The key to success in patients with difficult airway is effective airway assessment and meticulous planning. Making use of simple and time-tested equipment and modifying these methods accordingly enable successful airway control avoiding perioperative morbidity and mortality . We report a case of 75 year old male with posterior urethral stricture planned for perineal urethroplasty with anticipated difficult airway. The airway was managed using video laryngoscopy and the procedure was uneventful with good postoperative recovery. This case report insists on making use of other conventional methods in the effective management of an anticipated difficult airway during a pandemic.

The prepubic urethrostomy procedure has been shown to be a long-term diversion technique in dogs with intrapelvic urethral damage. This case report describes the clinical presentation and successful surgical management of an intrapelvic urethral stricture in a four month old male intact Labrador Retriever with hypospadias. The dog was presented for assessment of dysuria, stranguria, urinary incontinence, and urethrocutaneous fistula. After the diagnosis of perineal hypospadias with intrapelvic urethral stricture was made, surgery was performed to relieve the urethral obstruction and repair the urethrocutaneous fistula. A prepubic urethrostomy was made and the transected urethra was pulled through the inguinal canal into the prepuce. The postoperative recovery was uneventful. This technique can reduce postoperative peristomal skin irritation and leads to better cosmetic appearance than the standard technique.

AbstractLoss of airway control in children, if not resolved quickly, will lead to devastating consequences. Successful management of the pediatric difficult airway, both anticipated and unanticipated, is facilitated by preprocedure assessment and preparation. Accessibility of and continued hands-on training with modern airway instruments, familiarization with difficult airway guidelines, and collaboration with multidisciplinary airway teams can aid in the management of the difficult pediatric airway. This review outlines the importance of airway assessment and advanced airway equipment for children. It also discusses difficult airway management techniques and algorithms for the management and rescue of the pediatric difficult airway.

Dermatomyositis is not a very uncommon disease affecting skin and muscles. Some varieties have atypical presentations and early diagnosis is pivotal to the effective management as it has significant morbidity and mortality including malignancy. Corticosteroids and other immunosuppressive is the mainstay of treatment; however, it should also be aimed to reduce the complications of steroid therapy as there is a possibility of steroid induced myopathy where withdrawal of steroid with introduction of other immunosuppressive drug is promptly warranted. Here, we presented a 60 year old lady with dermatomyositis who developed steroid induced myopathy.J Dhaka Medical College, Vol. 24, No.2, October, 2015, Page 156-159

Video laryngoscopy has opened its way into airway management and continues to play a larger role in managing patients with difficult airway. Anesthesiologists use video laryngoscopy more often every day and therefore accept the risks of more challenging and difficult airway situations with more confidence. In the mentioned case below, a 45-year-old female with a large thyroid mass is presented to an academic based hospital, for elective thyroidectomy. The patient was intubated using a video laryngoscopy technique and the airway was secured. Video laryngoscopy helps secure difficult airways and could be chosen as first line option in such situations.

Abstract The article presents a case with diagnosed Hodgkin disease (HD) during pregnancy. The aim of this case study was to present diagnostic possibilities in determining HD stage during pregnancy and therapeutic dilemmas. The incidence of HD during pregnancy is 3.2% of all cases with this malignant hematological disorder. The treatment of this disease during pregnancy depends on disease-related factors, pregnancy-related factors, as well as possible implications for fetal morbidity and mortality. The need of analysis of the nature of the disease during pregnancy indicates examination of a larger series of pregnant women with HD and the drawn conclusions affect the decision whether chemotherapy treatment should start immediately or it should be postponed for after delivery.
