Esthetic Dentistry in Patients with Bilaterally Missing Maxillary Lateral Incisors: A Multidisciplinary Case Report

ABSTRACT Congenitally missing maxillary lateral incisors pose a difficult esthetic and treatment planning problem for clinicians. Age, location, space limitations, alveolar ridge deficiencies, uneven gingival margins, occlusion and periodontal factors often necessitate an interdisciplinary approach. Over the last several decades, dentistry has focused various treatment modalities for replacement of missing teeth. Treatment plans for patients with missing maxillary lateral incisors have traditionally included either space closure or space reopening. To remove healthy tooth structure of adjacent teeth to replace a tooth, for some patients and dentists, is a very aggressive treatment option which eliminates the option of three unit bridges. Removable partial dentures are also not preferred for its bulkiness and reduced esthetics. Presently, the single implant supported crown is a predictable method of tooth replacement in adolescents. The aim of this case report is to provide a conservative method for the management of bilateral missing lateral incisor. Post endodontic restoration in cases of grossly destructed teeth are difficult as the available tooth structure is less, margins are subgingival and of compromised prognosis because the restoration might encroach upon the periodontium affecting its status to deteriorate. Interdisciplinary approach using forced eruption or Orthodontic extrusion along with minimal surgical crown lengthening can give adequate supragingival tooth surface for proper restoration and protect its root support from getting violated. This is the case report of an unsalvageable tooth that is restored to satisfactory functional and esthetic restoration utilizing interdisciplinary approach.

Dentists have several treatment options when restoring patients with congenitally missing maxillary lateral incisors. Endosseous implant therapy combined with orthodontics can achieve predictable outcomes while meeting patient expectations. This case report exhibits an interdisciplinary approach involving orthodontics, implant surgery, and prosthodontics to resolve this common dental concern.

SummaryBackground/Aim: A problem of congenitally missing lateral incisors is frequently encountered in dentistry, with several available treatment modalities, the choice depending on each case. Case Report: A young female patient with bilateral missing lateral incisors was in need for dental treatment for esthetics. She had spacing among the upper anterior teeth with class I molar relationship. Orthodontic space creation was carried out followed by two-piece mini dental implant placement. Two-stage protocol was followed. After osseointegration, implants were exposed. Healing collars were installed to allow mucosal healing. Closed tray implant level impression was taken. Zirconium crowns were chosen for optimum esthetic results. Conclusions: Mini-implants can be used successfully for restoring congenitally missing lateral incisors after space opening. Esthetic results can be enhanced using Zirconium crowns

AbstractLoss of tooth structure in the esthetic region is a big challenge for restorative dentist as it not only involves the establishment of tooth structure but also complies microesthetics. If the tooth structure is lost near the gingival margin, then the periodontal aspect cannot be overlooked. Surgical crown lengthening and orthodontic extrusion of the remaining tooth structure are two possible treatment modalities for such cases. Which one is best suited depends on the indications for each procedure. This study reports an orthodontic extrusion for the restoration of fractured upper canine. Preference of crown lengthening by orthodontic extrusion over surgical crown lengthening is clarified along the case discussion.

Introduction: Maxillary canines play strategic roles in maintaining the normal function and visibility of teeth. However, rare cases of bilateral permanent congenitally missing maxillary canines were occurring. Replacing these missing canines with dental implant prostheses is an optimal treatment plan for aesthetics and functions. Case Report: In this case report, we described the replacement of bilateral maxillary canines with osseointegrated implants to support all-ceramic prostheses for a young female patient. This treatment avoids the conventional preparation of adjacent teeth as a part of prosthetic reconstructions. In addition to that, all-ceramic crowns are biocompatible with oral cavity structures. Conclusion: The stable short-term result has been achieved with the replacement of the bilateral congenitally missed canines with an implant and cemented all-ceramic crowns, with 3-years cumulative success rate.
