ESPEN 2021 Congress Review: targeted nutritional therapy at all stages of patient recovery

С 9 по 14 сентября 2021 года состоялся ежегодный международный Конгресс ESPEN, организованный Европейской ассоциацией клинического питания и метаболизма (The European Society for Clinical Nutrition and Metabolism). В связи с продолжающейся пандемией COVID-19 этот главный международный форум диетологов и нутрициологов уже второй год проходит в онлайн-формате. В рамках Конгресса по сложившейся традиции обсуждались вопросы, связанные с ролью метаболических процессов в развитии и течении различных заболеваний, а также значимостью проведения целенаправленной нутритивной поддержки как важного компонента в комплексной терапии пациента на разных этапах его лечения и реабилитации. Представленные на Конгрессе данные клинических исследований свидетельствуют о том, что пациенты с COVID-19 по сравнению с больными «нековидного» профиля нуждаются в более активной и целенаправленной нутритивной терапии, которая, в частности, осуществляется с помощью специализированных продуктов – готовых к употреблению смесей для диетического лечебного и профилактического питания. Специализированная нутритивная поддержка способствует ускорению восстановления пациентов в период лечения и реабилитации при различных заболеваниях, в том числе после перенесенной коронавирусной инфекции, и является жизненно необходимой пациентам как во время пребывания в стационаре, так и в процессе дальнейшего восстановления, которое может занять до полугода и даже больше. The annual Congress of ESPEN (The European Society for Clinical Nutrition and Metabolism) was held from September 9 to 14. The Congress was organized by the European Association for Clinical Nutrition and Metabolism. In connection with the COVID-19 pandemic, this main international forum of nutritionists has been held online for the second year. There were traditionally discussed issues related to the role of metabolic processes in the development and course of various diseases, as well as the significance of targeted nutritional support in the complex therapy of the patient at different stages of treatment and rehabilitation. The data of clinical studies presented at the Congress indicate that patients with COVID-19, in comparison with patients with a non-covid profile, need more active nutritional therapy, which, in particular, is carried out with the help of specialized products – ready-to-use mixtures for dietary therapeutic and preventive nutrition. Повышение качества оказания помощи больным новой коронавирусной инфекцией COVID-19 возможно только на основе мультидисциплинарного подхода. Оценка пищевого статуса и при необходимости его коррекция являются важной составляющей комплексных программ лечения и реабилитации таких больных. Значительное число пациентов с COVID-19 составляют люди пожилого и старческого возраста, у которых уже есть признаки нутритивной недостаточности. Нарушения питания и сопутствующая им саркопения – факторы риска неблагоприятного течения COVID-19. Прогрессирование саркопении при COVID-19 объясняют возникновением вирусного миозита, миопатией, спровоцированной цитокинами, и иммобилизацией. Уменьшение мышечной массы ассоциировано с плохим прогнозом заболевания и снижением качества жизни. Коррекция нарушений пищевого статуса больных COVID-19 должна проводиться на всех этапах лечения. Важность проблемы нутритивной поддержки отражена во множестве публикаций, посвященных лечению больных COVID-19. Необходимо заметить, что Европейское общество клинического питания и метаболизма (European Society for Clinical Nutrition and Metabolism, ESPEN) в кратчайшие сроки выпустило рекомендации по нутритивной поддержке больных новой коронавирусной инфекцией, что лишний раз подчеркивает актуальность проблемы. Это стало логичным продолжением рекомендаций по коррекции пищевого статуса больных, находящихся на лечении в отделении реанимации и интенсивной терапии. Аналогичные клинические рекомендации были разработаны в Великобритании, Бразилии и целом ряде других стран. Использование препаратов для перорального дополнительного питания в большинстве случаев позволяет обеспечить потребности пациентов в необходимых нутриентах, уменьшить выраженность саркопении и повысить эффективность реабилитационных мероприятий. Improving the quality of care for patients with the new coronavirus infection COVID-19 is possible only on the basis of a multidisciplinary approach. Evaluation of nutritional status and, if necessary, its correction are an important component of comprehensive treatment and rehabilitation programs for such patients. A significant number of patients with COVID-19 are elderly and senile people who already have signs of nutritional deficiency. Eating disorders and accompanying sarcopenia are risk factors for the adverse course of COVID-19. The progression of sarcopenia in COVID-19 is explained by the occurrence of viral myositis, myopathy provoked by cytokines, and immobilization. Decreased muscle mass is associated with poor disease prognosis and reduced quality of life. Correction of nutritional disorders in patients with COVID-19 should be carried out at all stages of treatment. The importance of the problem of nutritional support is reflected in many publications devoted to the treatment of patients with COVID-19. It should be noted that the European Society for Clinical Nutrition and Metabolism (ESPEN) promptly issued recommendations on nutritional support for patients with new coronavirus infection, which once again emphasizes the urgency of the problem. This was a logical continuation of the recommendations for correcting the nutritional status of patients undergoing treatment in the intensive care unit. Similar clinical guidelines have been developed in the UK, Brazil and a number of other countries. The use of drugs for oral supplementary nutrition in most cases allows to meet the needs of patients for the necessary nutrients, to reduce the severity of sarcopenia and to increase the effectiveness of rehabilitation measures.

Nutritional support is one of the additional treatment methods for the exocrine pancreatic insufficiency consequences in chronic pancreatitis. Nutritional insufficiency risk assessment in these patients, timely diagnosis, and its correction are based on the use of the most optimal methods and treatment schemes. The article presents an overview and analysis of the European Association for Clinical Nutrition and Metabolism (ESPEN) recently published guidelines on nutritional support in chronic pancreatitis

The role of dietitians is to counsel patients on special dietary modifications, develop dietary plans and provide dietary advice for healthy lifestyle in order to promote health and prevent disease. As lifestyle changes have become a primary reason for the increasing prevalence of non-communicable diseases in recent decades, it appears that nutritional therapy should play an important role in the multidisciplinary healthcare system. The aim of the review was to evaluate the role of nutritional support provided by dietitians in the prevention and treatment of chronic diseases in terms of its clinical and cost-effectiveness, according to their range of activity and qualifications. The evaluation was based on the review of the current literature. The costs of the treatment of non-communicable diseases encompass the costs of the treatment of the disease itself, costs of its complications and costs resulting from the decline of the productivity of patients. The results of studies indicate that nutritional support provides not only clinical but also cost benefits in the prevention and treatment of various non-communicable diseases. Every €1 spend on dietary counseling of patients with obesity or obesity-related diseases approximately returns a net €14 to €63 over a period of five years. Dietitians appear to be more effective in counseling overweight or obese patients with present risk factors compared to other specialists, however the barriers to accessing a qualified dietitian and problems with the reimbursement for services still exist. As adequate professional qualifications of dietitians appear to be an important factor determining the effectiveness of implemented dietary care, an additional training courses for dietitians and enhanced cooperation of hospitals and medical universities are needed.

In palliative care, the aims of nutritional support change with an emphasis on quality of life and symptom relief as opposed to active nutritional therapy. The concept of palliative nutrition has evolved to include all aspects of the palliative trajectory, not just end of life care, with each stage of the trajectory having different nutritional goals. The role of the dietitian in palliative care is a topic that until recently remained relatively unexplored. This chapter introduces the concept of diet as a potential survivorship intervention as opposed to nourishment only and contextualizes the dietitian’s role in palliative care.
