238 Background: For patients receiving cancer care, utilization of acute care resources can be frequent and, in many instances, is avoidable. At Thomas Jefferson University (TJU), up to 50% of emergency department (ED) visits for medical oncology patients on active treatment (receiving intravenous or oral chemotherapy within 30 days) may be preventable based on OP-35 criteria. The COVID-19 pandemic drastically altered healthcare delivery, prompting providers and patients to re-evaluate the safety and necessity of acute care. We aimed to evaluate the effect of the COVID-19 pandemic on ED utilization for medical oncology patients at TJU. Methods: We reviewed the total number of visits to the TJU ED for all patients and for medical oncology patients from January 1, to May 31, 2020. We defined the months of January and February as “Pre-COVID” and the months of April and May as “COVID”. We excluded data from March in our analysis. For medical oncology patients, we tallied both the number of patients with an ED visit and total ED visits for each month. We stratified patients by whether or not they were on active treatment. We reviewed the outcome of each ED visit and categorized results as admission (inpatient admission or observation) or discharge. We classified each ED visit as avoidable or unavoidable using OP-35 criteria. Results: In the Pre-COVID months there were 489 total visits by 432 oncology patients; 41% (179) of these patients were on active treatment. During COVID months there were 313 visits by 284 oncology patients; 48% (137) were on active treatment at the time of visit. During COVID, total ED visits decreased by 37%. Visits by medical oncology patients decreased by 35%. For medical oncology patients on active treatment, we observed a 21% reduction in ED visits. In the Pre-COVID months, 38% of oncology patient visits were considerable potentially avoidable and 41% of visits ended with a discharge to home. In comparison, during COVID, 31% of visits were considered potentially avoidable and 35% of visits ended with a discharge to home. Conclusions: We observed a decrease in ED utilization by oncology patients that mirrored decrease in total ED utilization following the COVID-19 outbreak. The decrease was less prominent for patients on active treatment. The percent of visits that were potentially avoidable and the percentage of patients discharged to home from the ED decreased slightly during the COVID period. Further analysis is ongoing to understand factors driving reduction in ED utilization observed immediately following the COVID-19 outbreak.

Conhecer a satisfação dos pacientes e seus cuidadores acompanhados pelos serviços de CP em oncologia. Realizou-se uma revisão integrativa da literatura nas bases de dados Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e PubMed publicados durante o período de janeiro de 2009 a janeiro de 2019, nos idiomas português e inglês utilizando descritores controlados contemplados no DeCS na expectativa de responder à questão norteadora: “Qual o resultado da análise de satisfação dos pacientes adultos com câncer em estágio avançado e seus cuidadores/familiares em relação aos serviços prestados pelos Programas de Cuidados Paliativos?”. De um total de 506 referências nove (n=9) artigos foram selecionados. Com os atendimentos demonstrados nas pesquisas sejam elas internacionais e nacionais sente-se a necessidade de aproximar esses públicos com o objetivo de integralizar CP em nível mundial, padronizar atendimentos e atender as demandas cada vez maiores com qualidade e humanização.Descritores: Cuidados Paliativos, Oncologia, Satisfação do Paciente, Cuidadores. Assessment of satisfaction of patients and their caregivers in relation to palliative care services in oncologyAbstract: Know the satisfaction of patients and their caregivers accompanied by PC services in oncology. With the attendances demonstrated in the research, both international and national, we feel the need to bring these audiences closer together in order to integrate PC worldwide, standardize care and meet the increasing demands with quality and humanization.Descriptors: Palliative Care, Medical Oncology, Patient Satisfaction, Caregivers. Evaluación de la satisfacción de pacientes y sus cuidadores en relación a los servicios de cuidados paliativos en oncologíaResumen: Conocer la satisfacción de los pacientes y sus cuidadores acompañados de servicios de PC en oncología. PURPOSE Telehealth in medical oncology has expanded secondary to the COVID-19 pandemic. However, quantitative research on medical oncology telehealth use shows conflicting results on patient satisfaction, whereas qualitative data are sparse. Our qualitative study aimed to identify the factors influencing patient acceptability of video visits for medical oncology care before and at the onset of the expansion of telehealth because of the COVID-19 pandemic. METHODS Semi-structured interviews were conducted between November 2019 and April 2020 with 20 patients who participated in a telehealth visit with a medical oncology provider at Thomas Jefferson University. RESULTS Of the 20 participants, 13 (65%) were female and 15 (75%) were White, with a mean (standard deviation) age of 60.5 years (11.8). Patients identified convenience, anxiety, COVID-19, and provider preference as positively influencing the acceptability of video visits; however, some patients noted limitations in provider connection, physical examinations, and visit length as disadvantages. Regarding receipt of serious or bad news, some preferred video visits for privacy, immediacy of results, news processing, and family comfort. Others preferred in-person encounters for provider support and the ability to receive written information and in-person referrals. CONCLUSION Patient-perceived factors influencing general acceptability, appropriateness of serious and bad news delivery, and future uses of telehealth were unique to each individual, but shared common themes. Understanding each patient's perspective of telehealth acceptability and tailoring use to their preferences is critical for continued utilization. Further research is needed to understand and address reasons for lack of telehealth uptake among certain patients.
