Trauma is one of the biggest causes of death in the world. Thousands of people died from trauma each years. Many trauma occur in developing countries or countries with low incomes. The survey carried out showed 90% of trauma occur in developing countries. Outcome head injury patients determined from the initial condition when the patient entered in the ER (Emergency Room). Analysis of the patient's condition will determine the appropriate nursing actions that affect patient outcome. The purpose of this study was to analyze factors associated with outcome head injury patients in ER Prof. Dr. Margono Soekardjo Purwokerto Hospital. This study is a prospective with observational analytic design. The sample in this study amounted to 56 people.The results of Spearman and coefficient contingency indicates that there is a relationship between initial GCS score (p = 0.000) and systolic blood pressure (p = 0.000)with a outcomes of head injury patient. There is no correlation between age (p = 0.478)respiratory rate (p = 0.956) and pulse (.318) with a outcomes of head injury patient. Analysis of logistic regression shows that systolic blood pressure (RR = 6.768) is the dominant factor associated with outcomes of head injury patient. Therefore, the need to improve the management hemodynamic of the patient's, especially blood pressure to prevent bad outcomes. Keywords: outcome, head injury, Emergency Room

Polytrauma is a major cause of mortality and morbidity in both developed and developing countries. The primary goal of this review is to provide a comprehensive overview on current knowledge in the management of pediatric polytrauma patients (PPPs). A database review was conducted based on a search in the Embase, Medline OVID-SP, Web of Science, Cochrane central, and Pubmed databases. Only studies with “paediatric population” and “polytrauma” as criteria were included. A total of 3310 citations were retrieved. Of these, 3271 were excluded after screening, based on title and abstract. The full texts of 39 articles were assessed; further selection left 25 articles to be included in this review. The most crucial point in the management of PPPs is preparedness of the staff and an emergency room furnished with age-appropriate drugs and equipment combined with a systemic approach.

An open trial of the use of amoxycillin injection in the treatment of pyogenic meningitis in children showed similar results to previous trials here of penicillin-chloramphenicol combinations. It is felt that, in developing countries at least, treatment of pyogenic meningitis in children should be with the latter combination, but where rapid and accurate bacteriology is available amoxycillin may be a valuable non-toxic alternative to chloramphenicol with similar activity to ampicillin.

To the Editor.— Greensher et al1 commented on the increasing advice given to emergency room and primary care physicians regarding the use of activated charcoal instead of syrup of ipecac as first-line therapy for ingested toxins. This may be the direction that hospital facilities should be taking but, hopefully, will not undermine the confidence that many practitioners and the public have in syrup of ipecac. Many developing countries take their lead from North America and there are still areas on this continent where immediate emergency room access is impossible.
