The article considers a clinical case of treatment of one of the variants of myofascial pain syndrome – piriformis syndrome without signs of sciatic nerve neuropathy. The peculiarity of the case is the comorbidity of the opioid syndrome with diabetic sensorimotor polyneuropathy and osteoporosis, which required the appointment of complex therapy. The diagnosis was confirmed by additional research methods: spondylography, MRI of the lumbosacral spine, ultrasound of the piriformis muscle, electroneuromyography. A patient management tactic was chosen based on federal clinical guidelines for the treatment of patients with nonspecific back pain. Treatment included non-medicinal and medicinal methods. In order to relieve pain, dexketoprofen was prescribed 2 ml intramuscularly per 2 ml of 0.5% lidocaine solution – 1 time а day No. 2 – under navigation by ultrasound. Subsequently, the transfer was made to oral administration of 25 mg 3 times а day for 3 days. A step-by-step scheme of prescribing dexketoprofen: its introduction into the piriformis muscle with subsequent transfer to oral administration allowed to significantly reduce the severity of pain after 5 days of treatment. The complex effect on the spasmodic piriformis muscle with the help of a tableted muscle relaxant in combination with postisometric relaxation made it possible to quickly stop the pain syndrome and prevent its chronization. The administration of the preparation of thioctic acid pursued two goals: to improve the metabolism of the spasmodic muscle and restore the energy metabolism of peripheral nerves. As a result of the use of complex, pathogenetically based therapy, a positive effect was achieved.

Abstract Background Patrinia scabra Bunge is a well-known herbal medicine for its favorable treatment on inflammatory diseases owing to its effective ingredients, in which iridoid glycoside plays an extremely significant role. This article aimed to improve the content of total iridoid glycosides in crude extract through a series optimization of extraction procedure. Moreover, considering that both pain and inflammation are two correlated responses triggered in response to injury, irritants or pathogen, the article investigated the anti-inflammatory and analgesic activities of P. scabra to screen out the active fraction. Method P. scabra was extracted by ultrasonic-microwave synergistic extraction (UMSE) to obtain total iridoid glycosides (PSI), during which a series of conditions were investigated based on single-factor experiments. The extraction process was further optimized by a reliable statistical method of response surface methodology (RSM). The elution fractions of P. scabra extract were prepared by macroporous resin column chromatography. Through the various animal experiment including acetic acid-induced writhing test, formalin induced licking and flinching, carrageenan-induced mice paw oedema test and xylene-induced ear edema in mice, the active fractions with favorable analgesic and anti-inflammatory effect were reasonably screen out. Results The content of PSI could reach up to 81.42 ± 0.31 mg/g under the optimum conditions as follows: ethanol concentration of 52%, material-to-liquid ratio of 1:18 g/mL, microwave power at 610 W and extraction time of 45 min. After gradient elution by the macroporous resin, the content of PSI increased significantly. Compared with other concentrations of elution liquid, the content of PSI in 30 and 50% ethanol eluate was increased to reach 497.65 and 506.90 mg/g, respectively. Owing to the pharmacology experiment, it was reasonably revealed that 30 and 50% ethanol elution fractions of P. scabra could relieve pain centrally and peripherally, exhibiting good analgesic and anti-inflammatory activities. Conclusion Patrinia scabra possessed rich iridoids and exhibited significant analgesic and anti-inflammatory activities.

Background: Physician review websites have influence on a patient’s selection of a provider. Written reviews are subjective and difficult to quantitatively analyze. Sentiment analysis of writing can quantitatively assess surgeon reviews to provide actionable feedback for surgeons to improve practice. The objective of this study is to quantitatively analyze large subset of written reviews of hand surgeons using sentiment analysis and report unbiased trends in words used to describe the reviewed surgeons and biases associated with surgeon demographic factors. Methods: Online written and star-rating reviews of hand surgeons were obtained from healthgrades.com and webmd.com . A sentiment analysis package was used to calculate compound scores of all reviews. Mann-Whitney U tests were performed to determine the relationship between demographic variables and average sentiment score of written reviews. Positive and negative word and word-pair frequency analysis was also performed. Results: A total of 786 hand surgeons’ reviews were analyzed. Analysis showed a significant relationship between the sentiment scores and overall average star-rated reviews ( r2 = 0.604, P ≤ .01). There was no significant difference in review sentiment by provider sex; however, surgeons aged 50 years and younger had more positive reviews than older ( P < .01). The most frequently used bigrams used to describe top-rated surgeons were associated with good bedside manner and efficient pain management, whereas those with the worst reviews are often characterized as rude and unable to relieve pain. Conclusions: This study provides insight into both demographic and behavioral factors contributing to positive reviews and reinforces the importance of pain expectation management.

Introduction: Chronic neck pain might lead to a change in muscle tissue fibres. Neck discomfort is a serious and prevalent sickness. It is common in the general population, and it frequently results in severe impairment. These alterations have an impact on the cervical spine's capacity to govern 3-D movement efficiently. A range of manual therapy approaches are available to assist relieve pain and impairment while also cervical spine range of motion enhancement and everyday mobility. More evidence for Muscle Energy Technique (MET) in treating such a condition was found. This case report describes muscular energy technique's effect on a patient who has been suffering from neck pain for a long time. Case Presentation: At the previous three months, a 27-year-old female accountant in a hospital with extended periods of sitting while working on a computer presented to our facility with neck pain. Increase in pain with looking up. Discussion: The therapy was well embraced through this patient to muscle energy technique resulting in a high degree of flexibility in the cervical spine, reduce pain and improves flexibility and strength. Conclusion: Physiotherapy has a significant effect Pain, strength, and range of motion are all factors to consider. The findings of this case study indicate that a specific muscle energy technique It's possible that a routine will help to alleviate the problem of neck pain, improves strength and functional ability.

Review question / Objective: The purpose of this systematic review is to accurately evaluate the efficacy of kinesio tape for ankle sprains. Condition being studied: Ankle sprain may cause a series of problems, such as pain, swelling, and various dysfunctions. As a physical therapy method that has attracted much attention in recent years, kinesio tape has been proven to relieve pain, reduce swelling and promote functional recovery for some diseases. In this way, we can provide a reference for clinical physical therapy. The objective was to describe and recommend the most effective combination of Natural-Based Product (NBP) and mucoadhesive for Recurrent Aphthous Stomatitis (RAS) treatment. This systematic review writing was based on PRISMA guidelines. The articles published in the last 10 y were selected using PubMed and Google Scholar database carried out during May 2021. The keywords were: natural-based product, mucoadhesive, and Recurrent Aphthous Stomatitis. The risk of bias was assessed using the Oxford Quality Scoring System. Six articles of Randomized Controlled Trial were selected. The NBP were: Aloe vera, Myrrh, Curcuma longa, propolis, ginger, Punica granatum flower, and sesame oil. The drug’s formulation was: gel, film, and spray. The mucoadhesive polymers as vehicles were Hydroxy Propyl Ethyl Cellulose (HPEC), Hydroxy Propyl Methylcellulose (HPMC), Benzocaine, Tragacanth Gum, Carbomer 934, Sodium CMC (carboxymethyl cellulose), and chitosan. Curcuma longa 10 mg/g with HPMC was the most effective to relieve pain, while Punica granatum flower extract with Carbomer 934 and Sodium CMC was the most effective to reduce the ulcer size in RAS. Both of the formulations were in gel form.

The study aim was to explore the effects of multisensory breastmilk interventions on short-term pain of infants during newborn screening. This is a randomized controlled trial. A total of 120 newborns were recruited and assigned by randomization to one of three treatment conditions: Condition 1 = routine care (gentle touch + verbal comfort); Condition 2 = breastmilk odor + routine care; or Condition 3 = breastmilk odor + taste + routine care. Pain was scored with the Neonatal Infant Pain Scale (NIPS). Data were collected from video recordings at 1 min intervals over the 11 phases of heel sticks: phase 1, 5 min before heel stick without stimuli (baseline); phase 2 to phase 6 (during heel stick); and phase 7 to phase 11 (recovery). Generalized estimating equations compared differences in pain scores for newborns over phases among the three conditions. Compared with the routine care, provision of the odor and taste of breastmilk reduce NIPS scores during heel sticks (B = −4.36, SE = 0.45, p < 0.001 [phase6]), and during recovery (B = −3.29, SE = 0.42, p < 0.001 [phase7]). Our findings provide new data, which supports the use of multisensory interventions that include breastmilk odor and taste in combination with gentle touch and verbal comfort to relieve pain in infants undergoing newborn screening.

ABSTRAKPijat bayi merupakan upaya meningkatkan kesehatan pada bayi balita pada upaya promotif terutama pada masa pandemi covid19, Pijat merupakan terapi sentuh tertua dan terpopuler yang dikenal. Pijat bertujuan untuk menghilangkan rasa sakit dan mengembalikan tubuh menjadi segar kembali. Sentuhan pijat bayi akan merangsang produksi hormon betha endorprin yang akan membantu mekanisme pertumbuhan dan merangsang produksi hormon oksitosin dan menurunkan produksi hormon kortisol sehingga bayi dan balita menjadi rileks dan tenang sehingga perkembangannya akan lebih optimal. Selain itu pijat memiliki beberapa efek positif dalam hal penambahan berat badan, pola tidur yang lebih baik, peningkatan perkembangan neuromotorik, ikatan emosional yang lebih baik, penurunan tingkat infeksi nosokomial salah satunya common cold. Solusi permasalahan yang ditawarkan yaitu edukasi manfaat pijat bayi untuk meningkatkan kesehatan bayi selama masa pandemia Covid-19. Setelah diberikan edukasi tentang manfaat pijat bayi, ibu balita dibekali modul yang dapat dipelajari di rumah dapat mempraktikan pijat bayi di rumah. Jumlah responden yang mengikuti kegiatan ini sebanyak 11 ibu yang memiliki bayi. Hasil pengabdian didapatkan pengetahuan ibu meningkat tentang pijat bayi yaitu sebanyak 46%. Kata kunci: pijat bayi; balita; pandemi covid19 ABSTRACTBaby massage is an effort to improve the health of infants under five in promotive efforts, especially during the covid19 pandemic. Massage is the oldest and most popular touch therapy known. Massage aims to relieve pain and restore the body to be fresh again. The touch of a baby massage will stimulate the production of beta-endorphins which will help the growth mechanism and stimulate the production of the hormone oxytocin and reduce the production of the hormone cortisol so that babies and toddlers become relaxed and calm so that their development will be more optimal. In addition, massage has several positive effects in terms of weight gain, better sleep patterns, increased neuromotor development, better emotional bonds, decreased levels of nosocomial infections, one of which is the common cold. The solution to the problem offered is education on the benefits of baby massage to improve the baby's health during the Covid-19 pandemic. After being given education about the benefits of baby massage, mothers of toddlers are provided with modules that can be studied at home and can practice baby massage at home. The number of respondents who participated in this activity were 11 mothers who had babies. The results of the service showed that the mother's knowledge increased about infant massage as much as 46%. Keywords: baby massage ;  toddlers; the covid pandemic 19

Патология опорно-двигательного аппарата и остеоартрит в частности остаются частой причиной обращения за медицинской помощью в амбулаторном звене. Среди разных суставов самым страдающим считается коленный – на него приходится до 25% всех поражений опорно-двигательной системы, что связано с его опорной функцией и частым травматическим поражением. Рост популярности активного отдыха, развитие игровых и экстремальных видов спорта приводят к росту частоты травм коленного сустава, связочного аппарата, а также внутрисуставных повреждений. Именно травматизация признается одной из ведущих причин последующего остеоартрита. Статья предлагает краткие алгоритмы диагностики и терапии остеоартрита для применения на амбулаторно-поликлиническом этапе оказания медицинской помощи. Уделено внимание применению препаратов гиалуроновой кислоты, проведен анализ литературных данных, оценены перспективы применения подобной терапии в комплексе лечебных мероприятий. Гиалуроновая кислота является натуральным компонентом синовиальной жидкости, она служит в качестве смазки суставных поверхностей и обеспечивает ряд защитных и регуляторных эффектов. Вводимая извне гиалуроновая кислота берет на себя нагрузку, восполняя недостающие функции синовиальной жидкости, что и позволяет считать такое применение протезированием недостаточной функции сустава. Препараты гиалуроновой кислоты — быстродействующие локальные средства для улучшения функции пораженного сустава и купирования болевого синдрома, что важно для поддержания активности пациентов. Данные эффекты обусловлены реологическими особенностями гиалуроновой кислоты, которые позволяют не только улучшить смазывающие свойства и скольжение в пораженном суставе, но и обеспечивают защиту суставного хряща от ударных нагрузок. Полученная промышленным способом гиалуроновая кислота уже более 30 лет успешно применяется в лечении остеоартрита. Pathology of the musculoskeletal system and osteoarthritis, in particular, remain a frequent reason for seeking outpatient care. Among the various joints, the knee joint is considered the most affected - it accounts for up to 25% of all lesions of the musculoskeletal system, which is associated with its supporting function and frequent traumatic injury. The growing popularity of outdoor activities, the development of playing and extreme sports leads to an increase in the frequency of injuries of the knee joint, ligamentous apparatus, as well as intra-articular injuries. It is traumatization that is recognized as one of the leading causes of subsequent osteoarthritis. The article offers brief algorithms for the diagnosis and treatment of osteoarthritis for use at the outpatient stage of medical care. Attention is paid to the use of hyaluronic acid preparations, an analysis of the literature data is carried out, the prospects for the use of such therapy in a complex of therapeutic measures are assessed. Hyaluronic acid is a natural component of synovial fluid that serves as a lubricant for articular surfaces and provides a range of protective and regulatory effects. Hyaluronic acid introduced from the outside takes on the load, replenishing the missing functions of the synovial fluid, which allows us to consider such use as prosthetics of insufficient joint function. Hyaluronic acid preparations are a fast-acting local remedy to improve the function of the affected joint and relieve pain, which is important to keep patients active. These effects are due to the rheological features of hyaluronic acid, which not only improve lubricating properties and sliding in the affected joint, but also protect the articular cartilage from shock loads. The industrially obtained hyaluronic xylote has been successfully used in the treatment of osteoarthritis for more than 30 years.

Abstract Background The objective of this study is to report typical clinical and laboratory characteristics of three cases of keratitis caused by Pythium insidiosum in China. Case presentation Three Chinese patients of Han nationality diagnosed with Pythium keratitis from 2017 to 2019 were included. One 45-year-old female and one 55-year-old male were exposed to river water, and one 51-year-old female was burned by ash in the eyes. All of them are of Han ethnicity. Upon slit-lamp examination, subepithelial and superficial stromal opacities were observed in a reticular pattern. After conventional treatment with antifungal agents, the clinical status worsened and therapeutic penetrating keratoplasty was performed. Unfortunately, enucleation was performed to remove all infected tissue and relieve pain. Pythium insidiosum was identified in culture and confirmed by internal transcribed spacer ribosomal RNA gene sequencing analysis. Following the systemic and local antibiotic regimens, the patients were cured ultimately and no regression of infection was observed. Conclusions It is significant for ophthalmologists and microbiologist to be alert to this eye-threatening infection, especially in patients who are resistant to antifungal treatments and with water-related exposure.
