Civil Marine Radar (CMR) is now a mature instrument of some 50 years steady development. Following a short history, more to provide benchmarks of achievement than to satisfy historians, this paper examines what will drive the future development of CMR. The answer may not follow recent developments, but may well be the result of a fresh maritime modus operandi to fulfil the navigation, collision avoidance and safe passage needs of all types of craft operating in all types of water with different pressures and Operational Requirements (ORS). Having postulated fresh ORS, suggestions are made on how best to fulfil them in a logical and cost-effective manner.

Object Monomelic amyotrophy (MMA) is a benign, self-limiting lower motor neuron disease. Optimal surgical strategies—discectomy decompression and fusion (DDF) or corpectomy decompression and fusion (CDF)—for patients with aggravated symptoms (within 6 months of presentation) are controversial, particularly in those who are ineligible for conventional treatment. These 2 methods of anterior cervical decompression and fusion for MMA in patients unwilling or unable to wear a conventional cervical collar long term were evaluated. Methods Anterior cervical decompression and fusion were performed in 48 male patients with MMA between September 2007 and September 2010. Patients were randomly treated with anterior cervical discectomy decompression with autologous iliac crest bone grafting and internal plate fixation (DDF group: 24 patients) or anterior cervical corpectomy, posterior longitudinal ligament resection, autologous iliac crest bone grafting, and internal plate fixation (CDF group: 24 patients). Subjective symptom assessments and electromyography (EMG) examinations were conducted both preoperatively and postoperatively. Results Subjective assessments and EMG studies (mean follow-up duration 25.9 months) indicated improvement in 64.6% and 60% of patients, respectively. No significant correlations between the effectiveness of surgery and age at symptom onset, preoperative symptom duration, or postoperative follow-up time were found. Conclusions The lack of a significant difference in effectiveness between DDF and CDF favors DDF as more clinically applicable because of its lower procedural risks. As a second-line alternative to cervical collar treatment, surgical anterior cervical decompression and fusion via DDF or CDF may prevent further deterioration and produce good short-term therapeutic effects in patients with MMA; however, cervical collar treatment is recommended for eligible patients because of its lower associated risks.

SummaryDistal femoral osteotomy is a surgical procedure used to correct patellar luxation, secondary to a femoral deformity. A distal femoral osteotomy using the tibial plateau levelling osteotomy-jig to temporarily provide stability of the distal femoral osteotomy, maintaining limb alignment in the frontal and axial planes prior to internal plate fixation of the osteotomy, has been described. This report describes a novel jig named Deformity Reduction Device (DRD). This device was developed with the specific aim of increasing precision and predictability during corrective osteotomy execution in order to be consistent with the preoperative planning. The distal femoral osteotomy DRD-assisted procedure is described in detail, discussing the theoretical and practical principles of the application.

Increasing physical activity of hemophilic children has resulted in more frequent fractures. Within the last ten years, fourteen severe hemophiliacs (8-19 years)sustained femoral fractures. Four of these hemophiliacs had an inhibitor. Eight were the result of minimal trauma. There was no obvious correlation in these eight cases with prolonged immobilisation and/or treatment with steroids. Co-existing knee hemarthrosis or thigh hematoma often led to a delay in the diagnosis of the fracture. Metaphyseal fractures(8 cases) were treated by plaster cast (4) or, skin or pin traction (4) with good anatomical result and normal union. Replacement therapy was given in 3 cases for 3 days and in 1 case for 2 weeks. No complication occurred in any of the non transfused patients(4). Epiphyseal and femoral shaft fractures were treated by skin or pin traction (3) or internal plate fixation (2)associated with replacement therapy for at least 2 weeks. Unsatisfactory anatomical result was obtained in three cases. One impacted fractured neck of femur was discovered on X-ray examination 3 weeks after trauma. Guide lines for orthopedic and medical treatment of femoral fractures in hemophiliacs are suggested according to the anatomical feature.

While many scholars rightly point to the contemporary influence of talkback radio as an increasingly prominent platform for civic and political debate, as talkback radio approaches its fortieth anniversary, little is known about the history and development of the format. It was in 1967 that metropolitan radio stations in Australia rushed to embrace a ‘new’ radio programming format, as talkback radio became formally — and finally — legally permissible. However, the documented history of commercial talkback in Australia began many years earlier and has been punctuated by frequent clashes between radio programmers and broadcasting regulators over issues relating to the nature of programming content. As a platform for the discussion of contemporary social issues, talkback has thrived by courting controversy and debate. The commercial talkback radio format has supported the rise of a small, but highly prominent, group of men and continues to be strongly guided by economic imperatives, as witnessed in recent developments such as the ‘cash for comment’ affair. This article details the growth of metropolitan commercial talkback radio in Australia over the last four decades and looks at the extent to which public policy and economic influences have shaped the development of the format.
