INTRODUCTION: Iatrogenic perforations are one of the most exasperating complications of root canal treatment. The prognosis of the root with iatrogenic perforation depends on the location and the procedures undertaken to manage the problem. This case report gives an account of a lateral perforation on a canine tooth and the management strategy under the circumstances. It also gives an overview of some of possible approaches to prevent iatrogenic perforations. METHODS: A right maxillary canine which had an iatrogenic perforation was carefully re-treated and the defect in the bone was exposed using a full thickness mucoperiosteal flap and packed with bone graft material A clinical re-evaluation was done at the end of 3 months. RESULTS: After 3 months the tooth was asymptomatic. There was no tenderness on palpation and on percussion. CONCLUSION: A thorough knowledge of the anatomy of the tooth, combined with the use of appropriate techniques can help in reducing the complications that may occur during endodontic therapy. However, if a problem does occur, a scrupulous management would help in salvaging the involved tooth.

Background. Retroperitoneal ectopic pregnancy is extremely rare. This unusual location represents a great challenge for clinicians due to the difficulties of diagnosis and high risk of life-threatening complications. Case Report. We report the case of a spontaneous early pregnancy of undetermined location in a patient with a history of previous laparoscopic surgery. Diagnosis steps using clinical examination, ultrasound, and magnetic resonance imaging led to the localization of the pregnancy, in the left side of the para-aortic region, in the retroperitoneal space. Conclusion. Retroperitoneal ectopic pregnancy is an uncommon entity with rather complex pathogenesis. Clinicians should carefully interpret clinical signs, biological findings, and imaging features and be aware of unusual locations such as the retroperitoneum for ectopic pregnancies. Early diagnosis and appropriate management strategy are conditio sine qua non for successful treatment outcomes.

We treated a 65-year-old Japanese man with a giant penile lymphedema due to chronic penile strangulation with a rubber band. He was referred to our hospital with progressive penile swelling that had developed over a period of 2 years from chronic use of a rubber band placed around the penile base for prevention of urinary incontinence. Under a diagnosis of giant penile lymphedema, we performed resection of abnormal penile skin weighing 4.8 kg, followed by a penile plasty procedure. To the best of our knowledge, this is only the seventh report of such a case worldwide, with the present giant penile lymphedema the most reported.
