ABSTRACTObjective: to investigate the level of knowledge of nursing professionals on the psychiatric reform, related to its technical and scientific training and professional practice. Methods: exploratory and descriptive research with a qualitative approach, to be held in the Residential Therapeutic Service of the Municipal Health Secretariat of Caicó - RN. The study subjects will be the health professionals who are part of the nursing staff (6 aides and nursing technicians and 1 nurse), and the data will be obtained through a semistructured interview, split into three parts (characterization of the professional, professional training and interview guide). The interviews will be recorded in digital audio, respecting the ethical principles in research with humans, and later transcribed in full. The analysis will be mediated by Microsoft Excel Software (characterization and vocational training), and by the theoretical and methodological feature of the Theory of Social Representations, being created thematic categories. Expected results: it is attempted to provide tools for reflection and guidance of health professionals working in the Residential Therapeutic Service, facing the new redesign of business practices, after the Psychiatric Reform. It may also indicate ways to overcome and improve service to all municipal replacement network which provides mental health services. Métodos: pesquisa de natureza exploratória e descritiva, com abordagem qualitativa, que será realizada no Serviço Residencial Terapêutico da Secretaria Municipal de Saúde de Caicó-RN. Os sujeitos de pesquisa serão os profissionais de saúde que integram a equipe de enfermagem (6 auxiliares/técnicos de enfermagem e 1 enfermeiro), e os dados obtidos através de uma entrevista semi-estruturada dividida em III partes (caracterização do profissional, formação do profissional e roteiro de entrevista). As entrevistas serão gravadas em áudio digital, respeitando-se os preceitos éticos na pesquisa com seres humanos, e posteriormente transcritas na íntegra. A análise será mediada pelo Software Microsoft Excel (caracterização e formação profissional), e pelo recurso Teórico-Metodológico da Teoria das Representações Sociais, sendo construídas categorias temáticas. Resultados esperados: intenta-se fornecer subsídios para a reflexão e orientação prática dos profissionais de saúde que atuam no Serviço Residencial Terapêutico, diante do novo redesenhar das práticas profissionais advindas da Reforma Psiquiátrica. Além disso, pode indicar caminhos de superação e melhoria no atendimento a toda rede substitutiva municipal que presta serviços de saúde mental. Método: estudio de investigación descriptiva, con enfoque cualitativo, que se realizará en el Servicio de Terapia Residencial de la Municipal de Salud en Caicó-RN. Los sujetos del estudio serán los profesionales de la salud que constituyen el equipo de enfermería (6 auxiliares/técnicos de enfermería y 1 enfermera), y los datos obtenidos a través de una entrevista semiestructurada dividida en 3 partes (caracterización del profesional, su graduación y la guía de entrevista). Las entrevistas serán grabadas en audio digital, respetando los principios éticos en la investigación con seres humanos, y posteriormente transcritas en su totalidad. El análisis será mediado por el Software Microsoft Excel (caracterización y graduación), y por el recurso teórico y metodológico de la Teoría de las Representaciones Sociales, siendo construidas categorías temáticas. Resultados esperados: se intenta proveer elementos para la reflexión y la orientación de los profesionales de salud que trabajan en el servicio de terapia residencial, frente al nuevo rediseño de las prácticas profesionales derivadas de la Reforma Psiquiátrica. Del mismo modo, eso puede indicar maneras para la superación y mejoramiento de la asistencia a toda la red municipal que presta servicios de salud mental. ABSTRACTObjective: to describe and to identify nurse’ attributions in the mental health services according to the Psychiatric Reform. Methodology: descriptive and exploratory study, from qualitative approach, developed by recent literature review, in which we searched articles from the Virtual Health Library (BVS), and also the collection of Scielo. The analyses were done by pre and selective readings, and also textual and interpretative investigation of the selected texts. Results: it had been found 7544 publications. We limited the choice to those articles related with the nurse’s attributions and the assistance in the Mental Health Services after the Psychiatric Reform. We selected 34 articles. We defined three categories from the readings: Nursing in the Psychiatric Reform, Strategies for the Psychosocial Rehabilitation, Nurse’s Attributions Mental Health Services. We have observed that the more referred articles to the following Mental Health Services: Daily Hospital, Emergence Care, Specific Day Centres, Family Health Program Therapeutic Residential Services.,Conclusion: This research let us realize that most of the characteristics of nursing attributions in the mental health services owns to the replacement of the asylum model for different services such as Psychosocial Attention Center (CAPS), Day-Hospital, Group Homes and basic attention partnership. Os artigos foram analisados após pré-leitura, leitura seletiva, análise textual e interpretativa. Resultados: foram encontradas 7544 publicações. A escolha ficou limitada àqueles artigos que abordavam as atribuições e a assistência do enfermeiro nos Serviços de Saúde Mental após a Reforma. Foram selecionados 34 artigos. A partir da análise textual foram definidas três categorias: Enfermagem na Reforma Psiquiátrica, Estratégias para a Reabilitação Psicossocial e Atribuições do enfermeiro nos serviços de Saúde Mental. Observamos que os Serviços de Saúde Mental mais relatados nos artigos foram: Hospitais-Dia, Unidade de Internação psiquiátrica em Hospital Geral, Centro de Atenção Psicossocial, Programa de Saúde da Família, Serviços Residenciais Terapêuticos. Conclusão: percebemos que as atribuições do enfermeiro nos serviços de saúde mental apresentaram diversas características devido à substituição do modelo manicomial por outros equipamentos como Centro de Atenção Psicossocial, Hospitais-Dia, Serviços Residenciais Terapêuticos e integração com atenção básica. Los artículos fueron analizados con una pre-lectura, lectura selectiva, y el análisis literal e interpretativo de los textos elegidos. Resultados: fueron encontradas 7544 publicaciones. Los resultados quedaron limitados a aquellos artículos  que abordaban las atribuiciones y la assistencia de los  enfermeros en los  Servicios de Salud Mental amen de la Reforma. Fueron selecionados 34 articulos. A partir del análisis textual fueron definidas tres categorias: Enfermeria em la Reforma Psiquiátrica, Estratégias para a Reabilitacion Psicosocial e Atribuiciones del enfermero en los servicios de Salud Mental. Observamos que los Servicios de saúde Mental mas relatados en los artículos fueron: Hospitales-Dia, Unidades de Internacion  psiquiátrica en Hospital General, Centro de Atencion Psicosocial, Programa de Salud de la  Família, Servicios Residenciales Terapeuticos. Conclusión: con esta investigación percibimos que las atribuciones del enfermero presentán diversas características en los servicios de salud mental debido a la substitución del modelo manicomial para los otros equipamientos como el centro de cuidado psicosocial (CAPS), el Hospital-Día, los servicios residenciales terapéuticos (SRT) y atención básica. Purpose The purpose of this paper is to describe the barriers and facilitators to an effective transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS). It also presents a new entry into considering how the transition can be improved. Design/methodology/approach Insights into the transition from CAMHS to AMHS were gathered through eight semi-structured interviews with mental health professionals. Two methods of data analysis were employed to explore the emerging themes in the data and the observed deficit approach to organisational development. Findings The findings identified a vast volume of barriers in comparison to facilitators to the transition. Adolescents who transition from CAMHS to AMHS initially experience difficulty adapting to the differences in the services due to the short duration of the transition period. However, despite the established barriers to the transition, adolescents tend to adapt to the differences between the services. Findings also showed a negative framing towards the transition amongst the mental health professionals which resembles a deficit approach to organisational development. Originality/value This paper explores mental health professionals’ perspectives regarding the transition in Dumfries and Galloway, Scotland. The transition is increasingly recognised as an area in health care that requires improvement. This research provides a new way to consider the transition by exploring the perceived deficit approach to organisational development in the services.

Methods for developing the skillsets necessary for performing forensic consultations in a competent, ethical manner are provided. Information regarding how to effectively market mental health services to legal professionals are reviewed. Various areas of subspecialty practice are described.

Abstract Today's youth are at a disturbingly high risk for mental health and illness problems and are largely dissatisfied with the existing mental health services. Youth Net/Réseau Ado (YN/RA), supported by input from mental health professionals, is a bilingual mental health promotion program that seeks out the opinions and attitudes of youth regarding mental health and illness issues, while connecting them with appropriate resources and mental health services. This paper describes the Youth Net/Réseau Ado program and provides some guidelines for the identification of mental health and illness problems, including indicators of the risk of suicide.

Abstract The number of people detained under the 1983 Mental Health Act has risen significantly in recent years and has recently been the subject of an independent review. Most existing research into the rise in detentions has tended to prioritise the perspectives of psychiatrists and failed to consider the views of Approved Mental Health Professionals (AMHPs), usually social workers, who ultimately determine whether detention is appropriate. This mixed-methods study focused on AMHPs’ views on the reasons behind the rise in detentions and potential solutions. It included a national online survey of AMHPs (n = 160) and semi-structured interviews with six AMHPs within a Community Mental Health Team in England. AMHPs reported that demand for mental health services vastly exceeded supply and, due to inadequate resources, more people were being detained in hospital. AMHPs argued that greater investment in preventative mental health services and ‘low intensity’ support would help to mitigate the impact of social risk factors on mental health; and greater investment in crisis services, including non-medical alternatives to hospital, was required. Such investment at either end of the spectrum was expected to be more effective than changes to the law and lead to better outcomes for mental health service users.

The Royal College of Psychiatrists (1991) has recommended that all local mental health services should include specialist psychotherapy departments. At present these are uncommon outside major teaching centres, although a considerable amount of simple psychotherapy is provided on an ad hoc basis by mental health professionals of various disciplines. This paper describes the structure, functioning and costs of a specialist department in a non-teaching district in the south west of England.

Purpose The purpose of this paper is to examine barriers to accessing mental health services, from the perspective of young people of refugee background who have been service users, and to suggest strategies to improve access to mental health services. Design/methodology/approach A qualitative study was conducted with 16 young people (aged 18-25), who had been refugees and who had attended mental health professionals in Australia. Interview transcripts were analysed thematically to examine participants’ perspectives on what hinders initial access to mental health services. Findings Stigma about mental health problems was particularly prominent. Many believed a high level of disturbance was the threshold for entering services, and for some there was no knowledge of such services’ existence. Options for assistance other than mental health services were often preferred, according to young people’s explanatory models. Apprehension was expressed that sessions would be uncomfortable, distressing or ineffective. The desire to be self-reliant functioned as a further barrier. Finally, structural obstacles and social exclusion deterred some young refugees from accessing services. Practical implications Implications include the need for service providers to be equipped to provide culturally sensitive, responsive services that ideally offer both practical and psychological assistance. Potential referrers, including health professionals and community leaders, could facilitate increased access if trained to recognise and address barriers. Finally, findings indicate potential content for awareness-raising initiatives for young refugees about mental health problems and services. Originality/value This paper is original in its sample, method, topic and findings; being drawn from the first known qualitative research exploring views of young mental health service users who have been refugees about barriers to accessing mental health services.

SummaryObjective - To survey and analyse the extent of utilization of «Non Pharmacological Interventions» (NPI) in the public mental health services of a large city. Setting - Inteview of the entire medical staff employed in 1991 in 10 Departments of Mental Health in the urban area of Naples, Italy. Main measures - A «ad hoc» questionnaire was prepared to gather informations on prevalence of use on NPI in different therapeutic settings, the selected technique according to the different diagnostic categories and the degree of professional training of the interviewed medical personnel. Result - The NPI, as the only treatment, was preferred by the large majority (86%) of the medical staff. The patients treated with NPI was double in outpatient than in inpatient. Although the medical staff did not use the same diagnostic criteria (ICD-9, DSM-III, others) anxiety (49%) and personality disorders (14,3%) were the diagnostic categories most commonly selected for NPI. At the time of the survey 24% of the patients were treated with psychotherapy. Support psychoterapy, psychoanalysis, family therapy were the most frequently employed. The support psychotherapy was mainly used in treatment of schizophrenia. Near to 50% of the medical staff reported a combined use of up to five different NPI. By and large choice of NPI was found dependent on the type of training and inversly correlated with the degree of seniority. Conclusions - The data obteined from the present survey show the large use and variability in NPI employed in public mental health services. The analysis of psycotherapy used suggests a different approach between public and private sector.
