Abstract Background Malunion or nonunion of vertically displaced pelvic fractures resulting in lower limb length discrepancies, claudication, and pain. There have been few previous reports of this type of corrective surgery of the old pelvis. We present a new surgical technique of sacral osteotomy combined with lumbopelvic distraction osteosynthesis (LPDO) in the treatment of malunion and nonunion of vertically displaced pelvic fractures and report on its short-term clinical results.Methods We retrospectively reviewed nine patients (five males and four females) with malunion or nonunion of vertically displaced pelvic fractures treated with sacral osteotomy and LPDO from April 2015 to January 2020. The age ranged from 14 to 45 years (average, 30.7 years). The time from injury to deformity correction surgery ranged from 3 months to 5 years (average, 12.8 months). The vertical displacement of a unilateral pelvis was 3.0-4.5 cm (average, 3.80 cm). According to the AO/OTA classification at initial pelvic fracture, eight cases were type C1.3 and one case were type C3.3. Sacral osteotomy and LPDO were used in all nine patients. The degree of unilateral pelvic reduction was assessed postoperatively based on measurements from the anteroposterior (AP) X-ray. The Majeed score and pain visual analog scale (VAS) score were used to assess the therapeutic effect of the patients during follow-up.Results In all nine patients, postoperative AP X-ray showed correction displacement of 1.7-3.9 cm (average, 3.20 cm). All the patients were followed up for 6 to 36 months (average, 12.7 months). At the last follow-up, the Majeed score of pelvic fracture increased from an average of 53.9 points (30-84 points) preoperatively to 87.0 points (72-94 points), and the VAS score for pain decreased from an average of 6.0 points (4-8 points) preoperatively to 1.2 points (0-3 points). None of the patients had complications, such as infection, implant broken, screw loosening, iatrogenic nerve, or blood vessel injury. Conclusion Sacral osteotomy combined with LPDO for the treatment of pelvic malunion or nonunion caused by sacral fracture can correct significantly vertical displacement of a unilateral pelvis, prolong limb length, and reconstruct the stability of a pelvic ring, leading to good clinical results.

In an experiment in low-mountain conditions, the features of changes in the structure of bone tissue in the zone of a gunshot fracture of the shank diaphysis were studied. It was revealed that when using the compression-distraction method according to G.A. Ilizarov, a pronounced angiogenic effect is observed, an improvement in reparative processes. The results obtained confirm the need to use the method of extrafocal compression-distraction osteosynthesis to accelerate the healing of gunshot fractures of tubular bones.

Background. Treatment of tibial nonunions is not a fully resolved problem. The use of ring fixators (RF) for this pathology is competitive in modern conditions. However, there are also disadvantages. Improvement of RF treatment technology is a promising direction for improving the results of treatment of this severe pathology. Purpose of work was to improve the results of the treatment of nonunions of the shin bones based on improving the technique of using RF taking into account the problematic issues of their use; to determine their effectiveness. Materials and methods. According to the proposed method, 42 patients with tibial nonunions were treated. Spoke-rod devices were used with the improvements we proposed — sharpening, modes of conducting and arrangement of spokes in a ring at angles in different planes. In the ring, which was used to bring the osteotomized segment down du-ring distraction osteosynthesis, the outward pins were inserted to prevent it from tipping over. Semi-closed osteotomy (minimally traumatic and safe) was performed with fan-shaped with a chisel from a cross section up to 2 cm, with the formation of a comminuted fracture. External fusion Softcast/Scotchcast allowed avoi-ding deformation of the regenerate and creating optimal conditions for the fusion of fragments. Results. The following results were obtained for the treatment of patients with nonunions of the lower leg bones using RF with our proposed modes of use according to the Modified Functional Evaluation System by Karlstrom-Olerud. Good results were obtained in 20 patients — 55.6 %. In 4 patients (11.1 %), the results were assessed as good with elements of mode-rate dysfunction. They were 22.000 ± 1.323 points. Satisfactory results were obtained in 7 patients (19.4 %), estimated at 28.0 ± 0.125 points. Bad, unsatisfactory results were observed in 1 case (2.8 %); excellent functional status — in 4 (11.1 %). Conclusions. Bearing in mind the severity of the lesion, we find this result encouraging. Given the insufficient number of observations, it is necessary to recommend the use of the proposed improvements in the use of RF for the treatment of tibial nonunions after fractures and further study of their effectiveness.

Background. Мeningococcal infection with damage to various organs and systems, including the musculoskeletal system, causes growth plate dysfunction, which usually leads to the formation of orthopedic consequences, including axis deviation and/or limb length discrepancy. Aim. This study aimed to analyze the features of limb deformities and methods for their correction in children with consequences of meningococcemia. Materials and methods. The retrospective analysis was performed on patients with consequences of meningococcemia who were examined and surgically treated in the clinic between 2012 and 2018. A total of 12 patients (six boys and six girls) were included, with an age range of 215 years. The examination included clinical, X-ray, and physiological methods. Treatment methods consisted of a combination of angular deformity correction and limb lengthening. Results. In 12 patients, 76 growth plate arrests of long bones were found. Most frequently (17.1%), growth plate arrests of the distal femur and proximal tibia were observed, which resulted in limb shortening and/or axis deviation. For restoration of limb alignment in 10 (83.3%) patients, transosseous compression-distraction osteosynthesis was performed. For limb deformity correction, guided growth technique was applied by using eight-plate for temporary epiphysiodesis of active functioning part of the growth plate in four (33.3%) patients, whereas partial growth plate arrest resection with following epiphysiodesis was achieved in two (16.6%). Conclusions. Meningococcal septicemia leads to long bone growth plate dysfunction. The main complaints in this patient are limb shortening and their deformity. Along with the transosseous compression-distraction osteosynthesis technique, using the guided growth method by carrying out temporary epiphysiodesis of the remaining functioning part of the growth plate of damaged bone was appropriate.
