The unified modelling language (UML) comprises a set of tools for documenting the analysis of a system. Although UML is generally used to describe and evaluate the functioning of complex systems, the extent of its application to the health care domain is unknown. The purpose of this article is to survey the literature on the application of UML tools to the analysis and modelling of health care systems. We first introduce four of the most common UML diagrammatic tools, namely use case, activity, state, and class diagrams. We use a simplified surgical care service as an example to illustrate the concepts and notation of each diagrammatic tool. We then present the results of the literature survey on the application of UML tools in health care. The survey revealed that although UML tools have been employed in modelling different aspects of health care systems, there is little systematic evidence of the benefits.

The time between detection and response to chronic diseases could go a long way in saving lives. The current trend in health monitoring systems is to move from the hospital centered device to eventually portable personal devices. Hence, Telemedical health care involves the remote delivery of medical care service to either out-of-hospital or admitted patients through wireless network and computer information technology. This paper systematically reviews the most recent works in telemedical health care system to propose a more efficient model. The focus is more on wearable sensors and devices with most attention given to cardiovascular patients in recent times. The huge literature available reflects the size of activity and attention given to telemedicine. The reviewed works are published within the last five years. Furthermore, the proposed systems are compared in terms of their connectivity, targeted application, type of sensor used, etc. Our study reveals Telemedicine to be a profound field with researchers from multidisciplinary sectors. However, there are still many gaps that need to be filled before maturity. Factors such as efficient wireless transmission, cyber data security, sensor design and integration, device miniaturization and intelligent algorithm for multi parameter data fusion require further considerations.

Australian Health Review has featured the Models of Care section for its third year. The aim of the section is to highlight the vast issues that impact various 'models of care', expose the breadth and depth of the models and their impact and increase the profile of quality-based articles. As interested stakeholders, we are continually exposed to the latest models of care claiming to be a solution for some of the health care system's weaknesses ? namely, issues of equity, accessibility, quality and affordability. Individuals and populations across the globe are touched by these matters, and health care systems grapple with ways in which to improve such weaknesses. In previous Editorials, I have covered a range of issues relating to models of care including: Functions (ie, assessment, costing, planning, implementing, client advocacy, monitoring, evaluating); Evidence; Information management and use of technology; Stakeholder interest (ie, government, health fund, provider management, health professionals, clients); and Policy impact. The reality is that the core issues mentioned above (access, equity, etc) are always going to be around, and any models of care claiming to be breakthrough solutions to health care service issues are hoaxes. So, it is no wonder that prescriptive models of care are often ineffective. Taking a broader view of the multitude of issues that impact the care process and eventual outcomes is more sensible.
