Aims. The purpose of this study was to analyze the initial experience with adolescents treated for long-bone deformities of the lower extremities of different etiologies using osteotomies and fixation by interlocking nails.Materials and methods. We analyzed the accuracy of long-bone deformity correction using referent lines and angles, the time of consolidation, number of complications, and functional result.Results. We found that the accuracy of femur deformity correction (dependent on the complicity of the deformity), as estimated by different parameters, varied from 77.8% to 91.7%. Simple deformities and deformities of moderate complicity had the most accurate correction; the group of complex multiplanar deformities of the femur had the least accurate correction. This group included five cases of residual deformity, in which three of these had an angle of residual deformity <10°. The accuracy of leg deformity correction was 90%. Evaluation of the functional results using the Lower Extremity Functional Scale indicated the high functionality of the method used.Conclusions. Correction of long-bone deformities using intramedullary osteosynthesis by interlocking nails is an effective treatment of all types of femur and lower leg deformities. When treating complex deformities of the femur, the path to operative treatment should be complex and in most cases the nailing should be accompanied by intraoperative external fixation frame assistance.

INTRODUCTION: - The X-linked vitamin D-resistant hypophosphatemic rickets (VDXLR) is a metabolic disorder. Medication treatment consists of oral phosphate substitution and supplementation of active vitamin D compounds. Our study aimed to review our patients with VDXLR, focusing on those undergoing surgery, mainly lengthening procedures. The main parameters of interest were growth, height, the axis of the lower limbs, pain, and degenerative arthropathy. METHODS: - Twelve patients with VDXLR were followed at our institution. Eight patients underwent surgical correction, and three of them in combination with bone lengthening. The corrections were executed at the end of growth in the patients. Clinical end points were height, leg axis, and pain. RESULTS: - Single bilateral surgical correction was performed in six patients; one patient had three and ve corrections. Bone lengthening was performed in three patients. At the last follow-up, the height of seven operated patients was within normal range. In addition, the leg axis was normalized in six patients with mild genua vara in two. Bone healing was excellent, and no surgical complications. There was no one radiological evidence of degenerative arthropathy. CONCLUSIONS: - In case of bone deformity, surgery can safely be performed, independent of age or bone maturation. All patients were happy with the outcomes of axial corrective surgery and bone lengthening, and in the majority. Only one corrective intervention was needed.

Наиболее важной задачей при дистракционном остеосинтезе является создание простых и эффективных способов контроля темпов дистракции позволяющих производить удлинение конечности в оптимальном режиме у различных групп пациентов. Цель исследования - анализ содержания сывороточных концентраций TGFb-1 и TGFb-2, BMP-4 и BMP-6 у лиц при увеличении роста с косметической целью и у пациентов с ахондроплазией на этапах дистракционного остеогенеза при удлинении голени методом Илизарова. Методика. Определение ростовых факторов проводили на комплексе оборудования фирмы Thermofisher (США): детектор Multiscan FC, встряхиватель iEMS, автоматический промыватель планшет WellWash. Для измерения концентрации факторов в сыворотке крови использовали наборы для иммуноферментного анализа (ИФА) eBioscience и RayBiotech Inc. (США). Результаты . Показано, что в сыворотке крови людей с ахондроплазией, еще до какого-либо ортопедического лечения концентрации TGF-b2 и ВМР-4, ниже, а TGF-b1 и ВМР-6, выше, чем концентрации этих факторов у людей при увеличении роста с косметической целью. У последних удлинение конечностей сопровождается возрастанием содержания в сыворотке крови факторов TGF-b1 и TGF-b2 на этапах начала и середины процесса дистракции. У пациентов с ахондроплазией, наблюдается обратная динамика: концентрация данных факторов (ВМР-4 и ВМР-3) в крови ахондропатов к концу дистракции увеличивается в 3,5 и 2 раза соответственно, снижаясь в периоде фиксации. Заключение. Таким образом, у пациентов с ахондроплазией происходит нарушение этапности процессов костного ремоделирования. Elaboration of simple and effective ways for managing the distraction rate to provide an optimum regimen of limb lengthening for different groups of patients is an important task in implementation of transosseous osteosynthesis. The aim of this study was to analyze blood serum concentrations of TGFb-1, TGFb-2, BMP-4, and BMP-6 in individuals undergoing cosmetic height increase and patients with achondroplasia at different stages of distraction osteogenesis in tibial lengthening using the Ilizarov method. Methods. Concentrations of growth factors were measured using a set of Thermofisher (USA) equipment, including a Multiscan FC detector, iEMS Shaker, and automatic WellWash Washer and ELISA kits (eBioscience and RayBiotech Inc., USA). Results. Serum concentrations of TGFb-2 and BMP-4 were lower and TGFb-1 and BMP-6 were higher in achondroplasia patients than in individuals with cosmetic height increase even before any orthopedic treatment. Long bone lengthening for cosmetic height increase was associated with increases in serum levels of TGFb-1 and TGFb-2 at the start and in the middle of distraction. In achondroplasia patients, opposite changes were observed; serum concentrations of BMP-4 and BMP-3 increased 3.5 and 2 times, respectively, by the end of distraction and decreased during fixation. Conclusion. Therefore, we observed a disorder of the stage-by-stage bone remodeling process in achondroplasia patients.

The article is dedicated to computer-assisted external fixation devices, so-called hexapods. The main advantage of these frames is capability to make mathematically precise correction of bone fragments in three planes and six degrees of freedom on the base of calculations made in special software application. Recently these devices are mostly applied in long bone deformity correction but the sphere of its effective useis not limited by only this direction. The article presents the history of investigation of these devices, their development, implemented comparative analysis of the basic hexapods: TSF (Taylor Spatial Frame), IHA (Ilizarov Hexapod Apparatus) and Ortho-SUV Frame.
