Hospice and palliative care aim to improve the quality of life for patients and families impacted by chronic, serious, and life-threatening illnesses. Patients and families benefit from earlier integration of palliative care and seamless transitions to hospice care. Nurses are an integral part of the care team, and their understanding of these services is critical. However, many nurses do not receive formal education in hospice and palliative care. This leads to lack of skills that nurses need to provide and discuss hospice and palliative care across care settings. Nurses in an acute care setting of a large health system were invited to participate in a survey, Attitudes Towards Care at the End-of-Life. The results of this survey revealed that nurses believe that patients and families need to be communicated with honestly regarding their disease, but nurses lack the knowledge of how to facilitate these discussions. Another key finding is that nurses feel less confident with non-pharmacologic interventions to relieve patients’ suffering. Education is urgently needed in these areas.

This study aims to examine the effect of a 12-week intradialytic exercise program on patients’ blood indices, physical performance, and quality of life. Forty-six outpatients who were diagnosed with end-stage renal failure by a nephrologist and received hemodialysis therapy for six months or longer were recruited. The data were analyzed using SPSS and Kruskal-Wallis test was employed to analyze the continuous variables. Among blood indices, Hematocrit decreased significantly after aerobic and complex exercise; hemoglobin dropped only after complex exercise; P and Ca×P increased for a considerable amount only after complex exercise, but the postexamination results show no significant difference. As for physical performance, sit-to-stand (STS)-5 decreased with resistance and complex exercise (p < .01), while there was a significant increase in STS-30 after aerobic, resistance, and complex exercise (p < .01). 6-minute walk test (MWT) grew significantly in all groups (p < .01), but alongside gait speed, and grip strength, it diminished significantly in the non-exercise group (p< .05). When it comes to SF-36, there was a significant rise in the physical component score (PCS) (p< .01) with resistance and complex exercise. In the meantime, mental component summary (MCS) went up significantly after all types of exercises (p < .01). In contrast, Beck depression inventory (BDI) showed a significant decline only with complex exercise (p < .01). To sum up, this study suggests intradialytic complex exercise appears to be effective in enhancing Short Form (SF)-36 and BDI as well as physical performance, which is why we recommend this type of complex exercise program for hemodialysis patients.

AbstractObjective:Occupational and physical therapists can have a significant impact on the quality of life of terminally ill cancer patients. In the critical care setting, rehabilitation is often overlooked. However, occupational and physical therapists work with critically-ill patients to create realistic and meaningful goals for improving comfort, mobility, socialization skills, and ability to care for oneself regardless of disease state and medical status. The following case report describes rehabilitation intervention with a young woman diagnosed with osteosarcoma and leukemia during the final stage of her life.Method:This case report highlights the use of patient-centered goals and the importance of close collaboration between the patient, occupational therapist, and physical therapists to achieve a higher quality of life.Results:A collaborative effort by the occupational and physical therapists yielded positive outcomes as defined by the patient, patient family, and the medical staff in the critical care setting.Significance of results:Palliative care patients may benefit from occupational therapy (OT) and physical therapy (PT) intervention. Rehabilitation specialists are skilled at working with patients to set realistic and meaningful functional goals. Further study on rehabilitation treatment to improve quality of life among patients in palliative care is needed.

Background: Intradialytic exercise is an effective intervention to reduce morbidity and mortality and increase quality of life among patients with chronic kidney disease undergoing dialysis. However, implementing and sustaining it in clinical practice has proved challenging. To identify how to best design an effective and sustainable intervention in clinical practice, we aimed to explore hemodialysis patients’ and nurses’ attitudes towards intradialytic exercise, including their motivation, anticipated barriers, and suggestions for the design of a proposed exercise program. Methods: Data were collected through qualitative semistructured interviews with patients and focus group interviews with nurses and analyzed inductively with content analysis. Results: Overall, patients’ and nurses’ attitudes towards intradialytic exercise were positive. Patients were motivated by their expectations about perceived benefits, such as improved quality of life and reduced musculoskeletal pain. Their main concern was triggering dialysis machine alarms and disturbing nurses. Nurses were more skeptical of intradialytic exercise and concerned about patient safety. Patients and nurses had several ideas on how to design a safe and motivating intradialytic exercise intervention. Conclusion: The analysis of patients’ and nurses’ experiences and attitudes generated recommendations for an intradialytic exercise program. Recommendations include individually tailored programs that are safe and that patients can do independently, continuous collaboration between patients, nurses, physicians, and physiotherapists, and educating nurses about the benefits and safety of intradialytic exercise.
