Diagnostic Dilemma of a Double Tooth: A Rare Case Report and Review

AbstractA rare case of monostotic ossifying fibroma of the dorsal spine in a postpartum woman is being described. Ossifying fibromas of the spine are very rare tumors. They are components of benign fibro-osseous lesions. We report a case of an 18-year-old woman, who presented with complaints of weakness of both the lower limbs in the immediate postpartum period and was diagnosed with ossifying fibroma of the dorsal [D3] vertebral body. The tumor was excised followed by spinal stabilization. Histopathologic findings were consistent with ossifying fibroma. Postoperative period was uneventful. Monostotic variant of ossifying fibroma in the dorsal spine is an extremely rare tumor, and its presence in the postpartum period adds on to the diagnostic dilemma. Hence this case report can be kept in mind while evaluating a case presenting with complaints of weakness of both the lower limbs in the immediate postpartum period.

This paper reports a very rare case of isolated ulcerative lesion of the palate in a patient with type-2 diabetes mellitus. The patient refused to run any radiological tests, other than the periapical radiograph. This rendered the diagnostic dilemma more challenging. Through histochemical and immunohistochemical stains, the case could be non-invasively diagnosed as caliberpersistent artery. All differential diagnoses were excluded based on concrete findings.

Occurrence of Sporotrichosis is uncommon in Nepal. Here, we describe a case of cutaneous Sporotrichosis of 43 years old adult male working as a farmer from Pokhara-30, Nepal with unusual skin tuberculosis like presentation since 3 years, initiating from the unusual location in right pinna. Histopathological observation was suggestive of cutaneous tuberculosis, lupus vulgaris, cutaneous leishmaniasis due to overlapping findings during biopsy. The case was diagnosed by the pharmacological intervention observing the effect of drugs i.e., terbinafine. The purpose of reporting this serendipitous case is to enhance timely diagnosis, avoid the diagnostic dilemma for future references.
