Abstract Background Adolescent fertility is a main indicator of the Sustainable Developmental Goal (SGD) three. Although Sri Lanka is exemplary in maternal health, the utilization of Sexual and Reproductive Health services (SRH) by adolescents is less documented. We describe the hidden burden, associated biological and psychosocial factors and utilization patterns of pre-conceptional services among pregnant adolescents in rural Sri Lanka. Methods The study is based on the baseline assessment of the Rajarata Pregnancy Cohort (RaPCo) in Anuradhapura. Pregnant women newly registered from July to September 2019 were recruited to the study. The period of gestation was confirmed during the second follow-up visit (around 25–28 weeks of gestation) using ultra sound scan data. A history, clinical examination, anthropometric measurements, blood investigations were conducted. Mental health status was assessed using the Edinburgh Postpartum Depression Scale (EPDS). Results Baseline data on gestation was completed by 3,367 pregnant women. Of them, 254 (7.5%) were adolescent pregnancies. Among the primigravida mothers (n = 1037), 22.4% (n = 233) were adolescent pregnancies. Maternal and paternal low education level, being unmarried, and less time since marriage were statistically significant factors associated with adolescent pregnancies (p < 0.05). Contraceptive usage before pregnancy, utilization of pre-conceptional health care services, planning pregnancy and consuming folic acid was significantly low among adolescents (p < 0.001). They also had low body mass index (p < 0.001) and low hemoglobin levels (p = 0.03). Adolescent mothers were less happy of being pregnant (p = 0.006) and had significantly higher levels of anxiety (p = 0.009). Conclusion One fifth of women in their first pregnancy in this study population are adolescents. Nulli-parous adolescents exert poor social stability and compromised physical and mental health effects. The underutilization and/or unavailability of SRH services is clearly associated with adolescent pregnancies.

Abstract Background Ethiopia is one of the Sub-Saharan Africa countries with the lowest modern contraceptive prevalence rate and the highest fertility rate. This study aimed to assess individual and community-level predictors of modern contraceptive use among sexually active rural women in Ethiopia. Data and methods A sample of 9450 sexual active rural women aged 15-49 was extracted from the 15, 683 nationally representative samples of 2016 Ethiopian Demographic and Health Survey (EDHS). Multi-level logistic regression model was considered to identify determinant factors of modern contraceptive use among sexually active rural women in Ethiopia. Result The prevalence of modern contraceptive use among respondents was 20% in rural Ethiopia. Injection (66.35%) was the most common type of modern contraceptive use. In the last full model of the multilevel analysis, individual and community-level factors accounted for 86.69% of the variation in the use of modern contraceptive methods. Secondary and above-educated women (AOR = 1.39, 95%CI: 1.06, 2.81), having 1-4 living children (AOR = 2.70, 95%CI: 2.07, 3.53), rich wealth status (AOR = 2.26, 95%CI: 1.96, 2.60), married women (AOR = 17.31, 95%CI: 10.72, 27.94), having primary educated husband (AOR = 1.45, 95%CI: 1.27, 1.67) and being working husband (AOR = 2.26, 95%CI: 1.96, 2.60) were significantly positively associated with individual-level factors of the use of modern contraceptive methods. Besides, modern contraceptive use was negatively associated with Muslim women (AOR = 0.29, 95%CI: 0.25, 0.33). Compared to the Tigray region, women living in the Afar, Somali, Harari, and Dire Dawa regions had lower use of modern contraceptive methods. Women who had access to mass media (AOR = 1.35, 95%CI: 1.16, 1.57) were more likely to use contraceptives than their counterparts. Conclusion The prevalence of modern contraceptive use among rural women has very low. Both individual and community-level factors were significant predictors of modern contraceptive use. Consequently, the government and other stakeholders need to address educational opportunities; creating awareness about modern contraception and valuable counseling would increase modern contraceptive methods utilization.

Introduction. Women who do not switch to alternate methods after contraceptive discontinuation, for reasons other than the desire to get pregnant or not needing it, are at obvious risk for unplanned pregnancies or unwanted births. This paper examines the factors that influence women to switch from Intrauterine Contraceptive Device (IUCD) to other methods instead of terminating contraceptive usage altogether.Methods. The data used for this study comes from a larger cross-sectional survey conducted in nine (9) randomly selected districts of Sindh and Punjab provinces of Pakistan, during January 2011. Using Stata 11.2, we analyzed data on 333 women, who reported the removal of IUCDs due to reasons other than the desire to get pregnant.Results. We found that 39.9% of the women do not switch to another method of contraception within one month after IUCD discontinuation. Use of contraception before IUCD insertion increases the odds for method switching by 2.26 times after removal. Similarly, postremoval follow-up by community health worker doubles (OR = 2.0) the chances of method switching. Compared with women who received free IUCD service (via voucher scheme), the method switching is 2.01 times higher among women who had paid for IUCD insertion.Conclusion. To increase the likelihood of method switching among IUCD discontinuers this study emphasizes the need for postremoval client counseling, follow-up by healthcare provider, improved choices to a wider range of contraceptives for poor clients, and user satisfaction.

This study was carried out to evaluate the pattern of contraceptive usage in family planning clinics of public hospitals within Ilorin metropolis, Nigeria. It was a retrospective study carried out between January 2010 and December 2012 involving a total of 10,002 contraceptive users attended family planning clinics in the centers within this period. Over a half of the clients (56.6%) were within the age bracket of 30-39 years, and 1.4% were less than 20 years. Majority (79.8%) of the users had two or more children while 0.9% were nulliparas. The most preferred contraceptive method was the hormonal injectable (59.5%); the percentage of users of oral contraceptive pills was 9.6% and utilization of the male condoms was 6.9%. The user rate for intrauterine contraceptive device was 17.3% while it was 6.6% for hormonal implant. Surgical sterilization was utilized by 0.1%. The commonest source of information was from healthcare personnel accounting for 41.9% followed by the mass media (21.6%). There was a low level of acceptance amongst teenagers; the level of acceptance of surgical sterilization methods was also low. There is need to improve on information dissemination regarding contraceptive use, via the internet and social media for teenagers who tend to be more internet-savvy. The number and age of clients were statistically significant factors for patterns of contraceptive usage at various family planning clinics in Ilorin metropolis.Bangladesh Med J. 2015 Sep; 44 (3): 140-145

Purpose The purpose of this paper is to obtain empirical evidence on the impacts of socio-economic and demographic factors on the fertility decisions taken by a common family in developing countries like Pakistan. Also, this study contravenes the conventional orthodoxy of childbirth decisions of a family by enlarging the canvas and conjectures the fundamental nexus amongst female’s education, fertility and contraceptive use. Design/methodology/approach The study is based on micro-level data, obtained from the Pakistan Demographic and Health Survey (2012-2013) which is the third survey carried out in Pakistan. Demographic and socio-economic profiles of 13,558 ever-married women, aged 15-49 years, were randomly selected from Gilgit Baltistan and the four provinces of Pakistan. Three dependent variables are used in empirical analysis i.e. current use of contraceptives, total fertility and cumulative fertility. In order to estimate the probability of contraceptive use maximum likelihood, Probit technique is employed with ordinary least squares on reduced form specifications of total fertility and cumulative fertility models. Findings The empirical results proved the hypotheses that educated females practice more family planning through modern contraceptives which leads to a decrease in total fertility rates. Some significant links among females’ education, contraceptive use and fertility define the quantity – quality trade-off and opportunity cost of time. Evidently, female education provides maturity and awareness of family size which is necessary to take crucial economic decisions. Research limitations/implications The empirical evidence suggests that maximum efforts should be made toward women’s education. The current standard of education in Pakistan is not enough to overcome the long-standing problem of excessive child birth. This could be done with the help of public – private partnership as the measures taken by the government alone are insufficient. The government should initiate some adequate measures such as education and awareness about contraceptive usage at the secondary school level that could be a vivacious step to support fertility reduction. Practical implications The framework used in this study provides a broader intra-household income–expenditure approach. With a smaller family size, the household’s income would be shared among fewer individuals. It is highly probable that parents would be more attentive if they need to look after a few children. That is the best way to progress their children with limited resources. Social implications From the socioeconomic perspectives, educated parents plan the ideal family size which allows them to spend more on their children’s upbringing. Originality/value This study captures the magnitude of fertility decisions with the relevance of the wife’s education because the present practice in Pakistan does not allow higher education for married women. This is why this study could be used as a benchmark for further study in the same area.

Thromboprophylaxis following arthroscopic knee surgery (AKS) is not clear in the literature. The purpose of this study was to present the incidence of symptomatic deep vein thrombosis (DVT) following elective AKS over the age of 40. The secondary purpose was to investigate risk factors associated with venous thromboembolic events (VTEs). Surgical database and outpatient clinic follow-up charts of the patients who underwent AKS for any reason were included in the study. Odds for risk factors such as previous medical history of thrombosis, any family history for clotting disorders, diabetes mellitus (DM), oral contraceptive usage, body mass index, history of malignancy, and smoking were evaluated. The incidence of DVT following AKS significantly increased in the patients older than 40 years who had a previous medical history of VTE, DM, and smoking. A variety of guidelines exist for VTE prophylaxis; however, one should focus on risk factors related to the patient’s medical history and current medical conditions. In this study, smoking, DM, and previous history of DVT increased DVT risk significantly, and thromboprophylaxis should be kept in mind for these patients.
