An epidemiologic approach to predictors of elective coronary artery bypass mortality in a non-university hospital population

Prospective data of 3,120 consecutive patients who had elective coronary artery bypass were analyzed to identify patient profile, cost, outcome and predictors of those readmitted to the intensive care unit. Group A ( n = 3,002) had a single intensive care unit admission and group B ( n = 118) were readmitted within 30 days after surgery. Parsonnet score, EuroSCORE, age, body mass index, chronic obstructive airway disease, peripheral vascular disease, renal dysfunction, unstable angina, congestive cardiac failure, and poor left ventricular function were higher in group B. Bypass and crossclamp times were longer, and the prevalence of inotropic and balloon pump support, arrhythmias, myocardial infarction, re-exploration, blood loss and transfusion, cerebrovascular accident, wound infection, sternal dehiscence, and multisystem failure were higher in group B. Despite a 4-fold increase in cost of care, the mortality rate (32.4%) of patients readmitted to intensive care was 23-times higher than routine patients (1.4%). Crossclamp time > 80 min, Parsonnet score > 10, EuroSCORE > 9, sternal dehiscence, ventricular arrhythmias, and renal failure predicted readmission.

Abstract Objective: To identify modifiable and non-modifiable risk factors among young patients undergoing elective coronary artery bypass graft surgery. Methods: The descriptive cross-sectional study was conducted at the Department of Adult Cardiac Surgery of the Armed Forces Institute of Cardiology and National Institute of Heart Disease, Rawalpindi, Pakistan, from January 1, 2015, to April 24, 2018, and comprised patients who underwent elective coronary artery bypass grafting. Data was collected using a predesigned questionnaire, and the risk factors studied were hypertension, diabetes, smoking, serum cholesterol, family history and body mass index. Data was analysed using SPSS 23. Results: Of the 1270 patients, 838(66.1%) were males and 432(33.9%) were females. The overall mean age was 39.29±8.64 years (range: 18-49 years). Of the total, 319(25.2%) patients were hypertensive; 170(13.3%) were diabetics; 303(23.9%) had history of smoking. Conclusion: There was a high prevalence of hypertension, smoking and obesity in the patients undergoing coronary artery bypass graft surgery at age <50 years.
