AbstractEtiology of ascites can be usually determined with ascitic fluid cytological and biochemical evaluation. Occasionally, the cause of ascites cannot be determined despite repeated ascitic fluid evaluations. These patients with undiagnosed ascites usually undergo diagnostic laparotomy/laparoscopy for etiological diagnosis. Endoscopic ultrasound (EUS) can help in resolving the diagnostic conundrum of undiagnosed ascites by visualizing as well as sampling peritoneal and omental deposits/thickening. However, rarely fine-needle aspiration from deposits may be falsely negative and patient may need repeat sampling. Newer EUS imaging techniques such as elastography and contrast-enhanced harmonic EUS by providing additive information on stiffness and enhancement pattern of the lesion can help in differential diagnosis.

Groove pancreatitis is a very uncommon type of chronic pancreatitis of uncertain etiology that occurs in the pancreatoduodenal groove. Despite the great advances in imaging techniques, making a definitive diagnosis is difficult because of the complex anatomy of this area. Therefore, surgical treatment is often required due to the impossibility of excluding malignancy. We present the case of a patient with a history of chronic pancreatitis admitted due to duodenal obstruction. The diagnosis was difficult, especially for the need to exclude the neoplasms of the duodenal-pancreatic area. Endoscopic ultrasound was essential to establish a definitive diagnosis, allowing FNAP and correct assessment of the duodenal wall.

Ultrasound of the breast is comfortable, widely available, and does not involve ionizing radiation. Indications for a breast ultrasound study include evaluation of a palpable lump, further evaluation of a mammographic or MRI abnormality, screening among women who are at increased cancer risk, and to provide guidance for a percutaneous biopsy. This chapter provides an overview of breast ultrasound, which is a key modality for diagnostic evaluation and, increasingly, for supplemental screening. Topics that will be covered here include the breast anatomy on ultrasound, imaging techniques, medical physics associated with breast ultrasound (including spatial compounding, Doppler imaging, and harmonic imaging), and clinical indications.
