the challenge, a team work by an orthopaedician and a physio-occupational therapist make it little easier. Postoperatively we prefer to offer rehabilitation through community base programme. Inspiration of hospital environment and equipment are going to help him for such kind of programme he accepts the challenge, basic aim of providing this programme is to reduce duration as well as incurred monitor losses. Special attraction of this case report is homemade bicycle, which served the purpose of providing repetitive rhythmic mobilisation of bilateral lower limbs which facilitates normal pattern of ambulation. Now he is ambulatory and goes back to his work. So, a person with spinal cord injury involved with their community with a little modification in his activity and equipment is worth for him. Traumatic paraplegia offers a big challenge to physio- occupational therapist due to long term recovery it takes.  For Such patients on regular follow up till complete recovery, we preferred community based rehabilitation. This made him independent and incurred reduced monitory losses.
