An Overview of Mutual Support Groups for Family Caregivers of People with Mental Health Problems: Evidence on Process and Outcomes

BACKGROUND Owing to the rapid development of social networking services, online support groups vary widely both in goal and structure. Several studies have shown the potential effectiveness of online support groups, such as reducing psychological distress (eg depression) among individuals with mental health problems. However, online support groups often do not aim at effectiveness regarding distress-relief-related outcomes. OBJECTIVE The U2plus.jp (hereinafter U2plus) is an online support group for individuals with depression; in it, people support each other in structured ways while engaging in simple cognitive behavioral therapy (CBT) exercises. This study aimed to examine if usage frequency of the U2plus functions are associated with decreased stigma and increased consumer. METHODS In total, 355 U2plus users took part in an online survey. They were asked what therapy they had ever received, how often they logged into it and used each of its functions, and completed the following questionnaires: The Patient Health Questionnaire-9 (PHQ-9), the Perceived Devaluation Discrimination Scale, and the General Help Seeking Scale. RESULTS Regarding the therapy they received, 89.3% (n=308) had been on medication for mental health problems, and 67.5% (n=233) had received psychotherapy or mental health counselling. Regarding the usage frequency, approximately 20% of the participants signed in to U2plus and used its functions more than once a week. The usage frequency of U2plus functions was not associated with perceived stigma. However, usage frequency of some functions was correlated to help seeking intentions from formal sources (eg doctors and psychologists). Moreover, 90% of the participants had a history of medication for their mental health. Additionally, the more depressed participants were, the more frequently they used U2plus. CONCLUSIONS It was suggested that online support groups may serve as an alternative treatment option for those who are already undergoing pharmacological treatment and are willing to seek help from whatever source they deem helpful.

Caring for a person diagnosed with Alzheimer’s disease has a negative impact on family caregivers’ psychological health. This study examined the factors related to ‘perceived health’ and ‘presence of new-onset mental health problems’ in family caregivers of people diagnosed with mild and moderate Alzheimer’s disease. A cross-sectional observational study carried out in Almeria’s Healthcare District (Spain). A total of 255 family caregivers (42.4% cared for people with mild Alzheimer’s disease and 57.6% cared for people with moderate Alzheimer’s disease) participated in the study from January to December 2015. Mainly, caregivers were women (81.5% in the mild Alzheimer’s disease group and 88.4% in the moderate Alzheimer’s disease group), and their average age was 56.54 years (standard deviation (SD) = 13.13) and 54.47 years (SD = 11.71), respectively. Around 47% of the caregivers had been caring for the person with Alzheimer’s between two and five years. The Goldberg General Health Questionnaire was used to measure perceived health and the presence of new-onset mental health problems. An exploratory descriptive analysis and a multivariate logistic regression analysis were conducted. For caregivers of people with mild Alzheimer’s disease, ‘perceived health’ was related to ‘perceived social support’ (r = −0.21; p = 0.028), ‘person’s level of dependency’ (r = −0.24, p = 0.05), ‘severity of the person’s neuropsychiatric symptoms’ (r = 0.22; p = 0.05), and ‘caregiver’s emotional distress in response to the person’s neuropsychiatric symptoms’ (r = 0.22; p = 0.05). For caregivers of people with moderate Alzheimer’s disease, ‘perceived health’ was related to ‘perceived social support’ (r = −0.31; p ˂ 0.01), ‘presence of neuropsychiatric symptoms’ (r = 0.27, p = 0.01), ‘severity of the person’s neuropsychiatric symptoms’ (r = 0.32, p = 0.01) and ‘caregiver’s emotional distress in response to the person’s neuropsychiatric symptoms’ (r = 0.029; p = 0.01). The presence of new-onset mental health problems was detected in 46.3% (n = 50) of caregivers of people with mild Alzheimer’s and 61.9% (n = 91) of caregivers of people with moderate Alzheimer’s. When people are diagnosed with mild Alzheimer’s disease, intervention programs for caregivers should aim to regulate emotions and promote positive coping strategies. When people are diagnosed with moderate Alzheimer’s disease, intervention programs for caregivers must allow them to adapt to caregiving demands that arise with the progression of Alzheimer’s disease.

Purpose The purpose of this paper is to explore what worked well in terms of peer involvement in a diverse network of community groups for people affected by mental health problems in Bath and North East Somerset (BANES), UK. Design/methodology/approach A participatory action inquiry approach engaged the network’s key stakeholders (group members, facilitators, and commissioners) in critical reflection on what supported successful groups. Findings Successful groups have six characteristics: mutual support, a positive shared identity, opportunities for taking on roles, negotiated ground rules, skilled facilitation, and a conducive physical environment. Additionally, each group achieved a balance between the following areas of tension: needing ground rules but wanting to avoid bureaucracy, needing internal structure whilst also committing to group activities, balancing leadership with accountability, wanting peer leadership whilst acknowledging the burden of this responsibility, and lobbying for change in mental health services whilst acknowledging the need for support from them. Research limitations/implications The evaluation shows a group’s success is about adaptability and group facilitation is the art of navigating a course through these competing demands above. These insights have informed plans for a practical guide for developing peer led groups and for training of peer leaders in BANES. Originality/value This evaluation focuses on self-efficacy. It draws on group members’ own perceptions of what worked best for them to provide transferable learning about how peer led support groups might develop more generally. It can thus inform the growth of a comparatively new kind of community-based support for people with mental health problems and for their carers.

The main aim of this research was to assess the relevance and impact of wellness recovery action planning (WRAP) as a tool for self-management and wellness planning by individuals with mental health problems from pre-existing and newly formed groups, where the possibilities for continued mutual support in the development of WRAPs could be explored. Interviews and focus groups were conducted and pre-post recovery outcome measures completed (Recovery Assessment Scale and Warwick Edinburgh Mental Well Being Scale). 21 WRAP group participants took part in the research. The WRAP approach, used in groups and delivered by trained facilitators who could also share their lived experience, was very relevant and appeared to have a positive impact on many of the participants. The impact on participants varied from learning more about recovery and developing improved self-awareness to integrating a WRAP approach into daily life. The apparent positive impact of WRAP delivered in the context of mutual support groups indicates that it should be given serious consideration as a unique and worthwhile option for improving mental health. WRAP groups could make a significant contribution to the range of self-management options that are available for improving mental health and well-being.

Background Adolescence and early adulthood are critical periods for the development of mental disorders. Online peer-to-peer communication is popular among young people and may improve mental health by providing social support. Previous systematic reviews have targeted Internet support groups for adults with mental health problems, including depression. However, there have been no systematic reviews examining the effectiveness of online peer-to-peer support in improving the mental health of adolescents and young adults. Objective The aim of this review was to systematically identify available evidence for the effectiveness of online peer-to peer support for young people with mental health problems. Methods The PubMed, PsycInfo, and Cochrane databases were searched using keywords and Medical Subject Headings (MeSH) terms. Retrieved abstracts (n=3934) were double screened and coded. Studies were included if they (1) investigated an online peer-to-peer interaction, (2) the interaction discussed topics related to mental health, (3) the age range of the sample was between 12 to 25 years, and (4) the study evaluated the effectiveness of the peer-to-peer interaction. Results Six studies satisfied the inclusion criteria for the current review. The studies targeted a range of mental health problems including depression and anxiety (n=2), general psychological problems (n=1), eating disorders (n=1), and substance use (tobacco) (n=2). The majority of studies investigated Internet support groups (n=4), and the remaining studies focused on virtual reality chat sessions (n=2). In almost all studies (n=5), the peer support intervention was moderated by health professionals, researchers or consumers. Studies employed a range of study designs including randomized controlled trials (n=3), pre-post studies (n=2) and one randomized trial. Overall, two of the randomized controlled trials were associated with a significant positive outcome in comparison to the control group at post-intervention. In the remaining four studies, peer-to-peer support was not found to be effective. Conclusions This systematic review identified an overall lack of high-quality studies examining online peer-to-peer support for young people. Given that peer support is frequently used as an adjunct to Internet interventions for a variety of mental health conditions, there is an urgent need to determine the effectiveness of peer support alone as an active intervention.

Concerned with the increasing incidence of mental health problems in children and adolescents and the impact of these problems on students’ school success and predisposition to self- and other-directed violence, the Multnomah Education Service District Department of School Health Services determined to become proactive by providing preventive interventions for students experiencing actual or potential mental health problems. An educational program was designed to assist school nurses in the identification of potential mental health problems. In addition, information about appropriate interventions for students at risk for aggression, violence, and other mental health pathology was presented. The program involved education on mental health assessment and intervention, as well as expert psychiatric clinical support for the development of student support groups. School nurses were then challenged to develop practice improvement projects incorporating this knowledge for a group of students in their work setting. This introductory article describes the project’s general rationale and implementation process. The four articles following in this issue of The Journal of School Nursing describe the goals, implementation, and outcomes of the practice improvement projects developed for early intervention with students exhibiting attention disorders, school absenteeism, social withdrawal, and depression.
