Hormonal therapy has been the treatment axis of metastatic prostate cancer since 1940. Identifying rare forms of debut is important in order to initiate the best treatment as soon as possible. Intravascular disseminated coagulopathy (DIC) related to prostate cancer is mostly presented as chronic forms. Acute forms can also be the way of presentation, recognizing them is crucial due to its life threatening nature. Here, we present a case of debut of metastatic prostate cancer as an acute DIC. Reporting this case is important because of the unfrequently presentation and to consider combined chemohormonal therapy since the diagnosis.

AbstractBowel obstruction is a frequently encountered condition worldwide that causes numerous admissions to hospitals. Metastatic carcinoma has been identified as one of the infrequently encountered causes of bowel obstruction. Prostate cancer typically metastasizes to lymph nodes, bone, lungs, liver, and brain. In this article, we reported the case of a 75-year-old man who presented with bowel obstruction due to narrowing and stricture of the rectum. Primary rectal mass was initially diagnosed, but upon further investigation, it was found that the mass resembled prostate tissue. The bowel obstruction was managed surgically with a colostomy. The patient was later referred to oncology for chemotherapy and hormonal therapy.

Cyproterone acetate (CPA) is an anti-androgenic drug that has been used to manage prostate cancer. The drug is well-tolerated, but has hepatotoxic effects. Hepatic failure induced by CPA is rare and urologists tend to overlook its severity. We report a patient with metastatic prostate cancer who developed CPA-induced hepatic failure that manifested as bilirubinuria, which was initially misinterpreted as gross hematuria. The patient died despite receiving critical care. The aim of this study is to sound the alarm about CPA-induced hepatic failure.
