ABSTRACT Equipment utilization management is the evaluation of the medical equipment necessity, appropriateness, and efficiency of the use in the healthcare services or procedures. The investment on an equipment is said to be a good one if it shows a utilization coefficient of 50% or above. The study was descriptive observational in nature and was conducted in the Histopathology Department, PGIMER, Chandigarh. The data for the entire year of 2012 was collected by studying various records of the department including purchase files, inventory registers, log books and service records of medical equipments. The results indicated that the Utilization Coefficients of various diagnostic equipments in the department of Histopathology, PGIMER, Chandigarh for the year 2012 were in the order of 58.1, 62.1 and 60.4% for high cost, medium cost and low cost equipments respectively. On an average, the utilization coefficient of medical equipments under study (Year 2012) of the Histopathology Department, PGIMER, Chandigarh, was found to be 60.2% (above 50%). This figure reflects that the budget spent on average medical diagnostic equipment in this department is very much justifiable. The study also showed that the cost incurred on various cost categories of diagnostic equipments, i.e. the low, medium and high cost equipments was also equally important. More than 30 years since enactment of the Americans with Disabilities Act, people with disability continue to face physical access barriers, notably inaccessible medical diagnostic equipment, in clinical settings. Access barriers affect breast cancer screening and treatment for women with disability. We used standard diagnosis codes and natural language processing to screen electronic health records (EHRs) in a digital data repository from a large health care delivery system for patients with pre-existing mobility disability diagnosed with breast cancer between 2005 and 2017. We reviewed EHRs of 20 patients, using conventional content analysis to examine breast cancer diagnosis and treatment experiences. Clinicians noted challenges positioning patients for routine procedures including manual breast exam, screening mammography, and breast biopsies. Given challenges accommodating disability for adjuvant therapies, mastectomy was favored over breast-conserving options despite early stages of diagnosis. Notations contained little information about proactive problem-solving for arranging accommodations. Notations described physical access barriers for breast cancer detection and treatment, with limited planning for mitigating barriers. Despite 2017 promulgation of federal Standards for Accessible Medical Diagnostic Equipment, implementing these standards requires further rulemaking.

Under the background of the unbalanced supply and demand of medical diagnostic equipment and rising health care costs, this study aims to optimize the service scheduling for medical diagnostic equipment so as to improve patient satisfaction by ensuring the equipment utilization rate and hospital revenue. The finite horizon Markov Decision Process (MDP) was adopted to solve this problem. On the basis of field research, we divided patients into four categories: emergency patients, inpatients, appointed outpatients, and the randomly arrived outpatients according to the severity of illness and appointment situations. In the construction of the MDP model, we considered the possibility of cancellation (no-show patients) in scheduling optimization. Combined with the benefits and costs related to patient satisfaction, based on the value iteration algorithm, we took patient satisfaction and hospital revenue as the objective functions. Results indicated that, compared with the current scheduling strategy, the integrated strategy proposed in this study has a better performance, which could maintain the sustainable usage rate of large medical resources and patient satisfaction.

We brought to the light that one of the the most controversial issues is the moment of adaptation to reconstructive procedures. Often it becomes unclear which and in which order to use medical diagnostic equipment, to training for fixed prosthetic stages. Our studies make it possible to predict not only the treatment time, but the amount of occlusal adjustment in conjunction with the reconstruction of the position of the mandible.One of the most debated issues is the time of adaptation to reconstructive manipulation. Often it becomes unclear how and in what order medical diagnostic equipment should be used in the preparatory stages of the stationary prosthetics. Our studies make it possible to predict not only the treatment time, and the amount of occlusal adjustment in conjunction with the reconstruction of the lower jaw.
