Differential Diagnosis of Stuttering for Forensic Purposes

Purpose: This case study demonstrates the application of an assessment protocol for differential diagnosis of psychogenic stuttering, neurogenic stuttering, developmental stuttering, and malingering. Method: A male in his late 30s, accused of armed robbery, was evaluated for stuttering at the request of his defense attorney. The speech assessment included 4 main sections: collection of speech samples, observation in multiple speaking conditions, evaluation of communication attitudes, and consideration of case history and background information. Results: The defendant stuttered severely in all speaking conditions. He demonstrated typical stuttering loci and consistency, but no adaptation. Communication attitudes were typical of people who stutter, but steady, direct eye contact was atypical. His statements about his speech conflicted with reports of outside witnesses. Conclusions: Characteristics were consistent with developmental stuttering and partial malingering. Both psychogenic and neurogenic forms of stuttering were suspected, but mixed results were largely unsupportive. Valuable protocol elements included speech sampling under multiple speaking conditions, careful examination of case history information, and indirect tests of malingering. Further knowledge and research are warranted to improve processes of differential diagnoses among subtypes of developmental, psychogenic, and neurogenic forms of stuttering as well as malingering.

Background: Neurogenic stuttering (NS) is the most frequently occurring acquired form of stuttering in children and adults. This form of stuttering is primarily caused by neurological incidents. Owing to controversies with regard to similarities between developmental stuttering (DS) and NS symptomatology, differential diagnosis is problematic. Differential diagnosis will guide the appropriate management of persons who stutter (PWS).Objectives: The aim of this study was to describe and highlight the characteristics of NS in order to compile a preliminary checklist for accurate diagnosis and intervention.Method: An explorative, applied mixed method, multiple case study research design was followed. Purposive sampling was used to select four participants. A comprehensive assessment battery was compiled for data collection.Results: The results revealed a distinct pattern of core stuttering behaviours in NS, although discrepancies existed regarding stuttering severity and frequency. It was also found that DS and NS can co-occur. The case history and the core stuttering pattern are important considerations during differential diagnosis, as these are the only consistent characteristics in people with NS.Conclusion: It is unlikely that all the symptoms of NS are present in an individual. The researchers scrutinised the findings of this study and the findings of previous literature to compile a potentially workable checklist.

Purpose Misophonia is a sound tolerance disorder condition in certain sounds that trigger intense emotional or physiological responses. While some persons may experience misophonia, a few patients suffer from misophonia. However, there is a dearth of literature on audiological assessment and management of persons with misophonia. The purpose of this report is to discuss the assessment of misophonia and highlight the management option that helped a patient with misophonia. Method A case study of a 26-year-old woman with the complaint of decreased tolerance to specific sounds affecting quality of life is reported. Audiological assessment differentiated misophonia from hyperacusis. Management included retraining counseling as well as desensitization and habituation therapy based on the principles described by P. J. Jastreboff and Jastreboff (2014). A misophonia questionnaire was administered at regular intervals to monitor the effectiveness of therapy. Results A detailed case history and audiological evaluations including pure-tone audiogram and Johnson Hyperacusis Index revealed the presence of misophonia. The patient benefitted from intervention, and the scores of the misophonia questionnaire indicated a decrease in the severity of the problem. Conclusions It is important to differentially diagnose misophonia and hyperacusis in persons with sound tolerance disorders. Retraining counseling as well as desensitization and habituation therapy can help patients who suffer from misophonia.

The focus of this research is to investigate the influence of wharfs on school children. Therefore it will report the findings of the result on the influences of wharfs on school children with specific case on Portee Wharf in Freetown, Sierra Leone in West Africa. In this regard, the introduction describes the research area, stating the statement of the problem, the overall goal and specific objectives that will be attained in this study, justification for selecting the topic, problems to be encountered during the course of carrying out this research and major influences. Besides, an indication of the methods used to investigate the topic will also be highlighted. Moreover, the studies will analyze the actual responses of the respondents of the activities of the wharf on school-going children. It will address the questionnaire in accordance with the following: Background information of respondents, this investigated areas such as sex, age, religion, occupation and tribe; It enquires about the activities of the wharfs, reasons and consequences of children engaged in wharfs and strategies to control problems that may emanate from the wharf. The paper will show the findings, gives the summary, conclusion and recommendations of problems identified while carrying out the research.

Stanley Levy and Charles Kozoll capture much of the complexity of decisionmaking and offer an important contribution to our understanding of this perplexing topic. In an article entitled “Teaching the Congress and Legislative Process Courses,” L. Sandy Maisel found by examining the syllabi of such courses that about half of them involved legislative case study assignments. He suggested that further exploration of these assignments would be appropriate. Since students are so commonly required to do case studies of bills in legislative courses, a description of this type of assignment and discussion of how to implement it would be useful. In this paper, I will cover my use of this type of assignment in a course on the legislative process. In describing this legislative case study requirement, sections on background information, the case study assignment, and student reaction to the assignment, together with a brief conclusion, will be presented.

Objective:To present the case of an 18-year-old collegiate decathlete with a Salter-Harris type I epiphyseal plate fracture of the proximal humerus.Background:A collegiate decathlete was playing flag football and fell on an outstretched arm. He was taken to the emergency room and diagnosed with a type I epiphyseal plate fracture.Differential Diagnosis:AC sprain, dislocation or subluxation, rotator cuff tear, labral tear.Treatment:Active and passive range of motion exercises were completed after two days of immobilization. He then started strengthening exercises and returned to competitive activity in 10 weeks.Uniqueness:Proximal humeral epiphyseal plate fractures are uncommon injuries, especially in athletes over the age of 15.Conclusions:If an accurate diagnosis is made, an appropriate conservative rehabilitation program can be implemented to safely return an athlete to participation without permanent deformity following a type I Salter-Harris fracture.

Thomas McCulloch, Presbyterian minister and educator, founder of Pictou Academy, first President of Dalhousie College 1838-1843, established a museum in Pictou, NS, by 1828, including a bird collection. To McCulloch, the order of the natural world instilled in students principles of a liberal education and a model of society. His first collections were sold, but when McCulloch came to Dalhousie in 1838 he started a new collection, hoping to make it the basis of a provincial museum. In this he was aided by his son Thomas, who had been trained as a taxidermist. The younger McCulloch kept and expanded the collection until his death, after which it passed to Dalhousie College. The current McCulloch Collection, mainly the work of Thomas McCulloch junior, seems to exemplify purposes and practices of 19th century natural history. But research shows that the collection has a hybrid origin and must be viewed with great caution as an historical artifact. This is a case study in the difficulty of interpreting 19th century natural history collections without careful examination of their history.
