Abstract Objective: The objective of this study was to describe the antenatal ultrasound appearances of congenital high airway obstruction syndrome (CHAOS), which typically manifests as large echogenic lungs acquiring whole of the thorax and dilatation of the trachea distal to the obstruction and causing intrinsic laryngeal/tracheal obstruction. Case report: A 30-year-old pregnant female with 24 weeks gestation come for routine absolute neutrophil count (ANC) scan. Ultrasonography (USG) was performed, and large echogenic lungs occupying whole of the thoracic cavity compressing the heart in between and causing inversion of the bilateral domes of the diaphragm were found. There was dilatation of the tracheal distal to the lesion and polyhydramnios. Conclusion: The described USG finding of CHAOS may be useful in deciding when to include this entity in the differential diagnosis of enlarged echogenic lungs.

Background: Perifolliculitis capitis abscedens et suffodiens (PCAS) is a suppurative process that involves the scalp, eventually resulting in extensive scarring and irreversible alopecia. This condition often presents in males of African American origin. Objective: This article describes the clinical presentation, diagnosis, and treatment of an Aboriginal Canadian male suffering from PCAS. A literature review on the etiology, pathology, differential diagnosis, and management is also discussed. Conclusion: Careful analysis of the pathology and clinical presentation can aid in the timely diagnosis and management of this challenging condition. The clinician dealing with patients suffering from PCAS has several treatment options available to help successfully manage patients with straightforward or recalcitrant disease.

Summary This case report reviews the rare condition of Riedel’s thyroiditis via a patient case. The report highlights the difficulties that one may encounter when managing such a case in regards to patient symptoms, side effects of medications and the relapsing nature of the condition. The case report also highlights novel treatment in the treatment of Riedel’s thyroiditis, rituximab, how this works and the resolution of symptoms that we have achieved with our patient on this treatment. Learning points: Riedel’s thyroiditis is characterised by chronic inflammation, which causes dense fibrosis in the thyroid gland. Riedel’s thyroiditis can present with neck pain, dysphagia and dyspnoea with a firm, non-tender mass found on examination. Riedel’s thyroiditis is part of the IgG4-related systemic disorders. Rituximab is a monoclonal antibody that works against the protein CD20.

Advanced-stage hepatocellular carcinoma presenting with endobronchial metastases is an extremely rare phenomenon, with only a few cases reported to date. Despite endobronchial metastases being a rare occurrence, the clinically significant sequelae require prompt diagnosis and management. Therapeutic bronchoscopy represents an effective palliative treatment for malignant airway obstruction. This case report describes a 62-year-old man who presented with bilateral endobronchial metastases from advanced-stage hepatocellular carcinoma, requiring symptom palliation with laser bronchoscopy.
