Abstract The speech perception ability of people with hearing loss can be efficiently measured using phonemic-level scoring. We aimed to develop linguistic stimuli suitable for a closed-set phonemic discrimination test in the Swedish language called the Situated Phoneme (SiP) test. The SiP test stimuli that we developed consisted of real monosyllabic words with minimal phonemic contrast, realised by phonetically similar phones. The lexical and sublexical factors of word frequency, phonological neighbourhood density, phonotactic probability, and orthographic transparency were similar between all contrasting words. Each test word was recorded five times by two different speakers, including one male and one female. The accuracy of the test-word recordings was evaluated by 28 normal-hearing subjects in a listening experiment with a silent background using a closed-set design. With a few exceptions, all test words could be correctly discriminated. We discuss the results in terms of content- and construct-validity implications for the Swedish SiP test.

The Picture Identification Task was developed to estimate the word-recognition performance of nonverbal adults. Four lists of 50 monosyllabic words each were assembled and recorded. Each test word and three rhyming alternatives were illustrated and photographed in a quadrant arrangement. The task of the patient was to point to the picture representing the recorded word that was presented through the earphone. In the first experiment with young adults, no significant differences were found between the Picture Identification Task and the Northwestern University Auditory Test No. 6 materials in an open-set response paradigm. In the second experiment, the Picture Identification Task with the picture-pointing response was compared with the Northwestern University Auditory Test No. 6 in both an open-set and a closed-set response paradigm. The results from this experiment demonstrated significant differences among the three response tasks. The easiest task was a closed-set response to words, the next was a closed-set response to pictures, and the most difficult task was an open-set response. At high stimulus-presentation levels, however, the three tasks produced similar results. Finally, the clinical use of the Picture Identification Task is described along with preliminary results obtained from 30 patients with various communicative impairments.

Subjects were English-speaking adults with hearing impairment. Etiology of hearing loss did not enter into selection. Consonant errors were observed on speech discrimination test lists employing a closed-set response system. Fifteen subjects were employed for the first list and 20 each for the remaining four lists, with an occasional subject serving in more than one group. Confusions between unvoiced and voiced consonants rarely occurred; the /r/ and /l/ were seldom confused with other phonemes; and nasals were seldom confused with non-nasals. Discrimination difficulty was related to both place and manner of articulation.

Deterioration in speech intelligibility, the most common complaint of people with hearing loss, leads to social isolation and dramatically reduces the quality of life. The correction of peripheral hearing loss by hearing aid fitting does not always solve this problem in patients with chronic sensorineural hearing loss (SNHL). That is why in the process of audiological examination of patients with complaints of hearing loss, it is necessary to include methods of speech audiometry, which make it possible to comprehensively assess the functioning of the auditory system. The aim of the study: assessment of speech intelligibility in patients of different ages with SNHL. 94 people from 20 to 74 years of age were examined: 54 patients with SNHL (14 young and 40 elderly) and 40 people with normal hearing thresholds (20 young and 20 elderly). Besides the standard audiological examination, speech audiometry testing was conducted: evaluation of monosyllabic words intelligibility in quiet, binaural rapidly alternating speech test, dichotic digits test, Russian matrix sentence test (RuMatrix). Results: Speech intelligibility in elderly patients with SNHL was shown to be significantly worse than in young patients with a similar degree of hearing loss. The RuMatrix test in noise was proved to be the most sensitive test for comprehensive evaluation of the hearing system functioning.

The Auditec recordings of the CID W-22 monosyllables were used to generate test and retest intelligibility functions on normally hearing listeners and subjects with mild-to-moderate sensorineural hearing loss. The normally hearing subjects were tested with 50-word lists at SPLs ranging from 15 to 50 dB. Lists of 25 words were used with the hearing-impaired group. The functions were analyzed to assess the reliability of threshold (50% point), slope (20%–80% points), and maximum intelligibility (PB max). The 50% point was obtained at 28 dB SPL for the normally hearing listeners and at a sensation level (SL) of 12 dB re spondaic thresholds for the hearing-impaired group. Very stable monosyllabic thresholds were found because 95% of the test-retest values were within 6 dB for both subject groups. Slopes of 4.9% per dB and 2.7% per dB were obtained for the normally hearing and hearing-impaired groups, respectively. Fair reliability was observed; 95% of the test-retest values encompassed a range of ± 1.9% per dB for the normally hearing subjects and ± 1.1% per dB for the hearing-impaired group. Although group slopes provide useful information for selecting the range and step size for generating psychometric functions, the value of routinely obtaining slope on an individual basis has not been demonstrated. Even though the same word lists were used for both test and retest measurements, reliability closely agreed with predicted results based on the binomial theorem. In contrast to the large variability for a single list of 25 words, very stable PB max scores were found when two or three scores were averaged on the plateau of the function.

Criteria for the construction and evaluation of clinical speech discrimination test word lists are reviewed. Particular emphasis is placed on the need for such lists to be appropriate and homogeneous in both range and average level of word difficulty. Individual word difficulty data obtained from a clinical population with discrimination losses are presented for the recorded CID W-22 Auditory Test Series. These data permitted the rearrangement of the 200 W-22 words into eight 25-word lists which promise to be more homogeneous in average level of difficulty. However, the range of word difficulty, of both the original or reconstructed tests, falls short of the optimal distribution for a general-purpose speech discrimination test.

The Modified Rhyme Test (MRT), recorded using natural speech and two forms of synthetic speech, DECtalk and Votrax, was used to measure both open-set and closed-set speech-recognition performance. Performance of hearing-impaired elderly listeners was compared to two groups of young normal-hearing adults, one listening in quiet, and the other listening in a background of spectrally shaped noise designed to simulate the peripheral hearing loss of the elderly. Votrax synthetic speech yielded significant decrements in speech recognition compared to either natural or DECtalk synthetic speech for all three subject groups. There were no differences in performance between natural speech and DECtalk speech for the elderly hearing-impaired listeners or the young listeners with simulated hearing loss. The normal-hearing young adults listening in quiet out-performed both of the other groups, but there were no differences in performance between the young listeners with simulated hearing loss and the elderly hearing-impaired listeners. When the closed-set identification of synthetic speech was compared to its open-set recognition, the hearing-impaired elderly gained as much from the reduction in stimulus/response uncertainty as the two younger groups. Finally, among the elderly hearing-impaired listeners, speech-recognition performance was correlated negatively with hearing sensitivity, but scores were correlated positively among the different talker conditions. Those listeners with the greatest hearing loss had the most difficulty understanding speech and those having the most trouble understanding natural speech also had the greatest difficulty with synthetic speech.

Syllable recognition ability and consonant confusion patterns were evaluated for 38 listeners with mild-to-moderate sensorineural hearing loss using the closed-set Nonsense-Syllable Test (NST). Performance for these materials varies as a function of consonant voicing, the position of the consonant in the syllable, and the accompanying vowel. Scores for listeners with steeply sloping audiometric configurations were consistently poorer than those for listeners with gradually sloping or flat audiograms. Consonant confusion analyses revealed place of articulation errors to be the most frequent, regardless of the listener's audiometric configuration. Analysis of consonant confusion patterns indicates the existence of a systematic relationship between consonant confusions and audiometric configuration. The NST findings are discussed in terms of the test's potential use and are compared to the results of existing confusion analyses.

Otitis media with effusion (OME) often results in hearing loss for children with the condition. In order to provide appropriate and effective audiologic management, it is important to understand the impact of OME on speech recognition ability when hearing loss is present. This study examined the speech recognition abilities of normal-hearing six- and seven-year-old children (n = 12) and adults (n = 12) using monosyllabic words and nonsense syllables presented at two levels of simulated conductive hearing loss characteristic of OME. Average speech recognition scores decreased as the degree of simulated conductive hearing loss increased. Both age groups scored significantly poorer for nonsense syllables than for monosyllabic words. In general, the children performed more poorly than the adults with the exception of the easiest listening condition for word stimuli. Furthermore, children appeared less able than adults to use their knowledge of familiar words to improve performance. These findings suggest that rehabilitative strategies may best be focused on combining familiarization techniques and amplification options.

Sixty children aged 3, 5, and 7 years were tested using a simple up-down adaptive speech threshold procedure. The test stimuli were familiar monosyllabic words presented as a closed set with a picture-pointing response. The results indicate that monosyllabic adaptive speech test (MAST) procedures can be used reliably with children as young as 3 years of age. Thirty of the children also received a different randomization of the same speech stimuli presented at a constant level, equal to their MAST threshold. The results confirmed the accuracy of the MAST estimate of the children's 50% speech threshold. Further support for the validity of the MAST threshold procedure with young children was obtained using a group of 10 children with conductive hearing loss. Their results show a significant correlation between the MAST threshold and pure-tone loss. The data also indicated significant improvement in MAST thresholds over the three age groups investigated. These developmental changes are discussed in terms of a word frequency effect.

The contribution of a two-channel vibrotactile aid (Trill VTA 2/3, AVR Communications LTD) to the audiovisual perception of speech was evaluated in four young children with profound hearing loss using words and speech pattern contrasts. An intensive, hierarchical, and systematic training program was provided. The results show that the addition of the tactile (T) modality to the auditory and visual (A+V) modalities enhanced speech perception performance significantly on all tests. Specifically, at the end of the training sessions, the tactile supplementation increased word recognition scores in a 44-word, closed-set task by 12 percentage points; detection of consonant in final position by 50 percentage points; detection of sibilant in final position by 30 percentage points; and detection of voicing in final position by 25 percentage points. Significant learning over time was evident for all test materials, in all modalities. As expected, fastest learning (i.e., smallest time constants) was found for the AVT condition. The results of this study provide further evidence that sensory information provided by the tactile modality can enhance speech perception in young children.
