Palato-gingival groove is a developmental anomaly often affecting the maxillary lateral incisor. The aim of this presentation is to describe the clinical management of a maxillary lateral incisor tooth with a palato-gingival groove with perio-endo lesion having immature apex. Despite complex anatomy, this case was managed by using Mineral Trioxide Aggregate plug technique with as a non-surgical endodontic treatment. In this study, 120 teeth having periradicular pathosis were treated with single visit root canal treatment and then obturated with either calcium hydroxide containing sealer or mineral trioxide aggregate based sealer. The healing of periradicular pathosis was examined at 3, 6, and 12 months by clinical and radiological evaluation. Collected data were analyzed with standard statistical methods by using SPSS version- 20. The results showed that at 3 and 6 months, the differences between calcium hydroxide containing sealer or mineral trioxide aggregate based sealer were not statistically significant. However, the healing capability of periapical radiolucency at 12 months between two groups was statistically significant (P=0.03). The final outcome of the clinical and radiological investigation showed successful results of 96.6% and 98.1% in calcium hydroxide and mineral trioxide aggregate group, respectively. In conclusion, both calcium hydroxide and mineral trioxide aggregate based sealers are almost equally effective in single visit root canal therapy for the management of periradicular pathosis. Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.

Maxillary lateral incisor agenesis is the most prevalent developmental dental anomaly. The management of missing lateral incisor was either closure using canine as substitution or creation of space orthodontically for prosthetic replacement. A careful diagnosis and treatment plan are deemed essential to address the patient’s needs as the spacing is present in the esthetic region. Such problem is very challenging for orthodontists, prosthodontists, and general practitioners. This case report describes the orthodontic management of a 22-year-old adult female patient with missing upper left lateral incisor tooth and upper anterior spacing by closing the space with canine lateralization and reshaping to simulate it with the lateral incisor. However, some modifications in the treatment mechanics are crucial to achieve the optimal esthetic and to improve the occlusion. Space closure with canine lateralization option seems less invasive, treatment can be completed relatively in a short period of time, and its adaptation with the facial changes throughout life without having artificial prosthesis provided other factors favoring for this option.
