Background: Inguinal hernia repair is very common in day to day general surgical practice. Result of surgical repair is often satisfactory but recurrences following surgery are troublesome both for the surgeon and the patient. Lichtenstein technique is now the most widely performed technique in groin hernia repair. Objective: The aim of this study was to assess short term outcome of complete variety of inguinal hernia repair by Lichtenstein technique. Methodology: This was a prospective observational study in which 30 complete variety of inguinal hernia repairs were performed by Lichtenstein technique between January 2014 and December 2017 by same surgical team in surgery department, Holy Family Red Crescent Medical College Hospital (HFRCMCH). Patients were scheduled for follow up visits at the end of first week, third month and sixth month after operation in out-patient department. The main outcome measures were postoperative complications, early recurrence, groin pain. Result: In this study age of the patients ranged from 30 years to 78 years, the mean age was 51.93 (±SD 10.12) years. Most of the hernia were of indirect type 18(60%) followed by direct type 9(30%). In indirect type (55.6%) of the hernias was right sided and the rest (44.4%) were left sided. On an average each operation lasted for 1.18 hours and oral feeding started 12.69 hours after the operation, Postoperative mean hospital stay that was 2.27 days. Postoperative complications were 2(6.7%) had a prolonged recovery and presented with abdominal distention after operation and 3(10%) patients developed postoperative urinary retention. Scrotal haematoma formation was observed in 2(6.7%) cases and other post-operative complications were local haematoma or seroma formation in incision site, periincisional skin echymosis, postoperative pyrexia, superficial wound infection and post-operative pain were found in 1 (3.3%) cases. Regarding outcome no recurrence or mesh rejection or mesh infection were observed within short time postoperative follow up period. Conclusion: In this study no recurrence or mesh rejection or mesh infection were observed within short time postoperative follow up period and patient's compliance was good with minimum morbidity. Therefore, Lichtenstein's technique of inguinal hernia repair considered as a safe and effective procedure. Northern International Medical College Journal Vol.10 (2) Jan 2019: 389-392

Abstract In this paper I argue, from the consideration of what I hope is the complete variety of a hearer’s approaches to a speaker’s utterance, that (1) the speaker’s intention does not settle the meaning of her utterance and (2) the hearer does not take a genuine interest in the speaker’s actual intention. The reason why the speaker’s intention does not settle utterance meaning is simply that no utterance meaning determination, as presupposed by intentionalists and anti-intentionalists alike, takes place. Moreover, in the regular course of interpretation the hearer does not care about the speaker’s actual intention, but only about what the speaker presents as her intention: the hearer’s goal is to come up with an interpretation which the speaker will accept rather than an interpretation which corresponds to the speaker’s intention. In cases of accountability, suspicion and lying, the speaker’s actual intention is irrelevant; it is at most the speaker’s hypothetical intention which is at stake.

Abstract To an algebraic variety equipped with an involution, we associate a cycle class in the modulo two Chow group of its fixed locus. This association is functorial with respect to proper morphisms having a degree and preserving the involutions. Specialising to the exchange involution of the square of a complete variety, we obtain Rost’s degree formula in arbitrary characteristic (this formula was proved by Rost and Merkurjev in characteristic not two).

Polymers are currently used in several industrial applications such as the interior components of the vehicle. We find them for example in the design of dashboards or door panels. These materials may be subjected to different types of load like the temperature variation or high strain rates due to crash impact. It is therefore necessary to well understand and to anticipate the complex mechanical behaviour of the polymers. This paper describes the results of experimental research about the dependence between the strain rate and the temperature. The main objective is to be able to model the complete variety of the behaviour in order to predict the risk of failure under dynamic loadings. The material chosen for this study is a copolymer propylene ethylene mineral (talc) filled 15% impact modified.

Abstract: Nowadays health becomes important, thats why many people even in health body still need medicine or vitamin to protect their body, especially today health body didn’t recognize time. Enterpreneur catch these business opportunity and realize it into a pharmac. An pharmacy in Jenderal Sudirman Street at Purworejo is one example of a response to the opportunity, thus providing a complete variety of types of drugs as well as 24-hour service. However, because of limited land availability, the shophouse became the answer. Shophouses transformed into a place of business that is cultivated good and comfortable. How the application of ergonomics of a pharmacy that located in a shophouse. Through the function conversion of the building, the availability, requirement and quality of the space and supporting things for pharmacy, should remain acommodated properly when used by users for 24 hours each day. This study reached by direvt observation and through inerviews with pharmacy employees as users of the space, so we can know the user needs with a proper ergonomics which adapted to its space.Keywords : Ergonomics, Pharmacy, Quality of Space, 24 HoursAbstrak: Kesehatan saat ini menjadi hal penting, sehingga banyak orang dalam keadaan sehat sekalipun tetap membutuhkan obat ataupun multivitamin guna menjaga stamina agar tetap sehat, terlebih lagi daya tahan tubuh mausia tidak mengenal waktu. Pengusaha menangkap peluang tersebut dan merealisasikannya ke dalam sebuah apotek. Apotek di Jalan Jenderal Sudirman, Purworejo merupakan salah satu contoh respon terhadap peluang baik tersebut, sehingga menyediakan ragam jenis obat yang lengkap serta pelayanan 24 jam. Namun, karena keterbatasan lahan yang ada, maka ruko menjadi jawabannya. Ruko disulap menjadi sebuah tempat usaha yang diusahakan baik dan nyaman. Bagaimana penerapan ergonomi sebuah apotek yang terletak di sebuah ruko. Melalui alih fungsi bangunan, ketersediaan dan kualitas ruang serta pendukung ruang apotek harus tetap terakomodasi dengan baik, saat digunakan oleh penggunanya selama 24 jam setiap harinya. Studi ini ditempuh dengan melakukan observasi langsung serta melalui wawancara dengan karyawan apotek selaku pelaku ruang, sehingga dapat diketahui kebutuhan ruang dengan ergonomi yang tepat dan sesuai dengan wadahnya.Kata Kunci : Ergonomi, Apotek, Kualitas Ruang, 24 Jam

Teissier has proven remarkable inequalities [Formula: see text] for intersection numbers si = (ℒi ⋅ ℳd-i) of a pair of nef line bundles ℒ, ℳ on a d-dimensional complete algebraic variety over a field. He asks if two nef and big line bundles are numerically proportional if the inequalities are all equalities. In this paper, we show that this is true in the most general possible situation, for nef and big line bundles on a proper irreducible scheme over an arbitrary field k. Boucksom, Favre and Jonsson have recently established this result on a complete variety X over an algebraically closed field of characteristic zero. Their proof involves an ingenious extension of the intersection theory on a variety to its Zariski Riemann Manifold. This extension requires the existence of a direct system of nonsingular varieties dominating X. We make use of a simpler intersection theory which does not require resolution of singularities, and extend volume to an arbitrary field and prove its continuous differentiability, extending results of Boucksom, Favre and Jonsson, and of Lazarsfeld and Mustaţă. A goal in this paper is to provide a manuscript which will be accessible to many readers. As such, subtle topological arguments which are required to give a complete proof in [S. Bouksom et al., J. Algebraic Geometry18 (2009) 279–308] have been written out in this manuscript, in the context of our intersection theory, and over arbitrary varieties.

AbstractThe main results of the paper are the following: 1. Every locally finite affine complete variety admits a near unanimity term; 2. A locally finite congruence distributive variety is affine complete if and only if all its algebras with no proper subalgebras are affine complete and the variety is generated by one of such algebras. The first of these results sharpens a result of McKenzie asserting that all locally finite affine complete varieties are congruence distributive. The second one generalizes the result by Kaarli and Pixley that characterizes arithmetical affine complete varieties.
