Background and Objectives In this study, we introduce our method of hearing aid (HA) verification using real ear measurement (REM). We verified HAs that have gone through the fitting program using speech mapping REM; we then compared the outcome with word recognition scores (WRS) to evaluate functional gain. Subjects and Method Fifty-six patients of sensorineural hearing loss (81 ears) were enrolled in the study. In REM, if the gap between the target gain of HA and real ear aided response (REAR) was less than 10 dB SPL, fitting was considered successful. In speech audiometry, unaided maximum discrimination score (PB max), unaided WRS at 65 dB HL and aided WRS at 65 dB HL were measured. By comparing PB max and aided WRS at 65 dB HL, patients were sorted into best (n=15), good (n=57), and poorly (n=9) aided groups and analyzed for the successes of fitting. Fitting was deemed unsuccessful if REAR was ≥10 dB SPL lower than the target value of HA.Results The mean aided WRS at 65 dB HL of best, good and poorly aided groups were 85.6%, 77.3%, and 54.2%, respectively. There were statistically significant differences between all groups (p=0.019, 0.001, 0.002). The success rates of HA fitting showed significant differences at 0.5, 0.75, 1, 4 kHz of 55 dB SPL (p=0.023, 0.005, 0.003, 0.014), and at 4 kHz of 65 and 75 dB SPL (p=0.004, 0.001). The high WRS group showed sufficient gain at many frequencies. Conclusion Well fitted HAs can provide sufficient increase in speech intelligibility. Using the speech mapping REM is a great method to verify fitting of HA.

Objectives -to determine the normative data of sentence speech intelligibility in a free sound field and to estimate the applicability of the Russian Matrix Sentence test (RuMatrix) for assessment of the hearing aid fitting benefit. Material and methods. 10 people with normal hearing and 28 users of hearing aids with moderate to severe sensorineural hearing loss were involved in the study. RuMatrix test both in quiet and in noise was performed in a free sound field. All patients filled in the COSI questionnaire. Results. The hearing impaired patients were divided into two subgroups: the 1st with high and the 2nd with low hearing aid benefit, according to the COSI questionnaire. In the 1st subgroup, the threshold for the sentence intelligibility in quiet was 34.9 ± 6.4 dB SPL, and in noise -3.3 ± 1.4 dB SNR, in the 2nd subgroup 41.7 ± 11.5 dB SPL and 0.15 ± 3.45 dB SNR, respectively. Excessive amplification by hearing aids causes temporary threshold shift (TTS) and permanent threshold shift (PTS). This investigation addressed the question whether it might be possible to predict the eventual amount of PTS caused by excessive amplification from the amount of TTS it causes after a day of hearing aid use. Asymptotic TTS (ATS) to be expected as a result of hearing aid use was predicted for 8 children with sensorineural hearing loss and the predicted ATS was compared with observed permanent deterioration of their thresholds attributed to hearing aid use. There was good agreement between the predicted ATS and observed PTS at 500 to 2000 Hz. It follows that, for prediction of PTS caused by hearing aid use, the mean of the sound levels produced in the ear by the hearing aid is the correct equivalent continuous level (ECL) to use and that the Modified Power Law (MPL) is the appropriate method of adjusting the predictions for sensorineural hearing loss, because these have been shown to be appropriate for prediction of TTS caused by hearing aid use. Predictions of the PTS to be expected for the children that were carried out using the MPL and the mean level as the ECL were in good agreement with the observed PTS at 500 to 2000 Hz, whereas predictions of PTS based on an alternative method of correction for sensorineural hearing loss (the Continuation Hypothesis) were significantly less than the observed amounts. The results of the PTS predictions therefore confirmed the conclusions drawn from the results of the ATS predictions.

Objective: To evaluate the long-term safety and performance of four different vibroplasty couplers (round window, oval window, CliP and Bell coupler) in combination with an active middle ear implant. Methods: This was a multicentre, prospective, long-term study including 5 German hospitals. Thirty adult subjects suffering from conductive or mixed hearing loss were initially enrolled for the study, 24 of these were included in the final analysis with up to 36 months of postsurgical follow-up data. Bone conduction and air conduction were measured pre- and postoperatively to evalu ate safety. Postoperative aided sound field thresholds and Freiburger monosyllable word recognition scores were compared to unaided pre-implantation results to confirm performance. Additional speech tests compared postoperative unaided with aided results. To determine patient satisfaction, an established quality-of-life questionnaire developed for conventional hearing aid usage was administered to all subjects. Results: Mean postoperative bone conduction thresholds remained stable throughout the whole study period. Mean functional gain for all couplers investigated was 38.5 ± 11.4 dB HL (12 months) and 38.8 ± 12.5 dB HL (36 months). Mean word recognition scores at 65 dB SPL increased from 2.9% in the unaided by 64.2% to 67.1% in the aided situation. The mean postoperative speech reception in quiet (or 50% understanding of words in sentences) shows a speech intelligibility improvement at 36 months of 17.8 ± 12.4 dB SPL over the unaided condition. The signal-to-noise ratio (SNR) improved by 5.9 ± 7.2 dB SNR over the unaided condition. High subjective device satisfaction was reflected by the International Inventory for Hearing Aids scored very positively. Conclusion: A significant improvement was seen with all couplers, and audiological performance did not significantly differ between 12 and 36 months after surgery.

The aim of this article is to describe the audiological patterns of 71 adult patients presenting severe to profound sensorineural hearing loss, who were rehabilitated by cochlear implants (CIs) and hearing aids. This is a retrospective study in a university setting, where the clinical records of 71 adult patients were reviewed and processed. Speech intelligibility was evaluated at one aided ear (CI) or at both aided ears (double CI or a combination of CI and hearing aid [HA]). Patients with a bilateral CI or with a bimodal hearing setup (CI and HA) performed better than those with a single CI; data from the phonetic matrices test showed that there was a statistically significant difference among patients aided by a single CI versus binaural setup (double CI or CI + HA). In particular, patients aided by a bilateral CI, or by a CI and HA, showed an improvement in the functional results of the speech tests, compared to patients using a single CI. Binaural hearing (either with a bilateral CI or bimodal) allows an improvement in the functional results at the speech tests, compared to the use of a CI only.

Background: Research shows that many older children and teenagers who have mild to moderately severe sensorineural hearing loss do not use their hearing instruments during all waking hours. A variety of reasons may contribute toward this problem, including concerns about cosmetics associated with hearing aid use and the inconvenience of daily maintenance associated with hearing instruments. Extended-wear hearing instruments are inserted into the wearer’s ear canal by an audiologist and are essentially invisible to outside observers. Purpose: The goal of this study was to evaluate the potential benefits and limitations associated with use of extended-wear hearing instruments in a group of children with hearing loss. Research Design: A two-way repeated measures design was used to examine performance differences obtained with the participants’ daily-wear hearing instruments versus that obtained with extended-wear hearing instruments. Study Sample: Sixteen children, ages 10–17 yr old, with sensorineural hearing loss ranging from mild to moderately severe. Data Collection and Analysis: Probe microphone measures were completed to evaluate the aided output of device. Behavioral test measures included word recognition in quiet, sentence recognition in noise, aided warble-tone thresholds, and psychophysical loudness scaling. Questionnaires were also administered to evaluate subjective performance with each hearing technology. Results: Data logging suggested that many participants were not using their daily-wear hearing instruments during all waking hours (mean use was less than 6 h/day). Real ear probe microphone measurements indicated that a closer fit to the Desired Sensation Level Version 5 prescriptive targets was achieved with the children’s daily-wear instruments when compared to the extended-wear instruments. There was no statistically significant difference in monosyllabic word recognition at 50 or 60 dBA obtained with the two hearing technologies. Sentence recognition in noise obtained with use of the extended-wear devices was, however, significantly better than what was obtained with the daily-wear hearing aids. Aided warble-tone thresholds indicated significantly better audibility for low-level sounds with use of the daily-wear hearing technology, but loudness scaling results produced mixed results. Specifically, the participants generally reported greater loudness perception with use of their daily-wear hearing aids at 2000 Hz, but use of the extended-wear hearing technology provided greater loudness perception at 4000 Hz. Finally, the participants reported higher levels of subjective performance with use of the extended-wear hearing instruments. Conclusions: Although some measures suggested that daily-wear hearing instruments provided better audibility than the extended-wear hearing devices, word recognition in quiet was similar with use of the two technologies, and sentence recognition in noise was better with the extended-wear hearing technology. In addition, the participants in this study reported better subjective benefit associated with the use of extended-wear hearing technology. Collectively, the results of this study suggest that extended-wear hearing technology is a viable option for older children and teenagers with mild to moderately severe hearing loss.

Background: In developing the PB-50 word lists, J. P. Egan suggested five developmental principles, two of which were “equal average difficulty” and an “equal range of difficulty” among the lists (page 963). Egan was satisfied that each of the 20 PB-50 lists had equivalent ranges of recognition performances and that the lists produced the same average performances. This was accomplished in preliminary studies that measured the recognition performance of each word and eliminated words that were always or never correct. In preparing for studies of interrupted words, we needed to know the range of difficulty inherent in the speaker specific NU-6 and Maryland CNC materials we planned to use when those words were not interrupted. There were only a few studies in the literature that touched on the range of difficulty characteristic of the word-recognition materials in common usage. The paucity of this information prompted this investigation whose scope broadened to include the CID W-22, Maryland CNC, NU-6, and PB-50 materials spoken by a variety of speakers. Purpose: The purpose was to evaluate the homogeneity with respect to intelligibility of the words that comprise several of the common word-recognition materials used in audiologic evaluations. Research Design: Both retrospective (10) and prospective (3) studies were involved. Data from six of the retrospective studies were from our labs. The prospective studies involved both listeners with normal hearing for pure tones and listeners with sensorineural hearing loss. Study Sample: The sample sizes for the 13 data sets ranged from 24 to 1,030, with 24 the typical number for listeners with normal hearing. Data Collection and Analysis: The retrospective data were from published studies and archived data from our laboratories. The prospective studies involved presentation of the word-recognition materials to the listeners at a comfortable level. An item analysis was conducted on each data set with descriptive statistics used to characterize the data. Additionally, skewness coefficients were calculated on the distributions of word performances and the interquartile range was used to determine minor and major outliers within each set of 200 words and their component 50-word lists (300 words for the Maryland CNCs). Results: For listeners with normal hearing the majority of performances on the words within a 50-word list were better than the mean performance, which produced negatively skewed distributions with outlier performances in every list. For listeners with sensorineural hearing loss the performances on the words within a 50-word list were evenly distributed above and below the mean performance, which yielded essentially normal distributions with few outliers. There were a few words on which performances were better by the listeners with hearing loss. Conclusions: Every list of word-recognition materials has a few words on which recognition performances are noticeably poorer than performances on the majority of the remaining words. If the intention of an experiment is to evaluate performance at the word level, then identifying these “outliers” becomes a necessity. Although not evaluated in this report, the implications for 25-word lists are they should be based on recognition-performance data and not compiled arbitrarily.

A custom in-the-ear (ITE) hearing aid fitting was compared to two over-the-ear (OTE) hearing aid fittings for each of 9 subjects with mild to moderately severe hearing losses. Speech intelligibility via the three instruments was compared using the Speech Intelligibility Rating (SIR) test. The relationship between functional gain and coupler gain was compared for the ITE and the higher rated OTE instruments. The difference in input received at the microphone locations of the two types of hearing aids was measured for 10 different subjects and compared to the functional gain data. It was concluded that (a) for persons with mild to moderately severe hearing losses, appropriately adjusted custom ITE fittings typically yield speech intelligibility that is equal to the better OTE fitting identified in a comparative evaluation; and (b) gain prescriptions for ITE hearing aids should be adjusted to account for the high-frequency emphasis associated with in-the-concha microphone placement.

Deterioration in speech intelligibility, the most common complaint of people with hearing loss, leads to social isolation and dramatically reduces the quality of life. The correction of peripheral hearing loss by hearing aid fitting does not always solve this problem in patients with chronic sensorineural hearing loss (SNHL). That is why in the process of audiological examination of patients with complaints of hearing loss, it is necessary to include methods of speech audiometry, which make it possible to comprehensively assess the functioning of the auditory system. The aim of the study: assessment of speech intelligibility in patients of different ages with SNHL. 94 people from 20 to 74 years of age were examined: 54 patients with SNHL (14 young and 40 elderly) and 40 people with normal hearing thresholds (20 young and 20 elderly). Besides the standard audiological examination, speech audiometry testing was conducted: evaluation of monosyllabic words intelligibility in quiet, binaural rapidly alternating speech test, dichotic digits test, Russian matrix sentence test (RuMatrix). Results: Speech intelligibility in elderly patients with SNHL was shown to be significantly worse than in young patients with a similar degree of hearing loss. The RuMatrix test in noise was proved to be the most sensitive test for comprehensive evaluation of the hearing system functioning.

Objective The efficacy of the Otologics Fully Implantable Hearing Device (MET) was assessed in adult patients with bilateral moderate to severe sensorineural hearing loss. Methods Surgical insertion of this totally implanted system was identical to the Phase I study. A repeated-measures within-subjects design assessed aided sound field thresholds and speech performances with the subject's own, appropriately fit, walk-in hearing aid(s) and the Otologics Fully Implantable Hearing Device. Results Six- and 12-month Phase II data will be presented. Ten patients were implanted and activated as part Phase II clinical trial. Three patients were lost to long term follow-up due to two coil failures and one ossicular abnormality preventing proper device placement. No significant differences between preoperative (AC = 59 dB, BC = 55 dB) and postoperative (AC = 61 dB, BC = 54 dB) unaided pure tone averages were noted (p < 0.05). Pure tone average implant aided thresholds (41 dB) were equivalent to that of walk-in-aided (37 dB) condition with no significant difference (p < 0.05) between patients’ walk-in-aided individual frequency thresholds and implant-aided thresholds. Word recognition scores and hearing in noise scores were similar between the walk-in-aided and for the implant-aided condition. Patient benefit scales will be presented at all end points. Conclusions Results of the Otologics MET Fully Implantable Hearing Device Phase II trial provide evidence that this fully implantable device is a viable alternative to currently available hearing aids in patients with sensorineural hearing loss.
