ABSTRACT Aim The aim of this study was to assess the association of dental caries and dental anomalies among 5- to 15-year-old children with cleft lip and cleft palate. Materials and methods A cross-sectional study was conducted among 500 children with cleft lip and/or cleft palate and 500 children without cleft of the age group 5 to 15 years. The clinical examination was carried out using decayed, missing, and filled teeth (DMFT) by Klein Plamer and Knutson, and dmft by Grubbel and 1997 World Health Organization (WHO) pro forma. Results The results showed a statistically significant increase in the prevalence of dental caries in children with cleft lip and/or cleft palate. The mean DMFT was found to be significantly higher in operated children than in the children who were not operated. Other dental abnormalities included an increased frequency of enamel hypoplasia (p < 0.001), hyperdontia (p < 0.014), anterior, unilateral and bilateral cross-bite (p < 0.001), and open bite (p < 0.001). Conclusion The results of this study show that children with cleft have higher prevalence of dental anomalies than the normal children. With increased occurrence of hypoplasia in children with cleft, reduced access for cleaning upper anterior teeth after surgical repair leads to poor oral hygiene status, leading to increased risk to dental caries. Clinical significance Given the high caries experience among the children with cleft lip and cleft palate, it is necessary to advocate a more rigorous approach to the prevention of dental disease in these high-risk children. They should therefore, be subjected to regular checkups, oral hygiene advice, diet advice, appropriate fluoride supplementation, and, when required, appropriate referral for secondary care. ABSTRACT Fluoridated dentifrices reduce dental caries in subjects who perform effective oral hygiene. Actinomyces naeslundii increases in teeth-adherent microbial biofilms (plaques) in these subjects, and a well-characterized serum immunoglobulin G (IgG) antibody response (Actinomyces antibody [A-Ab]) is also increased. Other studies suggest that a serum IgG antibody response to streptococcal d-alanyl poly(glycerophosphate) (S-Ab) may indicate caries experience associated strongly with gingival health and exposure to fluoridated water. The aim of this study was to investigate relationships between A-Ab response, oral hygiene, S-Ab response, and caries experience. Measurements were made of A-Ab and S-Ab concentrations, caries experience (number of decayed, missing, and filled teeth [DMFT], number of teeth surfaces [DMFS], and number of decayed teeth needing treated [DT]), exposure to fluoridated water (Flu), mean clinical pocket depth (PD; in millimeters), and extent of plaque (PL) and gingival bleeding on probing (BOP). A-Ab concentration, the dependent variable in a multiple regression analysis, increased with S-Ab concentration and decreased with PL and DMFT adjusted for Flu (R 2 = 0.51, P < 0.002). Residual associations with age, DMFS, DT, and BOP were not significant. In addition, an elevated A-Ab response, defined from immunoprecipitation and immunoassay measurements, indicated a significant, 30% reduction in DMFT after adjustment for significant age and Flu covariance (analysis of variance with covariance F statistic = 10.6, P < 0.003; S-Ab response and interactions not significant). Thus, an elevated A-Ab response indicates less caries in subjects performing effective oral hygiene using fluoridated dentifrices. Conversely, a low A-Ab response is suggestive of decreased A. naeslundii binding to saliva-coated apatite and greater caries experience, as reported by others.

AIM: This study was to evaluate saliva parameters of identical and non identical twins with their dental caries and oral hygiene status. MATERIAL AND METHODS: A cross sectional study was conducted to assess the oral health status and investigate the salivary parameters of identical and non–identical twins in Kodhini Village, Kerala. The WHO Oral Health Assessment Form was used to record the oral health status. The Simplified Oral Hygiene Index and Gingival Bleeding Index were also included. Unstimulated saliva was collected. Immunoglobulin A levels in saliva was estimated by Turbidimetric Immunoassay. Salivary Cortisol was analysed using ELISA method. For TAC thiobarbituric acid reactive from Fenton’s reaction. RESULTS: Since the absolute difference of the salivary parameters between the pairs of twins was not symmetrically distributed, we report median and interquartile range to describe these differences. There were no observed significant variations in the above salivary parameters. CONCLUSION: The similarity in their environmental factors as well as perhaps the similar genetic predisposition on salivary factors has led to the similarity in the dental caries experience in these twin children.

Background: The Yi ethnic group is the sixth largest minority in China. The aim of this study was to investigate dental caries status among Yi preschool children in the Yunnan province, China. Method: This cross-sectional study invited 5-year-old Yi children using multistage cluster sampling. Two trained and calibrated dentists examined the children in kindergartens. The children’s dental caries experience was assessed using the decayed, missing, and filled teeth (dmft) index. Visual plaque on an indexed tooth of each six sextants was recorded and the oral hygiene status was assessed using the Visible Plaque Index. Parents completed questionnaires that surveyed their educational attainment. The questionnaires also collected information about the children’s demographics, snacking habits, oral-health-related behaviors and dental visit experiences. The associations between caries experiences and potential factors were analyzed using zero-inflated negative binomial regression. Results: All 452 invited children (249 boys, 55%) participated in this study with a response rate of 100%. Dental caries prevalence rate was 83%. The mean dmft score and decayed teeth score were 5.2 ± 4.4 and 5.1 ± 4.4, respectively. Almost half of the children (n = 211, 47%) had visible plaque on four or more of the six sextants. Most (n = 366, 81%) of the children had not visited a dentist in the prior 12 months. Regression analysis found the children’s caries experience was associated with their dental visit experience. Conclusion: The great majority of the Yi preschool children experienced dental caries and almost all of the cavities were not restored. Their oral hygiene was poor and visible plaque was commonly found on their teeth.

ABSTRACT BACKGROUND Oral health is an important aspect of health for all children, and is more important for children with special health needs. AIM The study was done to assess the dentition status, periodontal status, dentofacial anomalies and treatment needs of differently abled children attending special schools in Chennai. MATERIALS AND METHOD A cross sectional study was conducted among 6 - 18 year old hearing and speech impaired children and visually impaired children in Chennai. A cluster sampling methodology was followed for the selection of study subjects. The final sample size for the study was 624 hearing and speech impaired children and 506 visually impaired children. The data was collected using WHO Assessment Form (WHO 1997) modified. Statistical analysis was done using Pearson chi-square test and Student t test. RESULTS Overall mean dmft/DMFT among differently abled children was (0.58+1.46)/ (0.50+0.93). Dental caries experience was more among hearing and speech impaired children with mean dmft/DMFT being (0.67+1.44) / (0.52+0.95) and it was statistically significant(P<0.05) with visually impaired children in case of dmft. Overall mean DAI was (22.3+5.4)) and malocclusion was predominant among visually impaired children compared to hearing and speech impaired children and it was statistically significant (P<0.001) The periodontal conditions were poorer among visually impaired children with most of them having calculus and it was statistically significant (P<0.05) compared to hearing and speech impaired children. Traumatic injuries were seen more among visually impaired children compared to hearing and speech impaired children and it was statistically very highly significant (P<0.001). Hearing and speech impaired children needed more treatment than the visually impaired children with majority of them needing restorations. CONCLUSION It can be concluded from the study that even though the oral health status and treatment needs of differently abled children is not alarming, a prevention based intervention program directed especially towards dental caries and periodontal health is recommended for these special groups for a better productive life in future.
