The aim of this retrospective study was to evaluate whether intraocular (IOP) elevation post-cataract surgery can be reduced by using tropicamide and phenylephrine only, without cyclopentolate. Medical records across two surgical facilities were analyzed. One surgical facility (Cohort A) used a combination of tropicamide, cyclopentolate, and phenylephrine preoperatively, while the other (Cohort B) used tropicamide and phenylephrine only. Of patients in Cohort A, 63.6% (n = 7) had a higher IOP in the operated eye, while it was only 27.3% (n = 3) in Cohort B. Therefore, it is preferable to exclude the use of cyclopentolate in the preoperative dilation regimen of patients undergoing cataract surgery. However, a study with a larger sample population is required to further evaluate the significance of these results.

Objectives. The aim of this study was to assess the effects of clonidine on intraoperative analgesia, sedation, intraocular and blood pressure, arrhythmia, and ischemia.Methods. Forty patients undergoing cataract surgery were allocated into two groups. They were monitored with Holter machine, the pupil was dilated, and 30 minutes later, 20 patients received clonidine (4 µg/kg), while the other 20 patients were given a 0.9% saline intravenously. Twenty minutes later, 2% lidocaine gel was applied. There were assessed intraoperative analgesia, intraocular pressure, blood pressure, heart rate, and the occurrence of arrhythmias and myocardial ischemia.Results. Pain intensity was lower in G1 during the phacoemulsification, irrigation, aspiration, and intraocular lens implantation. The HR and BP were lower with clonidine. The IOP was lower with clonidine after 15 minutes and at the end of the surgery. Sedation was higher with clonidine. The incidence of arrhythmia was lower at the end of surgery with clonidine. The incidence of myocardial ischemia did not differ between the groups.Conclusions. Clonidine (4 µg/kg) before a phacoemulsification reduced the intensity of pain during cataract surgery. It also induced sedation, reduction of BP, HR, and incidence of arrhythmia at the end of the surgery, and did not alter myocardial ischemia. Purpose. The aim of this study is to examine the influence of capsulotomy size on, spherical equivalent (SE), intraocular pressure (IOP), and macular thickness.Materials and Methods. Sixty-eight patients were examined preoperatively and 1, 4, and 12 weeks after Nd:YAG capsulotomy. Patients were divided into two groups based on the postoperative capsulotomy size. Changes in SE, IOP, and macular thickness were compared between two groups.Results. We found a higher hyperopic shift in large capsulotomy group. In both groups 1 and 2, IOP increased 1 week postoperatively. Intraocular pressure rise in group 2 was higher than in group 1. Both groups had increased macular thickness at 1 week postoperatively. The degree of macular thickening was similar in group 1 and group 2.Comment. Patients who underwent a larger capsulotomy have a higher hyperopic shift and IOP elevation. Rise in macular thickness was similar in large and small capsulotomy groups.

Abstract Background: To investigate the long term surgical outcomes of cataract surgery in severe and end glaucoma patients with preoperative intraocular pressure less than 21mmHg, and to detect the associated factors. Methods: A retrospective study of severe and end stage glaucoma patients who underwent cataract surgery or combined with goniosynechialysis from March 2015 to April 2018. Main outcome measures were visual acuity, intraocular pressure, number of glaucoma medications and complications. Results: Twenty patients (24 eyes) were included. The mean age was 64.6 ± 11.0 years and the mean followed up duration was 21.4 ± 7.3 months. The final visual acuity was significantly improved from 0.93±0.72 to 0.70±0.74, within 14 (58.3%) eyes improved, 5 (20.8%) eyes kept unchanged, and 5 (20.8%) eyes reduced. Linear regression analysis indicated that higher mean deviation, higher visual field index and lower glaucoma stage associated with greater final visual acuity improvement (r = -0.545, r = -0.501, r = 0.521 respectively). Moreover, the final number of medications were reduced from 1.1 ± 0.9 to 0.2 ± 0.5 (p < 0.01). The mean intraocular pressure was not significantly reduced with the final IOP of 13.2 ± 3.9 (6.8-25.9) mmHg (p = 0.97). While the eyes with intraocular pressure above 15 mmHg was reduced to 7 (29.2%) eyes compared to 11 (45.8%) eyes at baseline. Moreover, the number of eyes without the use of medications was increased from 6 (25%) preoperatively to 20 (83.3%) eyes postoperatively. Conclusions: Final visual acuity was significantly improved in the severe and end stage glaucoma patients and the number of eyes came off medications increased by 58.3% after cataract surgery. Preoperatively, the glaucoma stage, mean deviation and visual field index are important parameters to predict the visual acuity changes after cataract surgery.

To evaluate the rise in intraocular pressure (IOP) in post cataract surgery patients who received topical steroids.Patients in the age group of 50-75 years and both sexes who were coming to ophthalmology OPD at a tertiary care hospital of Western Maharashtra were studied. 500 subjects who underwent uncomplicated cataract surgery were taken into consideration; both age and sex matched. All patients were screened for raised intraocular pressure by Perkin’s tonometer preoperatively and at 5th day, 2nd week, 4th week, 6th week and 8th week postoperatively. Patients were allocated into two groups: either topical Prednisolone acetate e/d (1%) (Group I) or Dexamethasone e/d (0.1%) (Group II) started six times for 1st week (considering the cells in anterior chamber &#60;2+) & then tapered off subsequently in the following weeks for a period of 5 weeks (starting with six times for 1st week, then tapering to 4 times for the next week, 3 times for the next week, 2 times for the next week, 1 time for the next week and then stopped).In our study group, the prevalence of steroid-responders was 3.6% with 0.8% showing mild rise, 2.8% showing moderate rise in IOP on the fourth week of topical steroid usage. There was no difference in the mean IOP rise with the usage of two different topical steroids (E/d Prednisolone acetate (1%) and E/d Dexamethasone(0.1%)) in our study. The highest mean IOP was noted at the 5th postoperative day (20.26 mmHg) which was due to inflammation & there was a gradual decrease in IOP in the subsequent weeks of follow-up till 8th week. While as duration after cataract surgery progress till 8th week of follow up, moderate rise in IOP occurs which gradually comes down to mild rise till 8th week of follow-up.Steroid-induced IOP elevation typically occurs after 4 weeks of beginning steroid therapy and the prevalence of steroid responders in post cataract surgery patients was established.

Abstract: Complete medical record is clearly important to health service of a hospital. In cases of violence, medical records play a significant role in law enforcement. Prof. Dr. R. D. Kandou Hospital is categorized as type A service hospital that has been fully accredited in 2015. Therefore, its medical personnels should be responsible for the completeness of each patient’s medical record, especially of violence cases. This study was aimed to determine the completeness of physical violence cases’ medical records due to criminal acts at Prof. Dr. R. D. Kandou Hospital in the period of September 2017-August 2018. This was a descriptive retrospective study. The results obtained as many as 40 cases of physical violence due to criminal acts. The medical record completeness of physical violence cases due to criminal acts were as follows: in Emergency Care 40%, inpatients 12%, surgery 58.82%, anesthesia/ sedation 45.45%, blood transfusion 66.67%, and informed consent 96.87%. Conclusion: In physical violence cases due to criminal acts, the medical record completeness with informed consent had higher percentage than the other medical record documents.Keywords: completeness of medical record, physical violence casesAbstrak: Kelengkapan rekam medik merupakan hal penting dalam pelayanan kesehatan dari suatu rumah sakit. Dalam kasus kekerasan, rekam medik berperan dalam penegakan hukum. RSUP Prof. Dr. R. D. Kandou sebagai rumah sakit pelayanan tipe A yang telah terakreditasi paripurna pada tahun 2015, sudah seharusnya kelengkapan rekam medis bagi setiap pasien menjadi tanggung jawab tenaga medis terutama rekam medik kasus kekerasan yang ada di RSUP. Prof. Dr. R. D. Kandou. Penelitian ini bertujuan untuk mengetahui kelengkapan rekam medik kasus kekerasan fisik akibat tindakan pidana di RSUP Prof. Dr. R. D. Kandou periode September 2017 - Agustus 2018. Jenis penelitian ialah deskriptif retrospektif. Hasil penelitian mendapatkan sebanyak 40 kasus kekerasan fisik akibat tindak pidana. Secara keseluruhan kelengkapan rekam medik pada kasus kekerasan fisik akibat tindak pidana di Instalasi Rawat Darurat sebesar 40%, rawat inap 12%, tindakan operatif 58,82%, tindakan anestesi/sedasi 45,45%, transfusi darah 66,67%, dan informed consent 96,87%. Simpulan: Pada kasus kekerasan fisik akibat tindakan pidana, kelengkapan rekam medik dengan informed consent memiliki persentase yang lebih tinggi daripada berkas rekam medik lainnya.Kata kunci: kelengkapan rekam medik, kasus kekerasan fisik
