How an Actor Network Theory (ANT) Analysis Can Help Us to Understand the Personally Controlled Electronic Health Record (PCEHR) in Australia

Australia has designed, developed, and now implemented its national e-health solution known as the Personally Controlled Electronic Healthcare Record (PCEHR). This is a unique system as it subscribes to a shared governance model between patients and providers. To date, though, as with other national e-health solutions, there is poor uptake and much concern regarding the success of this multi-million dollar project. The authors contend that while these implementations and adoptions of e-health solutions are necessary, it is essential that an appropriate lens of analysis should be used in order to maximise and sustain the benefits of Information Systems/Information Technology (IS/IT) in healthcare delivery. Hence, in this chapter, the authors proffer Actor-Network Theory (ANT) as an appropriate lens to evaluate these various e-health solutions and illustrate in the context of the Personally Controlled Electronic Health Record (PCEHR), the chosen e-health solution for Australia.

Without exception these reforms include the implementation of a variety of e-health solutions. Such e-health solutions are complex and have far reaching implications. In this paper, the authors argue that while these implementations and adoptions of e-health solutions are necessary, it is essential that an appropriate lens of analysis should be used to maximise and sustain the benefits of IS/IT (information systems/information technology) in healthcare delivery. Background: Despite worldwide expanding implementation of electronic health record (EHR) systems, healthcare professionals conducted limited number of studies to explore factors that might facilitate or jeopardize using these systems. This study underscores the impact of nurses’ opinions, perceptions, and computer competencies on their attitudes toward using an EHR system.Methods: With randomized sampling, a cross-sectional exploratory design was used. The sample consisted of 169 nurses who worked at a public teaching hospital in Oman. They completed self-administered questionnaire. Several standardized valid and reliable instruments were utilized.Results: Seventy-four percent of our study nurses had high positive attitudes toward the EHR system. The least ranked perception scores (60.4%) were linked to perceiving that suggestions made by nurses about the system would be taken into account. Nurses who reported that the hospital sought for suggestions for customization of the system [OR: 2.54 (95% CI: 1.09, 5.88), p = .03], who found the system as an easy-to-use clinical information system [OR: 6.53 (95% CI: 1.72, 24.75), p = .01], who reported the presence of good relationship with the system’s managing personnel [OR: 3.59 (95% CI: 1.13, 11.36), p = .03] and who reported that the system provided all needed health information [OR: 2.97 (95% CI: 1.16, 7.62), p = .02] were more likely to develop high positive attitudes toward the system.Conclusions: To better develop plans to foster the EHR system’s use facilitators and overcome its usage barriers by nursing professionals, more involvement of nurses in system’s customization endeavors is highly suggested. When the system did not disrupt workflows, it would decrease clinical errors and expand nursing productivity. In order to maximize the utilization of the system in healthcare delivery, future research work to investigate the effect of the system on other healthcare providers and inter-professional communications is pressingly needed.

The purpose of this research is to demonstrate the efficiency of the Electronic Health Record (EHR) software that is adopted in the healthcare industry to provide better patient care. The authors examine the impact of EHRs on the efficient delivery of healthcare services. More specifically, they detail the origin of EHR, its significance in modern healthcare delivery along with the selection and implementation criteria for EHR software. They present a survey on the extent of adoption of EHR by clinicians. They also highlight the challenges and barriers faced by organizations in adopting EHR software such as cost, workflow impact and data security. Finally, the authors contemplate the future of EHR, its role in the implementation of health information exchange and its implementation in the cloud. They conclude that the implementation of EHR in the cloud is an important step towards better health management across the population with the end-goal of better health outcomes.

Healthcare delivery continues to be challenged in all OECD countries. To address these challenges, most are turning their attention to e-health as the panacea. Indeed, it is true that in today's global and networked world, e-health should be the answer for ensuring pertinent information, relevant data, and germane knowledge anywhere anytime so that clinicians can deliver superior healthcare. Sadly, healthcare has yet to realize the full potential of e-health, which is in stark contrast to other e-business initiatives such as e-government and e-education, e-finance, or e-commerce. This chapter asserts that it is only by embracing a rich theoretical lens of analysis that the full potential of e-health can be harnessed, and thus, it proffers Actor-Network Theory (ANT) as such a lens.

Background and Hypothesis:  In the US today, over 95% of healthcare institutions operate using the electronic health record (EHR). While proven to be a substantial improvement to medical practice, the substantial amount of retained information within those records has made searching the EHR for relevant material difficult and too time consuming. We hypothesize that by providing a search function within the EHR with added capability of collaborative filtration, physicians will be better able to retrieve important patient information and thus provide more efficient care.     Project Methods:   Emergency Department physicians of Sidney & Lois Eskenazi Hospital and Indiana University Health Hospital were recruited to partake in this study based on their use and familiarity of the EHR Cerner and/or Health Information Exchange (HIE) CareWeb Search function. Participants filled out a pre-interview, Likert-scale questionnaire to determine their general impressions of search functions and the frequency with which they were used. Additional insight was obtained during an interview focusing on participants’ previous experiences searching within the EHR/HIE. Participants were then shown a mock-up of potential collaborative filtering integration into CareWeb in order to collect opinions regarding the feature’s usability/practicality, display/format, and a number of suggested terms.    Results:   From the pilot study, current challenges that limit clinician search function use include limited time in clinician workflow, information overload, and inaccurate results. Clinicians are more likely to conduct searches when treating patients who have limited medical history, complex histories, known recent visitations, and/or who have been seen at other institutions. Participants demonstrated interest in a collaborative filtration search feature; they expressed a preference to have the feature recommend five related search terms.    Potential Impact:   The data from this study aims to refine the way healthcare providers search within the EHR/HIE. This will allow healthcare providers to more efficiently extract relevant patient information for improved healthcare delivery and proficient clinician workflow.

Globally in healthcare, the focus is on designing and implementing national e-health solutions in an attempt to address key challenges that are plaguing healthcare delivery. However, despite the initial euphoria and notwithstanding the significant investments made, to date, many of these e-health solutions have yet to prove their success or have been complete failures. This paper presents the findings from an exploratory study that examined e-health initiatives in five countries Australia, China, Germany, UK and US to understand why these e-health solutions have not as yet delivered the promised results. The paper proffers Actor-Network Theory (ANT) as an appropriately rich theoretical lens that can be used to assist in the understanding of key issues for successful e-health solutions.

INTRODUCTION:Since the adoption of electronic health record (EHR) systems, which contain large volumes of aggregated longitudinal clinical data, promises a number of substantial benefits including better care, improved safety issues and decreased healthcare costs (1). It is also associated with significant costs and large technical and organizational impacts, therefore it is important to conduct comprehensive evaluations of healthcare delivery outcomes. The purpose of the study is to gather evidence on safety and overall effectiveness of EHR implementation in Bambino Gesù Children's Hospital (OPBG).METHODS:Decision-oriented HTA (DoHTA) method (2) was applied to assess the technology on clinical, technical, organizational, economic, legal, ethical and safety domains. It's a new implementation of the European Network for Health Technology Assessment (EUnetHTA) CoreModel integrated with the Analytic Hierarchy Process. It allows defining an evaluation structure represented by a hierarchical decision tree filled by indicators of technology's performances, each of which was given a weight proportional to the impact that this criterion provides to achieve the purpose of the decision problem; finally, the alternatives’ ranking was defined.RESULTS:The multidisciplinary assessment took into consideration all of the aspects and recommendations about the benefits and disadvantages of EHR (3). The synthesis of scientific evidence integrated with results of the specific context analysis, resulted in the definition of components of the decisional hierarchy structure. In particular, EHR seems to offer many benefits in terms of safety and clinical effectiveness such as improved continuity and quality of care, and increased accessibility of the data. The implementation of EHR resulted in important organizational outcome such as EHR configuration, learning curve and training. For these reasons, the usability was the main technical characteristics of the technology taken into account. Finally, legal aspects on privacy and security of data, covered a key role in the assessment.CONCLUSIONS:A thorough evaluation of the EHR before its implementation has permitted hospital's decision makers to choose knowingly.
