Ultrastructure of a Paratesticular Rhabdomyosarcoma

Rhabdomyosarcoma, a malignant tumor of striated muscle, was described first in 1849 by Rokitansky. Since that time, another 150 cases have been documented. In recent years, it has been recognized that the paratesticular location of rhabdomyosarcoma is an important single site, particularly in adolescents. As a combination of surgery and chemotherapy can affect a cure, a definitive diagnosis should be established as early as possible.We report the case of an 18 year old boy, who presented with a scrotal mass, which was removed surgically, with the clinical impression of a hydrocele. On gross examination, the mass measured 4cm. He showed the presence of cystic change,foci of hemorrhage and solid white neoplastic tissue. Sheets of large polygonal cells were noted on histologic examination and there was infiltration of the epididymis.Microscopic examination of Epon embedded material cut at micron and stained with to luidine blue confirmed the presence of a diffuse infiltrate of large polygonal cells. The cells were arranged in sheets, which were separated by irregular fibrous bands.

Controversy continues to exist regarding the routine histologic examining of tonsillectomy specimens. Proponents suggest that among other reasons, missing an important diagnosis such as occult malignancy or granulomatous disease and possible medicolegal consequences argue in favor of routine histologic examination. Others state that we should consider the very low yield of significant histologic findings in routine tonsillectomy specimens and its added cost. We performed a retrospective evaluation of all cases of patients who underwent tonsillectomy with or without adenoidectomy between January 1992 and July 1996. Two hundred eighty-eight charts were evaluable. Group 1 consisted of all tonsillectomy specimens that were subjected to gross examination only. Group 2 consisted of all tonsillectomy specimens that were subjected to gross examination as well as microscopic examination. In group 1, no abnormal gross findings were noted. Group 2 consisted of specimens from an older population with a mean age of 21.6 years. There were 43 patients older than 20 years old in group 2, and in all cases except four the preoperative clinical impression correlated with the microscopic findings. Occult malignancy was found in only one of these four patients. No cases of granulomatous disease were discovered. Overall, no patients except one had any change in postoperative treatment on the basis of the results of microscopic examination. We believe that routine microscopic examination of tonsillectomy specimens results in unnecessary cost and consumption of resources and time. Microscopic examination should be done in only selected cases such as in patients with grossly asymmetric tonsils or in patients with a history of malignancy.

The forensic evidence of hanging is based on the autopsy demonstration of the traces left by the noose or the ligature on the neck, as well as on the histological assessment of the hanging mark vitality. However, the specific topography of haemorrhagic infiltration in the context of the cervical damaged tissues involved in hanging is not known. We carried out an extensive microscopic examination to identify if haemorrhagic infiltration in hanging appeared in specific topographic locations that could have been considered as elective. From 102 victims of suicidal hanging, a skin fragment was sampled from the maximum compression area of the sulcus, including the skin portions immediately above and below it. The sampling was also extended to collect the subcutaneous adipose tissue and part of the striated muscle. A standard histological examination was performed on all the samples, and hematoxylin and Eosin, Weigert’s resorcin-fuchsin and Goldner’s Masson trichrome staining were performed. In all the cases assessed, the microscopic examination allowed the detection of haemorrhagic infiltration mainly in the deepest areas under the sulcus and especially in the deep dermis and at the transition point between the dermis and the subcutaneous adipose tissue, as well as in the context of its supporting connective tissue. Such areas could therefore be considered as regions in which the presence of haemorrhagic infiltration is more likely to be demonstrated. Accordingly, we recommend performing sampling similar to ours and focusing the search for haemorrhagic infiltration as suggested.

Dentinogenic ghost cell tumor (DGCT) is an uncommon locally invasive odontogenic tumor regarded by many as a variant of calcifying odontogenic cyst. The peripheral variant of this clinical rarity appears as a well-circumscribed mass mimicking a nonspecific gingival enlargement. Microscopic appearance of odontogenic epithelium admixed with focal areas of dentinoid formation and sheets of ghost cells giving the definitive diagnosis of dentinogenic ghost cell tumor imply that microscopic examination is compulsory for any gingival mass. Van Gieson histochemical stain further confirmed the nature of dentinoid-like material. A complete workup of a case of peripheral dentinogenic ghost cell tumor is presented in this paper and the current concept as well as the appraisal of literature is presented.

Financial considerations have led to suggestions that routine microscopic evaluation of tonsils and adenoids is neither cost effective nor clinically indicated. However, the possibility of tonsillar lymphoma must be carefully weighed when making institutional policy decisions. One way to find an appropriate algorithm for pathologic examination is to examine the characteristics of biopsy-proved tonsillar lymphomas. To investigate the clinicopathologic characteristics of tonsillar lymphoma, we performed a retrospective analysis of patients who had non-Hodgkin lymphoma (NHL) and large-cell or Burkitt lymphoma involving the tonsils and adenoids and were registered on Pediatric Oncology Group (POG) protocols. Seventy-seven (9%) of 832 POG cases of NHL involved the tonsils and adenoids. Review of the pathology reports available from 29 of these cases revealed that NHL was incidentally discovered by pathologic examination in only 5 cases, all of which had clinical evidence of unilateral tonsillar enlargement or size discrepancy by gross examination. The other 24 cases indicated a clinical suspicion of tonsillar cancer, as judged by a preoperative diagnosis or by a request for frozen-section examination. We conclude that in most cases there is a clinical suspicion of tonsillar NHL at the time of gross examination. With routine cases, we predict that the use of comparative organ weights, a clinical impression of tonsillar asymmetry, and review of clinical history will increase the recognition of tonsillar lymphoma when “gross-only” protocols are employed for specimen handling.

Rhabdomyosarcoma, a malignant tumor of the striated muscle, is highly variable in terms of frequency and type according to age, the presence of this tumor in adults being even rarer. The present case is about oncological management of pleomorphic rhabdomyosarcoma in adults with ICE scheme, obtaining a good oncological response and can be considered a good scheme for tumor relapse.

This work was done to investigate Nocardiosis in cattle in order to evaluate the incidence of nocardiali mastitis in Al- dejiala station, bacteriological examination was conducted on 90 milk samples obtaited from mastitic cows Nocardia asteroides was found in 25 out of 90 (27. 77% mastitic cows. This M.O was recorered in pure culture in 5. cases and as mixed with other microorganisms in 20 cases . The gross examination of the affected udders showed multiple nodular lesions in firm fibrotic udders, some of them formed draining sinus which exuded pus to the exterior multifocal nodules of varying size were seen in the lungs Microscopic examination of the affected  udders revealed pyogranulomatous lesions in different areas of the glandular parenchyma multiple classical granulomatous lesions were also noticed in the lung tissues.

Primary testicular osteosarcoma is an extremely rare type of malignant tumor, and to our knowledge, only two other cases have previously been reported in the medical literature. A 63-year-old man, who presented with a large painless left inguinal and scrotal mass, underwent radical orchiectomy. Pathological examination was suggestive of testiscular osteosarcoma. The patient received no adjuvant therapy and after 12 months of follow-up, the patient remains free of disease.

Accessory spleens are one or more areas of normal ectopic splenic tissue of variable size supplied by the branches of the splenic artery. A 7 year-old mixed breed male dog was fixed for use as a model in anatomy hall. During the abdominal dissection, the gross examination showed a 6.3×3.1×1 cm mass that was situated on the caudodorsal border of the spleen. Its appearance and firm consistency were similar to those of spleen. There was no connection between the spleen and this mass. It was supplied by accessory splenic artery. Microscopic examination of the mass revealed some structures similar to spleen.

Time and experience has proven that striated muscle has only a limited number of reaction patterns. However, the electron microscopic examination has given information about the ultrastructural reaction of muscle fibers and about the pathologic physiology of diseased muscle. Although specific abnormalities are not useful when present individually. When present in a combination they may be diagnostic.

Abstract Bone marrow smears of necrotic tumor cells stained with Romanowsky dyes frequently have a definite reddish color that contrasts so strikingly with the blue color of normal marrow that necrosis may be suspected from gross examination. The bone marrow cells appear "smudged" and are difficult to identify on microscopic examination. Commonly, only a small amount of material is obtainable by ordinary needle aspiration of bone marrow containing necrotic tumor, and all of it is used to make smears. It is therefore important to recognize that bone marrow smears with a reddish gross appearance and in which the cells appear distorted and are difficult to identify may represent necrotic tumor and not an artifact produced by poor techniques.
