Isolation and detection of Leishmania species among naturally infected Rhombomis opimus, a reservoir host of zoonotic cutaneous leishmaniasis in Turkemen Sahara, North East of Iran

Background: Leishmaniasis is an expanding neglected tropical disease in the world reporting from 98 countries including Iran. This study focused on eco-epidemiological determinants of the disease following a rapid and unexpected increase of leishmaniasis incidence in a strategic residential district in North-East of Isfahan County, Iran. Methods: This study was accomplished from Apr 2012 to Jan 2014 in a strategic residential zone in North-East of Isfahan County, Esfahan, Iran. Monthly activity, parity, Leishmania infection and susceptibility tests, were determined on sand flies. Some portion of inhabitants and school children were surveyed to find active or passive cases of leishmaniasis and also wild rodents were collected to determine reservoir host. Results: Totally 5223 sand flies belonging to Phlebotomus and Sergentomyia genus were collected and identified; Ph. papatasi was the dominant species and started to appear in May and disappeared in Oct. The majority of living dissected sand flies were unfed and parous. Ph. papatasi showed 4.6% Leishmania infection through direct examination and 39.54% by nested-PCR respectively. Phlebotomus papatasi was susceptible against deltametrin 0.05%. Totally 2149 people were surveyed and incidence and prevalence of zoonotic cutaneous leishmaniasis estimated as 45.39 and 314.40 per 1000 population. Rodents showed 73.91% and 80% Leishmania infection by direct examination and nested-PCR respectively. Conclusion: Cutaneous leishmaniasis due to L. major has been established in this area. Rodent control operation and personal protection are highly recommended to control the disease in this focus.

Leishmaniasis emergence in Europe is reviewed, based on a search of literature up to and including 2009. Topics covered are the disease, its relevance, transmission and epidemiology, diagnostic methods, treatment, prevention, current geographical distribution, potential factors triggering changes in distribution, and risk prediction. Potential factors triggering distribution changes include vectorial competence, importation or dispersal of vectors and reservoir hosts, travel, and climatic/environmental change. The risk of introducing leishmaniasis into the European Union (EU) and its spread among Member States was assessed for the short (2-3 years) and long term (15-20 years). There is only a low risk of introducing exotic Leishmania species because of the absence of proven vectors and/or reservoir hosts. The main threat comes from the spread of the two parasites endemic in the EU, namely Leishmania infantum, which causes zoonotic visceral and cutaneous leishmaniasis in humans and the domestic dog (the reservoir host), and L. tropica, which causes anthroponotic cutaneous leishmaniasis. The natural vector of L. tropica occurs in southern Europe, but periodic disease outbreaks in Greece (and potentially elsewhere) should be easily contained by surveillance and prompt treatment, unless dogs or other synanthropic mammals prove to be reservoir hosts. The northward spread of L. infantum from the Mediterranean region will depend on whether climate and land cover permit the vectors to establish seasonal biting rates that match those of southern Europe. Increasing dog travel poses a significant risk of introducing L. infantum into northern Europe, and the threat posed by non-vectorial dog-to-dog transmission should be investigated.

Background and aims: Cutaneous leishmaniasis is a major health issue in many parts of Iran. Diagnosis of cutaneous leishmaniasis in a non-endemic area is not as simple as that in endemic foci. The management and treatment of this disease are global dilemmas. The purpose of this study was to identify Leishmania species isolated from human cutaneous leishmaniasis lesions in patients referred to health centers of Golestan province, Iran. Methods: Cutaneous leishmaniasis patients with suspected lesions were clinically examined. History of journey to the endemic areas of zoonotic cutaneous leishmaniasis and/or anthroponotic cutaneous leishmaniasis and the characteristics of their lesion(s) were recorded. Diagnosis of the lesion was done by using direct smear microscopy and conventional polymerase chain reaction. Results: Out of 360 patients, 202 (57.4 %) were male and 158 (42.8 %) were female. A total of 360 samples were selected from different cities. The number of infected samples in the cities is as follows: Azad Shahr [3 (1.5%)], Aq Qala [11 (5.7%)], Ali Abad [4 (2.1%)], Bandar Turkmen [3 (1.5%)], Bandar Gaz [2 (1%)], Ramian [3 (1.5%)], Kalaleh [23 (12.1%)], Kord koy [1 (0.5%)], Galikesh [7 (3.7 %)], Gomishan [12 (3.6 %)], Gorgan [13 (6.8 %)], Gonbad Qabus [99 (52.1 %)], Marave Tappe [7 (3.6%)] and Minoodasht [2 (1%)]. In the molecular investigations, 186 (96.8%) samples were observed to acquire rural cutaneous leishmaniasis, 4 (2.1%) to acquire urban cutaneous leishmaniasis and 170 (47.2%) to be uninfected. Conclusion: Although history of visiting endemic areas is an important factor to be considered in diagnosis, parasitological confirmation is necessary for the initiation of treatment.
