Abstract ObjectiveThis study aimed to compare the characteristics of 18F-fluorodeoxyglucose positron emission tomography/computed tomography (18F-FDG PET/CT) in gastric lymphoma (GL) and gastric carcinoma (GC).METHODSPatients with newly diagnosed GL or GC who underwent 18F-FDG PET/CT prior to treatment were included in this study. The PET/CT features of gastric wall lesions, including FDG avidity, pattern, and intensity [maximal standard uptake value (SUVmax)] were reviewed and analyzed. The correlation of SUVmax with gastric clinicopathological variables was investigated by Student’s t test, Mann-Whitney U test, chi-square test, and receiver-operating characteristic (ROC) curve analysis to determine the differential diagnostic value of SUVmax-associated parameters in GL and GC.RESULTSA total of 68 patients with GL and 117 with GC were included in this study. Abnormal gastric FDG accumulation was found in 66 (97.05%) patients with GL and 109 (93.16%) patients with GC. Majority of the GL patients presented with type I and III lesions, whereas GC patients mainly with type II and III lesions. The SUVmax [14.89(6.56,22.12) vs 4.08(5.75,10.24), P<0.001) and SUVmax/THKmax (maximal thickness) [0.72(0.50,1,10) vs 0.39(0.28,0.64), P<0.000] were both higher in patients with GL when compared with GC. The SUVmax was higher in GL patients with DLBCL than in those with MALT (17.51±10.31 vs 4.97±2.92, P =0.0010) and higher in patients with advanced Lugano stage (II1/II2/IV) than in those with stage I (18.27±10.19 vs 9.92±9.22, P = 0.002). In GC patients, the SUVmax was higher in T3+T4 than in T1+T2 [6.79(4.47,12.41) vs 4.09(2.32,6.31), P = 0.000], but there were no significant differences between TNM stage I+II and TNM stage III+IV. The ROC curve analysis suggested a better performance of SUVmax/THKmax in evaluating gastric lesions between GL and GC when compared to SUVmax alone. The sensitivity of SUVmax and SUVmax/THKmax for differential diagnosis of GL and GC were 0.917 and 0.578, respectively. However, the specificity of SUVmax/THKmax for differential diagnosis of GL and GC was higher than that of SUVmax alone (0.894 VS 0.485).CONCLUSIONThe PET/CT features differed between GL and GC, and these can improve the evaluation of PET/CT of gastric wall lesions and help differentiate GL from GC.

e19541 Background: Positron emission tomography (PET)/computed tomography (CT) scan has a well-established role in the management of non-Hodgkin's lymphoma (NHL). However, in case of the primary gastric lymphoma, which is the most frequent extranodal NHL, the role of PET/CT scan is still controversial. Methods: We retrospectively analyzed 42 patients with primary gastric lymphoma who underwent PET/CT scans; 32 patients with diffuse large B-cell lymphoma (DLBCL) and 10 patients with extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT lymphoma) were analyzed. The PET/CT scans were compared with clinicopathologic features and the results of CT and endoscopy. After corresponding treatment, response was evaluated by conventional CT scans or PET/CT scans and endoscopy with biopsy Results: Nine patients were up-staged based on the results of their PET/CT scan compared to CT (7 DLBCL, 2 MALT lymphomas) while six patients were down-staged by the PET/CT scan. The high SUVmax group, defined as SUVmax ≥ median value, was significantly associated with an advanced Lugano stage (P < 0.001). Three patients with DLBCL, who showed an initially high SUVmax, died of disease progression. Although not statistically significant, there was a tendency of inferior outcome in the group with high SUVmax. Among 24 patients for whom follow-up PET/CT scan with endoscopy was performed, 11 patients with ulcerative or mucosal lesions showed residual FDG uptake. All of these gastric lesions were grossly and pathologically benign lesions without evidence of lymphoma cells. Conclusions: PET/CT scan can help staging patients with primary gastric lymphoma, and the maximum SUV has possibility to have prognostic value. However, the residual FDG uptake observed during follow-up should be interpreted cautiously in association with the results of endoscopy and multiple gastric biopsies. Background: The diagnostic performance of [18F]fluoro-ethylcholine-PET-CT&4D-CT (FEC-PET&4D-CT) to identify parathyroid adenomas (PA) was analyzed when ultrasound (US) or MIBI-Scan (MS) failed to localize. Postsurgical one year follow-up data are presented. Methods: Patients in whom US and MS delivered either incongruent or entirely negative findings were subjected to FEC-PET&4D-CT and cases from July 2017 to June 2020 were analyzed, retrospectively. Cervical exploration with intraoperative PTH-monitoring (IO-PTH) was performed. Imaging results were correlated to intraoperative findings, and short term and one year postoperative follow-up data. Results: From July 2017 to June 2020 in 171 FEC-PET&4D-CTs 159 (92.9%) PAs were suggested. 147 patients already had surgery, FEC-PET&4D-CT accurately localized in 141; false neg. 4, false pos. 2, global sensitivity 0.97; accuracy 0.96, PPV 0.99. All of the 117 patients that already have completed their 12-month postoperative follow up had normal biochemical parameter, i.e., no signs of persisting disease. However, two cases may have a potential for recurrent disease, for a cure rate of at least 98.3%. Conclusion: FEC-PET&4D-CT shows unprecedented results regarding the accuracy localizing PAs. The one-year-follow-up data demonstrate a high cure rate. We, therefore, suggest FEC-PET-CT as the relevant diagnostic tool for the localization of PAs when US fails to localize PA, especially after previous surgery to the neck.
