<sec><title>Study Design</title><p>Prospective clinical study.</p></sec><sec><title>Purpose</title><p>To translate and validate the Quality of Life Questionnaire of the European Foundation for Osteoporosis (ECOS-16) in patients with osteoporotic vertebral fractures in Iran.</p></sec><sec><title>Overview of Literature</title><p>It is important to assess the psychometric properties of instruments measuring patient-reported outcomes.</p></sec><sec><title>Methods</title><p>The translation was performed using the backward-forward translation method. The final version was generated by consensus among the translators. Every woman who had a T-score of &lt;−2.5 completed ECOS-16. Patients were divided into two study groups according to the World Health Organization's criteria: those with at least one vertebral fracture (surgery group) and those with no fractures (control group). They were asked to respond to the questionnaire at three points in time: preoperative and twice within 1-week interval after surgery assessments (6-month follow-up). The 36-item short-form health survey (SF-36) also was completed. The psychometric properties of the questionnaire were assessed using internal consistency, test-retest reliability, convergent validity, discriminant validity, and responsiveness.</p></sec><sec><title>Results</title><p>Of 137 recruited women, 39 underwent surgery and 98 did not. Analysis of the ECOS-16 scales showed an appropriate reliability with Cronbach's alpha of &gt;0.70 for all scales. Test-retest reliability as indicated by intraclass correlation coefficient was found to be 0.85 (0.68–0.91). Additionally, the correlation of each item with its hypothesized domain of the ECOS-16 showed acceptable results, suggesting that the items had a substantial relationship with their own domains. Further analysis also indicated that the questionnaire was responsive to change (effect size, 0.85; standardized response mean, 0.93) (<italic>p</italic>&lt;0.001). Significant correlations existed between scores of similar subscales of ECOS-16 and SF-36 (<italic>p</italic>&lt;0.001).</p></sec><sec><title>Conclusions</title><p>ECOS-16 is an acceptable, reliable, valid, and responsive measure to assess the quality of life in patients with osteoporotic vertebral fractures.</p></sec>

Osteoporosis is a significant public health problem. Vertebral fractures are the most common fracture in patients with osteoporosis, occurring in approximately 750,000 cases each year. The fractures may cause acute or chronic pain, reduce the quality of life, and shorten life expectancy. Several medications are available that reduce the risk of fracture. Vertebroplasty and kyphoplasty (balloon-inflated expansion of collapsed vertebrae followed by injection of bone cement) may reduce or relieve pain in selected patients. Although surgery is rarely necessary for the management of osteoporotic vertebral fractures, it may be indicated for other reasons. No studies have been conducted to determine if the outcome of spinal fusion is different in patients with osteoporosis and, if it is, whether management of the patient's osteoporosis will improve the outcome.

Abstract Background. This study aims to examine the validity of European quality of life-5 dimensions, 3-level version (EQ-5D-3L) on patients with osteoporotic vertebral fractures and compare health related quality of life (QOL) of these patients with them of general population in Korea. Methods. Totally, 163 patients with osteoporotic fractures in any vertebrae from outpatient clinics of multiple hospitals. After informed consents, study participants administered general and clinical information including sex, age, fracture site, and fracture age. Then they filled up several questionnaires such as EQ-5D-3L, 12-item short form health survey version 2.0 (SF-12v2), Roland-Morris disability questionnaire (RMQ) and Pain numeric rating scale (PAIN NRS). To examine the discriminant and convergent validity of EQ-5D-3L, Spearman's correlation analyses among the questionnaires were conducted. Also, PAIN NRS, physical component summary (PCS) and mental component summary (MCS) from SF-12v2 and RMQ scores were compared on problems reporting of each dimension of EQ-5D-3L. Finally, results in EQ-5D-3L from study participants were compared with those things from the comparison group of general populations which were match-sampled according to age and sex from the 2013 Korean National Health and Nutritional Examination Survey. Results. Spearman's correlation coefficients among EQ-5D-3L and PAIN NRS, SF-12v2, and RMQ were mostly statistically significant. The coefficients between the relevant dimensions such as mobility of EQ-5D-3L and physical functioning of SF-12v2were higher than coefficients between the irrelevant ones. Study participants reporting problems in each dimension of EQ-5D-3L showed lower scores on PAIN NRS, PCS and MCS from SF-12v2 and RMQ scores and those were statistically significant. For comparisons with the general populations, problems reporting in the patient group were statistically higher than those things from the general people. Conclusions. This study showed that EQ-5D-3L could be valid to measure a health related QOL of patients with osteoporotic vertebral fractures in Korea.
