Reassessment of the cost-effectiveness of hormone replacement therapy in Sweden: Results based on the Women's Health Initiative randomized controlled trial

Objectives:The purpose of the study is to reassess the cost-effectiveness of hormone replacement therapy (HRT) based on new medical evidence found in the Women's Health Initiative (WHI). Within a model framework using an individual state transition model, the cost-effectiveness of 50- to 60-year-old women with menopausal symptoms is assessed based on a societal perspective in Sweden.Methods:The model has a 50-year time horizon divided into a cycle length of 1 year. The model consists of the following disease states: coronary heart disease, stroke, venous thromboembolic events, breast cancer, colorectal cancer, hip fracture, vertebral fracture, and wrist fracture. An intervention is modeled by its impact on the disease risks during and after the cessation of therapy. The model calculates costs and quality-adjusted life years (QALYs) with and without intervention. The resulting cost per QALY gained is compared with the value of a QALY gained, which is set to SEK 600,000. The model requires data on clinical effects, risks, mortality rates, quality of life weights, and costs valid for Sweden.Results:The cost-effectiveness ratios are estimated at approximately SEK 10,000, which is below the threshold value of cost-effectiveness. On the condition that HRT increases the quality of life weight more than 0.013 units, the therapy is cost-effective.Conclusions:In conclusion, given the new evidence in WHI, there is still a high probability that HRT is a cost-effective strategy for women with menopausal symptoms.

Bioidentical hormone replacement therapy, a form of hormone balancing and treatment that uses hormones identical to the ones naturally produced by the body, is an effective and well-tolerated method of hormone replacement therapy. The Marion Gluck Clinic conducted a small-scale study to assess the effectiveness of compounded bioidentical hormone replacement therapy treatment protocols and their impact on the quality of life of women experiencing symptoms of the menopause. Quality of life was assessed by completing the Greene Climacteric Scale questionnaire before and after bioidentical hormone replacement therapy treatment. Statistical significance of the data was tested using a Student's two-tailed, paired t-test. The results demonstrated a significant improvement of 52% in quality of life after bioidentical hormone replacement therapy treatment. All 21 menopausal symptoms assessed were greatly reduced after BHRT treatment. Evidence is provided showing that bioidentical hormone replacement therapy, performed according to the Marion Gluck Clinic local clinical guidelines, improves quality of life and reduces menopause-associated symptoms in women. In addition, this pilot study paves the way for a future full-scale study, where the authors aim to assess quality of life and safety in a significantly larger number of women.

There is presented the review of domestic and foreign references on the conserved oncological safety of the use of menopausal hormone therapy after treatment for endometrial cancer, cervical cancer, borderline and malignant ovarian tumors, various variants of sarcomas of the uterus, vulva and vaginal cancer. To the opinion of the authors the refusal to prescribe menopausal hormone therapy to patients with oncogynecologic diseases in the anamnesis is usually not justified, the category of patients, to whom hormone replacement therapy is contraindicated, is well described and mentioned in the text. In other cases sex hormones can be used to treat menopausal symptoms and improve the quality of life of patients.
