Baby powder is widely used in the home for the routine care of infants' skin. The object of this report is to point out the frequency of baby powder aspiration and the potential hazard of careless use. A recent report reviewed more than 25 cases of talcum powder aspiration with a mortality of 20%.1 These cases represent only the most severe episodes. The true incidence of baby powder inhalation is grossly underestimated. Moss2 states that approximately 50 cases a year are reported to the New York City Poison Control Center alone.2 Dusting powders used in the past were predominantly composed of zinc stearate, a talc substitute.

Amid significant increases in ridership (9.8% over the past 5 years) on the more than 100 year-old New York City Transit (NYCT) subway system, NYCT has become aware of increased crowding on station platforms. Because of limited platform capacity, platforms become crowded even during minor service disruptions. A real-time model was developed to estimate crowding conditions and to predict crowding for 15 min into the future. The algorithm combined historical automated fare collection data on passenger entry used to forecast station entrance, automated fare collection origin–destination inference information used to assign incoming passengers to a particular direction and line by time of day, and general transit feed specification–real time data to determine predicted train arrival times used to assign passengers on the platform to an incoming train. This model was piloted at the Wall Street Station on the No. 2 and No. 3 Lines in New York City’s Financial District, which serves an average 28,000 weekday riders, and validated with extensive field checks. A dashboard was developed to display this information graphically and visually in real time. On the basis of predictions of gaps in service and, consequently, high levels of crowding, dispatchers at NYCT’s Rail Control Center can alter service by holding a train or skipping several stops to alleviate any crowding conditions and provide safe and reliable service in these situations.

Knowledge of the root cause(s) of delays in transit networks has obvious value; it can be used to direct resources toward mitigation efforts and measure the effectiveness of those efforts. However, delays with indirect causes can be difficult to attribute, and may be assigned to broad categories that indicate “overcrowding,” incorrectly naming heavy ridership, train congestion, or both, as the cause. This paper describes a methodology to improve such incident assignments using historical train movement and incident data to determine if there is a root-cause incident responsible for the delay. It is intended as first step toward improved, data-driven delay recording to help time-strapped dispatchers investigate incident impacts. This methodology considers a train’s previous trip and when it arrived at the terminal to begin its next trip, as well as en route running times and dwell times. If the largest source of delay can be traced to a specific incident, that incident is suggested as the cause. For New York City Transit (NYCT), this methodology reassigns about 7% of trains originally without a root cause identified by dispatchers. Its results are provided to NYCT’s Rail Control Center staff via automated daily reports which, along with other improvements to delay recording procedures, has reduced these “overcrowding” categories from making up 38% of all delays in early 2018 to only 28% in 2019. The results confirm both that it is possible to improve delay cause diagnoses with algorithms and that there are delays for which both humans and algorithms find it difficult to determine a cause.

Most pediatricians are aware of the adverse effects of talcum powder aspiration which may result in acute or chronic lung disease. At least three fatalities have been reported following the aspiration of talcum powder by infants. Verbal communication with the New York Poison Control Center reveals that approximately 50 cases of talcum powder aspiration are reported to that center annually. Despite the potential dangers of talc aspiration, little has been done to publicize these dangers to the general public.
