AbstractIt is a rare and underdiagnosed entity. The adagium “one only sees what one knows” is certainly true in cases of Blake's pouch cyst, as all types of posterior fossa cysts and cyst-like malformations may present nearly identical on initial imaging studies. Only one case of Blake's pouch cyst has been reported from this country, except for a case in utero, in which a diagnosis of Blake's pouch cyst was made on prenatal ultrasound and later confirmed by MRI. In this report we describe a case of Blake's pouch cyst in a 9-month-old male child along with the principles of diagnosis of Blake's pouch cyst, in combination with literature review. Differentiating Blake's pouch cyst from other posterior fossa cysts and cyst-like malformations and recognizing the accompanying hydrocephalus that are essentially noncommunicating have important implications not only on clinical management but also on genetic counseling, which is unnecessary in case of Blake's pouch cyst.

AbstractIdiopathic systemic capillary leak syndrome (ISCLS) is characterized by a triad of hypotension, hemoconcentration and hypoalbuminemia due to a shift of intravascular fluid and albumin to the extravascular area. We describe a hypovolemic patient with hemoconcentration and hypoalbuminemia who was successfully treated with high-dose intravenous immunoglobulins (IVIG). The purpose of this case report is to discuss the clinical management of idiopathic systemic capillary leak syndrome on the background of relevant literature review.

A Killian-Jamieson diverticulum is an outpouching from the lateral wall of the proximal cervical esophagus. These diverticula are rare and are distinct from the more commonly known Zenker’s diverticulum. Literature regarding Killian-Jamieson diverticula and its suggested management is scarce. The present report describes a patient with symptomatic bilateral Killian-Jamieson diverticula. The patient had both diverticula excised and an esophagomyotomy performed. Following surgery, the patient’s symptoms resolved and he recovered well. A literature review and discussion of the etiology, clinical presentation and radiographic findings of Killian-Jamieson diverticulum follow, as do recommendations for clinical management.

The presentation of an incarcerated appendix within a femoral hernia accounts for 0.5-3.3% of all femoral hernias. It is rarely apparent diagnostically prior to surgery. A 48-year-old female had a delayed presentation with a 3-day history of an irreducible right inguinal swelling. Imaging failed to elucidate an incarcerated appendix, which was found at operation. The patient made a full recovery. The rarity and presentation of this condition is discussed. On literature review it typically is not suspected at operation due to its rarity and the difficulty of interpreting it on examination and imaging.
