The Role of Fluorescein Angiography and Optical Coherence Tomography in the Examination of Circulatory Disorders of the Optic Disc

Purpose: To assess the current role of fluorescein angiography after the introduction of optical coherence tomography angiography in real-life clinical practice. Methods: This was a multicentric retrospective observational study to evaluate the number of fluorescein angiography and optical coherence tomography angiography procedures performed by different devices from January 2013 to December 2018. The centers involved were Centro Italiano Macula (Rome), and ophthalmology departments of University “G. D’Annunzio” Chieti–Pescara (Chieti) and “Azienda Ospedaliero Universitaria Careggi” (Florence). Results: Out of 19,898 total fluorescein angiography procedures performed in the observation period, 3444 (17.3%) were in 2013, 3972 (19.9%) were in 2014, 3601 (18.1%) were in 2015, 3407 (17.2%) were in 2016, 3285 (16.5%) were in 2017, and 2189 (11%) were in 2018. Out of 7949 optical coherence tomography angiography procedures performed in the observation period, none were performed in 2013, 550 (6.9%) were in 2014, 908 (11.5%) were in 2015, 2098 (26.4%) were in 2016, 2090 (26.3%) were in 2017, and 2303 (28.9%) were in 2018. Conclusion: Fluorescein angiography procedures were performed less often after the introduction of optical coherence tomography angiography technology. The ease, speed, and safety of the optical coherence tomography angiography procedure in everyday clinical practice have facilitated more optical coherence tomography angiography application compared to fluorescein angiography in recent years. In the future, we will probably evaluate the different pathologies that still need an evaluation by fluorescein angiography.

We describe a 7-year-old girl who developed exudation nasally to the right optic disc due to retinal arteriovenous malformation. Fluorescein angiography, spectral domain optical coherence tomography, and optical coherence tomography angiography were performed. We give an overview of the different imaging techniques and discuss the differential diagnosis. Since there was no visual impairment, no treatment was started. A spontaneous decrease in edema and exudation was noted after 6 months.

Purpose. The purpose of this study was to report a case of bilateral vitreopapillary traction, previously misdiagnosed as papilledema.Methods. A case report is presented of a 47-year-old woman with a prior diagnosis of papilledema, who is shown to have bilateral vitreopapillary traction rather than true optic disc swelling, confirmed by optical coherence tomography (OCT).Results. OCT showed vitreous traction surrounding the optic discs of both eyes. Fluorescein angiography demonstrated focal leakage of both discs.Conclusion. Bilateral disc elevation caused by vitreous traction can be confused with papilledema. In such cases, OCT can be used to arrive at the correct diagnosis. Although the phenomenon of vitreopapillary traction is well reported, this case indicates that not all ophthalmologists recognize the condition.

Background: The study was conducted to evaluate the role of fundus fluorescein angiography (FFA) and spectral domain-optical coherence tomography (SD-OCT) in choroidal neo-vascularisation (CNV).Methods: This was a hospital based prospective study carried out in the post-graduate department of ophthalmology, SKIMS medical college, Bemina, Srinagar, Jammu and Kashmir. All patients diagnosed with CNV fulfilling the criteria during the study period w.e.f. October 2018 to March 2020 were enrolled. Visual acuity and pinhole test using Snellen’s chart for literate and E chart for illiterate patient, slit lamp biomicroscope for anterior segment examination, ophthalmoscopy, including stereoscopic examination of the posterior pole, 90D examination of the fundus, Intra-ocular pressure measurement, FFA and SD-OCT was done in these patients.Results: Diagnostic accuracy of OCT was observed with a sensitivity 79.1% (95% confidence interval (CI): 67.3-90.7), specificity 84.3% (95% CI: 74.5-92.9), positive and negative predictive values 78.7% and 85.4%, respectively, (95% CI: 65.5-95.6) and (74.8-93.4) and diagnostic accuracy of FFA was observed with a sensitivity 81.4% (95% confidence interval (CI): 70.6-93.5), specificity 82.31% (95% CI: 71.9-89.3), positive and negative predictive values 79.9% and 83.7%, respectively, (95% CI: 68.8-92.9) and (70.3-91.2).Conclusions: FFA is the gold standard procedure for screening ARMD and detection of dry ARMD, but OCT is more specific diagnostic tool in detecting early subretinal neovascular membrane and also to assess the extent, location and activity of the neovascular membranes. This study concludes that SDOCT is highly sensitive for identifying AMD, CNV, and CNV activity and due to its non-invasive nature with no adverse effects and less time consuming can be used as 1st line of diagnostic modality and FFA be reserved for cases where SD-OCT is not helpful.

We present a 57-year-old referred reduced visual acuity who was in treatment with paclitaxel for developing metastatic breast adenocarcinoma. Ophthalmoscopic examination, optical coherence tomography, and autofluorescence show the cystoid macular edema, but fluorescein angiography is normal, without leakage of dye in the late times. The patient responds well 8 weeks after stopping antineoplastic. Paclitaxel can cause cystoid macular edema and lifting a recovery both anatomical and functional of the macula.
