<p><strong>Introduction</strong><strong></strong></p><p>A trial or mock embryo transfer (ET) may influence pregnancy rates and it performed prior to ET allows the clinician to assess the uterine cavity and the utero-cervical angle. The aim of this study is to compare the consistency of the type of ET in mock ET with real ET.</p><p><strong>Material &amp; Methods</strong></p><p>A retrospective comparative analysis of  patients who underwent in vitro fertilization or ICSI cycle from January 2014 to December 2014 in Halim Fertility Center was done. The type of transfer was divided into two groups: ‘easy’ or ‘difficult’. An easy ET was defined as a transfer that occurred without the use of manipulation or other instrumentation and difficult ET was considered when additional instrumentation was required.</p><p><strong>Results</strong></p><p>From the study, 103 patients who underwent Mock-ET, we  found 58 patients (56.3%) with easy ET and 45 patients (43.7%) with difficult ET, which with hard catheter ET in 17 patients (16.5%), with osfander assistance in 20 patients (19.4%) and with stylet in 8 patients (7,8%). 58 patients with Easy Mock ET group were entirely easy real ET (100%) and 45 patients with difficult Mock ET group also entirely were difficult real ET (100%). The Statistical analysis shows no significant difference between the mock ET and real ET groups (p&gt;0,05). In easy real ET, clinical pregnancy rates were 32.8% and in difficult real ET, clinical pregnancy rates were 26.7% with no significant difference between the  groups (p&gt;0,05).</p><p><strong>Conclusion:</strong></p><p>Mock ET prior to the treatment cycle is consistent with real ET.</p>

BACKGROUND: Some studies suggested that saline infusion sonohysterography (SIS) has been used to detect uterine cavity abnormalities before in vitro fertilization (IVF) cycles to improve treatment success rates and decrease the number of cycle cancellations and embryo implantation failures. Some of the factors contributing to the dissemination and acceptance of the technique include the fact that it is a simple, less painful, less expensive, less invasive, and well-tolerated procedure when compared to hysteroscopy. AIM: The aim of the study is to evaluate uterine cavity abnormality with SIS performed before IVF. METHODS: A descriptive retrospective study involving 551 female partners who had SIS before IVF/ICSI treatment at private Halim Fertility Center from January 2014 until December 2017. Five hundred and fifty-one infertile woman was included in this study before IVF/ICSI cycles. Patients agreed to have an ultrasound assessment of the uterine cavity with the use of saline as the contrast medium. SIS procedure was scheduled postmenstrual period in the early-mid follicular phase and 1-3 months before starting IVF/ICSI treatment. RESULTS: From 551 patients, we found 527 (94.4%) cases with the normal uterine cavity. The uterine cavity abnormalities were detected in 5.56% of cases included in this study (28 (5.02%) cases with endometrial polyps, two (0.36%) cases with intrauterine adhesions, and one (0.18%) case with Müllerian duct anomalies). CONCLUSION: SIS before IVF treatment could be a good option for evaluating uterus cavity before IVF to improve success rates of pregnancy.

Abstract Study question Can the adipokines levels and ratios in the follicular fluid (FF) be used to predict in vitro fertilization (IVF) outcomes? Summary answer The leptin level and ratios leptin/ghrelin and leptin/adiponectin in FF are reliable prognostic factors for IVF outcomes in women with normal body mass index (BMI). What is known already The adipose tissue serves not a simple fat storage, rather an important endocrine organ, producing adipokines, such as leptin, adiponectin, ghrelin and others. Adipokines have been shown to regulate the cardiovascular system, food intake, metabolism, inflammation, metastatic spread of tumors,and also reproduction, affecting the activity of the hypothalamo-pituitary-gonadal axis. The plasma and FF adipokines have been used as prognostic factors for IVF outcomes, but the obtained results are controversial. The most promising in this case may be the distribution of patients into groups in accordance with their BMI and a separate study of adipokine ratios in them. Study design, size, duration Women (n = 53), who undergo IVF, were divided on two groups, according to their BMI: normal BMI (18.5–24.9 kg/m2, n = 25) and increased BMI (&gt;25.0 kg/m2, n = 28). Depending on IVF outcomes after the antagonist protocol, the groups formed were subsequently subdivided into two groups each: non-pregnant with normal BMI (nPN, n = 16), pregnant with normal BMI (PN, n = 9), non-pregnant with increased BMI (nPI, n = 21), pregnant with increased BMI (PI, n = 7). Participants/materials, setting, methods: Inclusion criteria for participants were: age 18–45 years and infertility due to male/tubal factor. Exclusion criteria were: polycystic ovarian syndrome (PCOS), diabetes mellitus, or plasma level of anti-Müllerian hormone &lt;1.0 ng/mL. The FF from the first punctated follicle was collected and tested for leptin, adiponectin and ghrelin levels using ELISA kits. If gestational sac was seen in uterine cavity using ultrasound on day 21–25 after embryo transfer, pregnancy was diagnosed. Main results and the role of chance Women with increased BMI had, as a rule, higher FF levels of leptin and the leptin/ghrelin and leptin/adiponectin ratios, compared with women with normal BMI. Furthermore, leptin level was higher in PN as compared to nPN (21.61±2.55 vs 10.54±1.16, p &lt; 0.05), but did not differ in the PI and nPI groups. The same pattern was obtained for the leptin/ghrelin and leptin/adiponectin ratios. The ghrelin level showed an inverse pattern. It was higher in nPN as compared to PN (3.80±0.78 vs 1.39±0.19, p &lt; 0.05), but did not differ in women with increased BMI. The obtained data demonstrate that the association between the adipokine levels in FF and the IVF outcomes is better in women with normal BMI than in women with increased BMI. Adipokines, which differed among the study groups, were evaluated as prognostic factors in women with normal BMI.The values were calculated at which pregnancy became unlikely: leptin level &lt;15 ng/mL, leptin/ghrelin ratio &lt;9, and leptin/adiponectin ratio &lt;5. For each indicators, the sensitivity and specificity were &gt;88%. The predictive power of these indicators in the clinic using the odds ratio (95% confidence interval) was: leptin level 7.11 (1.23–40.99, p &lt; 0.05), leptin/ghrelin 29.53 (1.53–570.83, p &lt; 0.05), and leptin/adiponectin 7.11 (1.23–40.99, p &lt; 0.05). Limitations, reasons for caution The main limitation of the study is a relative small number of patients, although the differences between the investigated groups were significant. Furthermore, women with low ovarian reserve, age &gt; 40 years, endometriosis or PCOS were not included in the study. Wider implications of the findings: The obtained results indicate the good prospects for using such indicators as the adipokines levels and their ratio in FF to predict IVF outcomes in women with low ovarian reserve, endometriosis and PCOS, but with normal BMI. Chronic inflammatory process in endometrium is an obvious factor of infertility and recurrent pregnancy loss. But uncertainty of its etiology in most of the clinical cases results in complications of diagnostic process of the disease despite the broad spectrum of diagnostic techniques. Shift of the focus from specific infectious agents towards opportunistic pathogens also leads to several complications in interpretation of bacteriological examination results. Moreover, the role of viruses as ethiological factor of chronic endometritis is still not well established. At the same time the capability of invasive diagnostic techniques is always discussed by the clinicians because of additional risks from intervention into uterine cavity. From this point of view treatment of chronic endometritis (CE) and pregravid preparation of women involved in in vitro fertilization algorithm with such pathology requires comprehensive analysis. Routine antibiotic administration, principles of choice of these medications and effectiveness of other therapeutical approaches, for example, physiotherapy techniques, are still controversial. This article highlights modern ideas about chronic endometritis: ethiological factors of the disorder, coincidence with infertility and reproductive failure, diagnostic methods, treatment and pregravid preparation of women with the disease and ways of optimization of patient management for preserving their reproductive potential.
