SummaryA case of a 49-year-old man suffering from bilateral adrenocortical carcinoma with local and secondary rapid progression is reported. The results of adrenocortical scintigraphy (NP 59) and histological findings allowed the diagnosis. This case report and a literature review showed the importance of using adrenocortical scintigraphy as a complementary imaging procedure of CT or MR images.

Physical imaging technique described as Diffusion Weighted-Magnetic Resonance Imaging (DW-MRI) is based on classically principle of Brownian motion, where the molecules are thermal agitated and is highly influenced by the cellular availability of water. The aim of this study is to discuss the use of DW-MRI as a cancer diagnostic application tool using the basic physics principles as versus other available procedures and modalities in terms of accuracy and acceptability. Based on extravascular diffusion measurements where the measured signal is related to tissue cellularity, tissue organization and extracellular space tortuosity and on the intactness of cellular membranes that are intrinsically hydrophobic. The methodology involve the application of DW-MRI procedure, to qualitatively and quantitatively access DW-MR images to diagnose brain tumors, prostate and other organ cancers compared to other imaging modalities including other MRI procedures. It also include safety assessment and other consideration before, during and after imaging with MRI as compare to other radiological modalities. The results of the data of ten (10) MRI centers and 112 DW-MRI images and 99 other procedure and modalities were analysed, 34% were prostate cases, 27% were brain cases and 39% formed all other cases. In addition, DW-MRI compare to other single imaging procedure formed 53% of all diagnostic procedure that had 87% accurate predictability of prostate and brain cases. It can therefore concluded that DW-MRI is the best single imaging procedure that can be used to diagnose prostate cancers and brain tumors. It has a major advantage of non-ionizing radiation technique, with multiple planes image acquisitions, together with superior soft tissue contrast. In addition its perfusion allow for precise tissue characterization rather than merely 'macroscopic' imaging and superior visualization of both active parts of the brain during certain activities and understanding of the underlying networks. However, there are two outstanding challenges of DW-MRI scans in Ghana: it is expensive as compared to other modalities and not safe for patients with some metal implants. Despite these challenges, its advantages override its disadvantages and therefore it is recommended to clinicians as the first diagnostic tool to use in prostate cancer and brain tumor diagnoses.

Objective: To evaluate the validity of ultrasonography in patients who have obstructive jaundice. Design: Descriptive study.Place and duration of study: The study was carried out from September 2006 to May 2008 in department of Radiology Combined MilitaryHospital Quetta. Patients and Methods: A total of 30 patients; 14 male and 16 female underwent operation for obstructive jaundice. All of themhad preoperative ultrasonography. The site and nature of biliary obstruction were noted and the accuracy was determined with per-operativefindings / histological diagnosis as gold standard. Results: The cause of obstructive jaundice identified by ultrasonography with reasonablesensitivity of 90% and specificity of 90% for choledocholithiasis and sensitivity of 55.5% and specificity of 95.2% for pancreatic head tumours.Conclusions: Ultrasonography should be the first and best initial imaging procedure in patients who have obstructive jaundice and showsreasonable sensitivity and specificity to identify causes of obstruction in obstructive jaundice.

Introduction: Various positioning techniques are utilized to enhance the visualization of lower cervical vertebrae on lateral radiographs. However, the effectiveness of these techniques still remains unclear. This study was conducted to determine the effect of the weight-bearing (WB) technique in visualizing lower cervical vertebrae and cervicothoracic junction (C7-T1) on standing lateral cervical radiographs of adult non-trauma patients. The study was conducted using both computed radiography (CR) and digital radiography (DR) systems.Methods: Forty-four CR (29 WB and 15 non-WB – NWB) and 61 DR (26 WB and 35 NWB) lateral C-spine radiographs were prospectively evaluated to assess the visible number of cervical vertebral bodies and C7-T1 junction. The instructions given by the radiographer to the patient for the imaging procedure were also assessed on the Likert scale (very good, good, fair, poor, very poor).Results: There was no significant difference (p > 0.05) in the visualization of the number of vertebral bodies between the two techniques of WB and NWB for CR or DR. Further, no significant relationship (p > 0.05) was observed between the WB technique and the visualization of C7-T1 junction in DR systems. However, a significant difference was identified for CR (p = 0.012). The instruction given to the patient significantly correlated with the visibility of the lower C-spine region within each group of WB and NWB in both imaging systems.Conclusions: The visibility of the number of vertebral bodies in the lower C-spine region in either CR or DR systems did not demonstrate any enhancement with the WB technique. Regardless of the imaging system or techniques used, adequate instructions given to the patient before and during the imaging procedure of C-spine lateral radiography demonstrated a significant improvement in visualizing the lower C-spine region. In this preliminary study, the application of erect WB radiography technique in evaluating the lower cervical region of adult non-trauma patients gives limited advantage.

The coexistence of multiple, synchronous primary tumors of different histology within the same kidney is a rare condition. We report herein a series of five patients with two tumors of different histology involving synchronously the same kidney. Materials and Methods We reviewed the pathology reports of a series of 381 patients who underwent surgery for primary renal tumors at our institution from 2000 to 2007. In the files of all patients with synchronous tumors of different histology, special attention was given to the results of imaging studies. Results Five out of 381 patients (1.37%) had coexistence of two primary tumors of different histology within the same kidney. Four patients had ultrasonography as the first imaging procedure, one patient had ultrasonography as the second imaging procedure; all had preoperative CT of the abdomen. Both lesions were detected by preoperative CT in 4/5 of the cases; in the remaining one, the smaller lesion was not visible, even in retrospect. Conclusions The coexistence of multiple and synchronous primary tumors of different histology within the same kidney has been only rarely described. To the best of our knowledge, in literature there are only case reports with the exception of a case of renal oncocytoma with evolving papillary RCC. We believe that this condition could be more frequent if the radiologist and the anatomopathologist try to find it.
