SummaryA case is described where phototherapy rapidly restored sleep-wake disturbances and concurrent symptoms of delirium in a patient who did not respond to conventional treatment with haloperidol. In this case, the delirious state may have been caused by a change in metabolism as a result of sepsis. Exposition of the patient to 8,000 Lux during 1.5 h for 5 consecutive days was sufficient to suppress all delirious symptoms and to normalize sleep. This result may encourage the setup of systematic studies towards the extent of desynchronization between various rhythmic cycles in delirium and towards the value of phototherapy for the restoration of normal circadian rhythmicity.

Background: Chronic limb-threatening ischemia is a severe form of peripheral artery disease that leads to high rates of amputation and mortality if left untreated. Bypass surgery and antegrade endovascular revascularization through femoral artery access from either side are accepted as conventional treatment modalities for critical limb ischemia. The retrograde pedal access revascularization is an alternative treatment modality useful in specific clinical scenarios; however, these indications have not been well described in literature. This case report highlights the use of retrograde pedal access approach as primary treatment modality in a patient with an extensive comorbidities precluding general anesthesia nor supine positioning. Case Presentation: The patient is a 60-year-old female with multiple severe cardiopulmonary comorbidities presenting with dry gangrene of the right great toe. Her comorbidities and inability to tolerate supine positioning precluded her from receiving open surgery, general anesthesia or monitored sedation, or percutaneous femoral access. Rather, the patient underwent ankle block and retrograde endovascular revascularization via dorsalis pedis artery access without post-operative complications. Discussion: The prevalence of comorbidities related to peripheral artery disease is increasing and with it the number of patients who are not optimal candidates for conventional treatment methods for critical limb ischemia. The retrograde pedal access revascularization as initial treatment modality offers these patients an alternative limb salvaging treatment option.

Objective: Oral ulcers are painful sores that appear in the mouth. Most of them are usually harmless and clear up on their own. Sometimes, they are non-responsive and difficult to manage. This report describes the case of an 80-year-old female patient with chronic oral ulcers of three years duration and which were refractory to conventional therapy. The ulcers were subsequently treated using 3% citric acid as a sole topical agent, applied three times a day that resulted in complete healing of oral ulcers by day 10 of treatment. The findings of this case report suggest that topical citric acid is a possible alternative treatment for chronic oral ulcers unresponsive to conventional treatment.

Purpose. To report the use of infliximab in the rapid stabilization of a case of progressive, bilateral rheumatoid peripheral ulcerative keratitis (PUK) that failed to respond to conventional immunosuppressive therapy.Methods. A single interventional case report.Results. A patient with rheumatoid arthritis presented with bilateral PUK following a 2-month history of ocular discomfort and redness. His systemic prednisolone (PDN) and methotrexate (MTX) were increased and, despite an initial favorable response, bilateral recurrent corneal perforations ensued. Both eyes underwent cyanoacrylate glue repair, amniotic membrane transplantation (AMT), and penetrating keratoplasty (PKP). Recurrence of the disease and bilateral perforations of the second PKP in both eyes prompted administration of intravenous infliximab immediately after the fourth PKP. The disease activity rapidly settled in both eyes, and at eighteen-month followup, after 12 infliximab infusions, the PUK remains quiescent with no further graft thinning or perforation.Conclusion. Infliximab can be used to arrest the progression of severe bilateral rheumatoid PUK in cases that are refractory to conventional treatment.

Oral myiasis is a rare pathology and a risk to the patient's life. Conventional treatment is mechanical removal of the larvae (maggots) one by one, which is painful, distressing, and embarrassing both to the patient and to the dentist. We present a case report of a patient with cerebral palsy that had oral myiasis treated successfully with ivermectin. The use of this antibiotic in humans, its mechanism of action and indications are discussed.

O meningioma é a neoplasia intracraniana mais comum entre cães e gatos, é um tumor benigno que atinge as meninges causando um crescimento anormal da região, comprimindo as estruturas neurais. Este trabalho visa relatar uma ocorrência de meningioma lipomatoso em canino da raça Maltês, seu processo de diagnóstico e suas alternativas de tratamento. Canino, fêmea, 8 anos de idade, começou a apresentar mudanças bruscas de comportamento, como dificuldade para respirar e postura anormal. Foram realizados exames como radiografia, ecocardiografia, hemograma e bioquímica, e o diagnóstico definitivo foi estabelecido através da ressonância magnética do crânio. O tratamento de escolha foi a cirurgia para excisão do tumor através da histopatologia, seguido por radioterapia e corticoides. O tratamento homeopático foi utilizado para minimizar os efeitos da radioterapia e do crescimento acelerado do tumor. Após esse período, o paciente apresentou melhoras no quadro clínico, os medicamentos homeopáticos continuam em uso e têm como prioridade oferecer uma melhor qualidade de vida, tendo em vista a possibilidade de recidiva tumoral. Conclui-se que, tratamentos integrativos e complementares como a homeopatia, mesmo diante de uma doença grave podem auxiliar na qualidade de vida e bem estar do animal que apresentou neoplasia intracraniana e risco de metástases.

ABSTRACTRibbond is a bondable, biocompatible, esthetic, translucent and easy-to-use reinforced ribbon. By virtue of its wide spectrum of intended properties, it enjoys various applications in clinical dentistry. This case report demonstrates usage of Ribbond as a space maintainer, a fixed partial denture with a natural tooth pontic, an endodontic post and cores and a splint material in children.
