Online Simulation Education for Audiometry Training

Purpose: The purpose of this clinical focus article was to describe a new online simulation program for pure-tone audiometry. Method: Fictional but realistic patient profiles and testing environments were created to teach students about hearing screening protocols and pure-tone audiology. The diversity of the demographics of the United States is represented throughout the program. The web app was created using HTML/JS/CSS with a Flask server backend and MySQL database. Results: The program allows students to learn the process of conducting a hearing screening and measuring audiometric thresholds using a web-based virtual clinical audiometer. The virtual audiometer includes standard audiometer features and allows for instruction based on standard guidelines. The diversity of the patients within the simulation program allows for discussions of diversity to be woven throughout the curriculum. Conclusions: The new simulation program is designed for use as a clinical training tool enabling undergraduate and graduate students to actively participate in hearing screening testing and pure-tone audiometry using any web browser. The program is also designed with the intent to improve pedagogical outcomes at the undergraduate and graduate level for communication sciences and disorders education for pure-tone audiometry by providing instructors with content that focuses on the diversity that is represented in the demographics of the United States.

Background: Health disparities have become apparent since the beginning of the COVID-19 pandemic. When observing racial discrimination in healthcare, self-reported incidences, and perceptions among minority groups in the United States suggest that, the most socioeconomically underrepresented groups will suffer disproportionately in COVID-19 due to synergistic mechanisms. This study reports racially-stratified data regarding the experiences and impacts of different groups availing the healthcare system to identify disparities in outcomes of minority and majority groups in the United States. Methods: Studies were identified utilizing PubMed, Embase, CINAHL Plus, and PsycINFO search engines without date and language restrictions. The following keywords were used: Healthcare, raci*, ethnic*, discriminant, hosti*, harass*, insur*, education, income, psychiat*, COVID-19, incidence, mortality, mechanical ventilation. Statistical analysis was conducted in Review Manager (RevMan V.5.4). Unadjusted Odds Ratios, P-values, and 95% confidence intervals were presented. Results: Discrimination in the United States is evident among racial groups regarding medical care portraying mental risk behaviors as having serious outcomes in the health of minority groups. The perceived health inequity had a low association to the majority group as compared to the minority group (OR = 0.41; 95% CI = 0.22 to 0.78; P = .007), and the association of mental health problems to the Caucasian-American majority group was low (OR = 0.51; 95% CI = 0.45 to 0.58; P < .001). Conclusion: As the pandemic continues into its next stage, efforts should be taken to address the gaps in clinical training and education, and medical practice to avoid the recurring patterns of racial health disparities that become especially prominent in community health emergencies. A standardized tool to assess racial discrimination and inequity will potentially improve pandemic healthcare delivery.

As noted in a Summer 2007 EPRI Journal article entitled Running Dry At The Power Plant, “Securing sufficient supplies of fresh water for societal, industrial, and agricultural uses while protecting the natural environment is becoming increasingly difficult in many parts of the United States. Climate variability and change may exacerbate the situation through hotter weather and disrupted precipitation patterns that promote regional drought.” [1] Currently, in the United States, thermoelectric power production accounts for approximately 41% of freshwater withdrawals [2] and 3% of overall fresh water consumption. [3] The Electric Power Research Institute, EPRI, as part of its Technology Innovation (TI) program, is collaborating with Johnson Controls to conduct a feasibility study comparing the performance of a water saving Thermosyphon Cooling Hybrid System (TCHS) with other heat rejection systems for power plant applications. The TCHS employs a sensible heat rejection device, a thermosyphon cooler (TSC) in conjunction with an evaporative heat rejection device, an open cooling tower, to satisfy the annual cooling requirements of a given power plant. By reducing the evaporative heat load, the TCHS can significantly reduce the annual water consumed for cooling while still maintaining peak power plant output on the hottest summer days. Details of the interactive simulation program developed to compare various power plant heat rejection systems on an annual 8,760 hourly basis are discussed. Examples of the types of results and comparisons that can be made from the data obtained from the simulation program are presented.

ABSTRACT Background International medical graduates (IMGs) have significant exposure to clinical training in their home country, which provides opportunity for international comparison of training experiences. One relevant IMG population is Japanese physicians who have completed some training before entering residency programs in the United States and desire to improve medical education in Japan. Objective We examined Japanese IMGs' perceptions of the respective attributes of residency in the United States and Japan. Methods Individual semistructured interviews were conducted with 33 purposively sampled Japanese IMGs who had completed training. We used exploratory thematic analysis, iterative data collection, and thematic analyses with constant comparison. Results Comments were organized into 3 categories: (1) attributes of US residency preferable to Japanese residency; (2) attributes of residency training with no clear preference for the US or Japanese systems; and (3) attributes of Japanese residency preferable to US residency. Within each category, we matched themes to residency program requirements or culture of medical training. Main themes include high regard for Accreditation Council for Graduate Medical Education (ACGME) Common Program Requirements, with emphasis on efficient achievement of clinical competency through graded responsibility, contrasted with preference expressed for a Japanese training culture of increased professional commitment facilitated by a lack of work hour limits and development of broad clinician skills, including bedside procedures and radiology interpretation. Conclusions Japanese training culture contrasts with a US model that is increasingly focused on work-life balance and associated compartmentalization of patient care. These findings enhance our understanding of the global medical education landscape and challenges to international standardization of training.

Objective: This paper examines the experiences of music therapy students throughout their clinical training. Three surveys inquired about: 1) the perception from both interns and supervisors as to interns’ needs, 2) interns’ preparedness, their skills, their priorities when choosing an internship, and whether their expectations for training were met (with comparisons between American and International respondents), and 3) satisfaction with clinical training. Method: Three separate surveys were distributed. The first survey’s respondents included pre-interns ( n = 19) and internship supervisors (n = 14) who had completed their training in the Great Lakes Region of the United States. The second survey’s respondents included American interns (n = 50), American professionals (n = 353), International interns (n = 12), and International professionals (n = 50). Respondents for the third survey included professional music therapists who completed their curriculum in the United States and held the MT-BC professional credential (N = 777). Results: Some differences between interns’ and supervisors’ perceptions of the interns’ needs were found in Survey 1; significant differences were found between the preparedness and strengths/weaknesses between groups in Survey 2; and Survey 3 found general satisfaction with training with some areas respondents felt needed improvement. Conclusions: While there is overall satisfaction with training for music therapists, there are inconsistencies in students’ experiences in, and perceptions of, their training.

According to the CDC, over three thousand people die every year from drowning in the United States. Many of these fatalities are preventable with properly trained lifeguards. Traditional lifeguard training relies on videos and mock rescues. While these methods are important, they have their shortcomings. Videos are static and do not build muscle memory. Mock rescues are labor-intensive and potentially put others in danger. Virtual reality (VR) can be used as an alternative training tool, building muscle memory in a fully controlled and safe environment. With full control over variables such as weather, population, and other distractions, lifeguards can be better equipped to respond to any situation. The single most important aspect of life guarding is finding the victim. This head rotation skill can be practiced and perfected in VR before guards ever get onto the stand. It also allows guards to practice in uncommon but nevertheless dangerous conditions such as fog and large crowds. VR also allows the user to get immediate feedback about performance and where they can improve.
