ERCP (endoscopic retrograde cholangiopancreatography) represents a safe endoscopic procedure and serious complica- tions (perforation, haemorrhage, and acute pancreatitis) are usually uncommon. We present the case of a 38-year-old patient with gallstones in the common biliary duct who developed acute pancreatitis after ERCP. One month later a huge fluid col- lection with necrotic tissue in the right paracolic gutter was found, the fluid being drained by percutaneous drainage under ultrasonographic guiding. The particularity of the case is the post-ERCP pancreatitis, complicated with walled-off necrosis, resolved without surgical intervention by using percutaneous drainage.

AbstractAcute pancreatitis is one of the major gastrointestinal conditions that lead to around 300,000 hospital admissions per year in the United States. While mild inflammation of the pancreas is often managed conservatively, progression of the disease process to necrosis significantly increases the overall morbidity and mortality and often requires surgical or other interventional techniques for management. The purpose of this review is to describe the role of percutaneous drainage for the management of complicated pancreatitis.

This chapter covers core curriculum topics relating to pancreatic disorders. This includes the investigation and management of acute pancreatitis incorporating knowledge on aetiology, staging of acute pancreatitis, complications including pseudocyst and walled off necrosis, nutritional support, and role of imaging and endoscopy. The chapter also covers genetic pancreatic disorders, anatomical variants of the pancreas, and describes the different cell types of the pancreas. These is also an emphasis on the aetiology and complications of chronic pancreatitis including pancreatic exocrine insufficiency and chronic pain. Finally the chapter also covers the presentation, diagnosis and management of benign and malignant pancreatic tumours. Additional curriculum material regarding pancreatic disorders will also be covered in the mock examination chapter.
