Contrast Sensitivity and Visual-Field Thresholds in Patients with Thyroid Optic Neuropathy

Thyroid optic neuropathy is one of the most troubling complications of endocrine ophthalmopathies. It is related to the degree of extraocular muscle swelling in the apex of the orbit. The purpose of this study was to investigate contrast sensitivity and visual-field thresholds in patients with thyroid optic neuropathy. We examined twenty-two patients aged 29 – 63 years (mean 45.3 years). The control group consisted of fifteen healthy persons of similar age. Contrast sensitivity was measured by means of Volkov's charts (sinusoidal gratings) at eight spatial frequencies from 17.5 to 0.46 cycles deg−1. The visual field was investigated with a static automatic perimeter (Allgan Humphrey Field Analyzer) by means of the central 30-2 threshold test. All patients underwent a complete ophthalmological examination including best corrected Snellen visual acuity, fundus copy, and proptosis measurement with the Hertel exophthalmometer. The mean proptosis of patients was 19.4 mm. Fifteen of the patients had decreased visual acuity. Contrast sensitivity at low spatial frequencies was significantly reduced in the patients. It was established that a reduction of visual-field threshold accompanies the decrease of visual acuity. Our results show that contrast sensitivity and visual-field threshold testing are very sensitive at detecting early optic neuropathy and may be a useful means of following patients after treatment.

Purpose. Long-term evaluation of the visual refractive outcomes and the quality of life after implantation of the WIOL-CF (Medicem, Czech Republic) in both eyes. Design. retrospective, nonrandomized noncomparative case series. Methods. 50 eyes of 25 patients, including 11 women (44%) and 14 men (56%). The age range of the patients was 38 to 77 years (mean age 55.48 ± 10.97 years). All patients underwent bilateral implantation of the WIOL-CF. Exclusion criteria were previous ocular surgeries except for cataract surgery and refractive lens exchange, irregular corneal astigmatism of >1.0 diopter, and ocular pathologies or corneal abnormalities. Postoperative examinations were performed at 14 days and 3, 6, 12 months of surgery; the last follow-up was between 24 and 36 months after the procedure. All exams included manifest refraction, monocular uncorrected visual acuity (UCVA) and distance-corrected visual acuity (DCVA) in 5 m (Snellen), monocular uncorrected visual acuity in 70 cm and 40 cm (Jeager) and binocular UCVA, DCVA in 5 m, 70 cm, and 40 cm, binocular contrast sensitivity (CS) under photopic conditions, binocular defocus curves, high-order aberrations, quality-of-vision VF-14 questionnaire, and spectacle independence. Results. Significant improvement in monocular visual acuity at all distances was demonstrated; the mean postoperative spherical equivalent was 0.32 ± 0.45D. The postoperative means of binocular distance UCVA and BCVA were also improved ( p  < .001) and so were the mean uncorrected intermediate VA (2.053 ± 1.268) and near uncorrected VA (2.737 ± 1.447). There was a significant improvement in contrast sensitivity at all spatial frequencies and higher-order aberration, compared to preoperative results. Conclusions. The evaluation of a WIOL-CF showed good distance, intermediate, and near visual acuity. Contrast sensitivity increased after surgery in all spatial frequencies. Patient satisfaction was high despite some optical phenomena. The rate of postoperative spectacle independence also turned out high. Financial Disclosure. This study was conducted to detennine if contrast sensitivity could predict age-related differences in the ability to discriminate simple road signs, as these differences have not been predicted by Snellen visual acuity. Contrast sensitivity, Snellen visual acuity, and discrimination distances for projected images of highway signs were measured for 7 older observers, ages 55 to 79, and 13 younger observers, ages 19 to 30. All subjects had 20/20 visual acuity or better, but the older group had significantly lower contrast sensitivity than did the younger group at three spatial frequencies: 3, 6, and 12 cycles/deg of visual angle. The older group required a significantly larger sign symbol in order to determine if it denoted a + or T intersection. Correlations between measures showed that highway-sign discrimination distance was significantly related to contrast sensitivity at two spatial frequencies, 1.5 and 12 cycles/deg, but discrimination distance was not related to visual acuity. Implications for highway-sign design and driver vision standards are discussed.

Purpose: To confirm the influence of multilevel metabolic disturbance of insulin dependent diabetes mellitus (IDDM) on the vision even before the onset of the other changes routinely evaluated by ophthalmologists. Methods: Contrast sensitivity functions (CSFs) were estimated using the VCTS 6500 board. The standardised measurement procedure was performed. The value of the threshold contrast sensitivity was obtained for five spatial frequencies (1.5 - 3 - 6 - 12 - 18 c/deg). Other data was collected (duration of diabetes, BCVA, funduscopy, fluoresceine angiography, HbA1C). The study group consisted of 48 IDDM patients (94 eyes) without diabetic retinopathy and with Snellen BCVA > 1.0. The control group (56 normals, 98 eyes) was age and BCVA matched. Results: Highly statistically significant decrease of the CSFs in all spatial frequencies in the study group was obtained. Correlation between duration of the diabetes and impaired degree of CSFs was present in the middle spatial frequency. No significant changes in CSFs were found among patients with pathological value of glycated hemoglobin HbA1c (>7.8 %). Conclusions: If compared with routinely used Snellen visual acuity, the CSFs are more complex descriptors of the subjects vision abilities. IDDM has an influence on these sensitive functions, especially during examination in the middle spatial frequency of 6 and 12 c/deg, before disturbing visual acuity and before changes in the retinal morphology. Decrease of CSFs was influenced mainly by the patients’ age and partially (in the middle spatial frequency) by the IDDM duration.

Purpose. To analyse visual field (VF) defects obtained using semiautomated kinetic perimetry (SKP) in patients suffering from Leber hereditary optic neuropathy (LHON). Methods. Twenty-two eyes of eleven consecutive LHON male patients with confirmed mitochondrial 11778G>A DNA mutation were prospectively examined with the V4e stimulus of SKP in both eyes. The mean time after the onset of LHON was one year. The area of obtained isopters was measured in square degrees (deg2). Additionally, static automated perimetry (SAP) within 30° was performed. Results. Visual acuity ranged from counting fingers to 50 cm to 0.4. VFs obtained with SKP showed central scotomas in 18 eyes (82%); the periphery of the VF in these eyes remained intact. The mean area of central scotoma was 408.8 deg2, and the mean area of the peripheral VF was 12291.1 deg2; SAP also revealed central scotoma in these patients. In four eyes (18%) with the worst visual acuity, only the residual central island of VF was found using SKP (mean area 898.4 deg2). SAP was difficult to obtain in these patients. Conclusions. SKP provides additional clinical information in regard to the visual function of LHON patients. SKP enables the quantification of the area of central scotoma, preserved peripheral VF, and residual central island of vision. Using V4 stimulus is especially useful in LHON patients with poor visual acuity, when SAP is difficult to obtain.

AIM: To assess the efficacy, safety, stability and predictability of an implantable Phakic contact lens in patients with stable keratoconus. METHODS: The uncorrected and the best corrected visual acuity, defocus curve, contrast sensitivity, refraction and possible side effects were measured in 14 patients after utilizing implantable phakic contact lens (IPCL) to correct refractive errors. The result was assessed for more than 6mo. RESULTS: The mean preoperative spherical equivalent (SE) and astigmatism got changed from -6.94±2.79 DS and -4.24±1.42 DC to -0.23±0.43 DS and -1.05±0.49 DC, respectively at the last examination after 6mo. Before the preoperative the mean Snellen visual acuity was 0.18±0.10 logMAR. The mean uncorrected and the best corrected visual acuity got changed to 0.13±0.10 logMAR and 0.05±0.15 logMAR, respectively in 6mo. The mean safety indices were 1.11. There was no loss of visual acuity in any of the eyes and 22 eyes (78.5%) gained one or more lines. Twenty eyes (71.4%) were within 0.50 ?D and 27 (96.42%) were within ±1.00?D of the desired SE refraction. There was a change in manifest refraction of -0.23±0.43 (range from -1.00 to +0.75) from the first week of surgery to 6mo. Contrast sensitivity got improvement value at 3 per degree (P<0.005) after Toric IPCL implantation. The total 6mo corneal endothelial cell loss (ECL) was <5%. Intraocular pressure (IOP) was 11.32±2.28 mm Hg after 6mo. CONCLUSION: The clinical consequences of the present study exhibit the efficacy, safety, and predictability of Toric implantable Phakic contact lens in the correction of myopia and myopic astigmatism related with stable keratoconus.

Objective.To assess the dynamics of structural-functional markers in patients with combined pathology: primary open-angle glaucoma (POAG) and age-related macular degeneration (AMD) with the background of lutein-containing drugs at long-term follow-up.Patients and Methods.The period of research was from November 2016 to January 2018; A parallel prospective study of patients (88 eyes), mean age 67.81 ± 8.41 (M ± σ); 67.64 (63.14; 73.08) (Me, Q25%, Q75%) years with combined pathology POAG and AMD (AREDS I and II). Patients were divided into 2 groups, the first group — a study group received Retinorm, by 1 capsule 3 times a day during a meal for 1 year, a second control group of 17 patients (32 eyes) did not receive lutein-containing medicines during the observation period. The study group consisted of 29 patients (55 eyes). There were 7 males and 22 females among them. The control group consisted of 19 patients (33 eyes), 2 males and 17 females among them. Visual acuity, standard automatic computer perimetry, optical coherence tomography, determination of spatial contrast sensitivity was carried out in all patients at the beginning of the study and after 1 year.Results.The maximum correlated visual acuity (MCVA) according to the table ETDRS increased in the study group and decreased in the control group, 95% the confidence interval for the changes in the MCVA: 0.1813636; 1.018182; 1.927273 и –3.878788; –2.393939; –0.8477273 (optotypes), respectively (W = 499.5; p = 0,0002919). Statistically significant changes in the thickness of the layer of nerve fibers of the retina (RNFL) neither in the middle nor in the sectors was revealed, and the thickness of the retina in oval foveal zone by sector: upper, temporal and lower statistically significantly decreased in the control group, while remaining unchanged in the observation group (р < 0.05). Spatial contrast sensitivity (SCS) after 1 year of use Retinorm remained at the same level in the control group, while there was a statistically significant increase in spatial contrast sensitivity in the main group, especially at high spatial frequencies is observed (р < 0.05).Conclusion.The results of the study indicate a positive effect of Retinorm used for 1 year. It was reflected in the improvement visual acuity in the distance, improvement of spatial contrast sensitivity, especially at high spatial frequencies, and the preservation of structural retinal indices both in the foveal zone and peripapillary.

Abstract Purpose To investigate the relationship of ocular blood flow (via arteriovenous passage time, AVP) and contrast sensitivity (CS) in healthy as well as normal tension glaucoma (NTG) subjects. Design Mono-center comparative prospective trial Methods Twenty-five NTG patients without medication and 25 healthy test participants were recruited. AVP as a measure of retinal blood flow was recorded via fluorescein angiography after CS measurement using digital image analysis. Association of AVP and CS at 4 spatial frequencies (3, 6, 12, and 18 cycles per degree, cpd) was explored with correlation analysis. Results Significant differences regarding AVP, visual field defect, intraocular pressure, and CS measurement were recorded in-between the control group and NTG patients. In NTG patients, AVP was significantly correlated to CS at all investigated cpd (3 cpd: r =  − 0.432, p< 0.03; 6 cpd: r =  − 0.629, p< 0.0005; 12 cpd: r =  − 0.535, p< 0.005; and 18 cpd: r =  − 0.58, p< 0.001), whereas no significant correlations were found in the control group. Visual acuity was significantly correlated to CS at 6, 12, and 18 cpd in NTG patients (r =  − 0.68, p< 0.002; r =  − 0.54, p< .02, and r =  − 0.88, p< 0.0001 respectively), however not in healthy control patients. Age, visual field defect MD, and PSD were not significantly correlated to CS in in the NTG group. MD and PSD were significantly correlated to CS at 3 cpd in healthy eyes (r = 0.55, p< 0.02; r =  − 0.47, p< 0.03). Conclusion Retinal blood flow alterations show a relationship with contrast sensitivity loss in NTG patients. This might reflect a disease-related link between retinal blood flow and visual function. This association was not recorded in healthy volunteers.

Background/aimsTo evaluate the time needed for patients with Boston type 1 keratoprosthesis (KPro) to reach their best-corrected visual acuity (BCVA) and all contributing factors.MethodsWe retrospectively reviewed 137 consecutive eyes from 118 patients, measured how long they needed to reach their BCVA and looked at factors that might affect this time duration including patient demographics, ocular comorbidities and postoperative complications.ResultsThe mean follow-up was 5.49 years. The median time to BCVA postoperatively was 6 months, with 47% of patients achieving their BCVA by 3 months. The mean best achieved logMAR visual acuity was 0.71, representing a gain of 6 lines on the Snellen visual acuity chart. Postoperative glaucoma, retroprosthetic membrane (RPM) and endophthalmitis prolonged this duration. We found no correlation between the following factors and time to BCVA: gender, age, indication for KPro surgery, primary versus secondary KPro, number of previous penetrating keratoplasties, previous retinal surgery, intraoperative anterior vitrectomy and preoperative glaucoma.ConclusionIn our retrospective cohort, the majority of subjects reached their BCVA between 3 and 6 months after KPro implantation. This duration was significantly prolonged by the development of postoperative glaucoma, RPM and endophthalmitis.

OBJECT Visual impairments are the most common objective manifestations of suprasellar lesions. Diffusion tensor imaging (DTI) is a noninvasive MRI modality that depicts the subcortical white matter tracts in vivo. In this study the authors tested the value of visual pathway tractography in comparison with visual field and visual acuity analyses. METHODS This prospective study consisted of 25 patients with progressive visual impairment due to suprasellar mass lesions and 6 control patients with normal vision without such lesions. Visual acuity, visual field, and the optic fundus were examined preoperatively and repeated 1 week and 3 months after surgery. Visual pathway DTI tractography was performed preoperatively, intraoperatively immediately after tumor resection, and 1 week and 3 months after surgery. RESULTS In the control group, pre- and postoperative visual status were normal and visual pathway tractography revealed fibers crossing the optic chiasm without any alteration. In patients with suprasellar lesions, vision improved in 24 of 25. The mean distance between optic tracts in tractography decreased after tumor resection and detectable fibers crossing the optic chiasm increased from 12% preoperatively to 72% postoperatively 3 months after tumor resection, and undetectable fibers crossing the optic chiasm decreased from 88% preoperatively to 27% postoperatively 3 months after tumor resection. Visual improvement after tumor removal 1 week and 3 months after surgery was significantly correlated with the distance between optic tracts in intraoperative tractography (p < 0.01). CONCLUSIONS Visual pathway DTI tractography appears to be a promising adjunct to the standard clinical and paraclinical visual examinations in patients with suprasellar mass lesions. The intraoperative findings, in particular the distance between optic tract fibers, can predict visual outcome after tumor resection. Furthermore, postoperative application of this technique may be useful in following anterior optic pathway recovery.

Introduction: Refractive error is a common cause of amblyopia. Objective: To determine prevalence of amblyopia and the pattern and the types of refractive error in children with amblyopia in a tertiary eye hospital of Nepal. Materials and methods: A retrospective chart review of children diagnosed with amblyopia in the Nepal Eye Hospital (NEH) from July 2006 to June 2011 was conducted. Children of age 13+ or who had any ocular pathology were excluded. Cycloplegic refraction and an ophthalmological examination was performed for all children. The pattern of refractive error and the association between types of refractive error and types of amblyopia were determined. Results: Amblyopia was found in 0.7 % (440) of 62,633 children examined in NEH during this period. All the amblyopic eyes of the subjects had refractive error. Fifty-six percent (248) of the patients were male and the mean age was 7.74 ± 2.97 years. Anisometropia was the most common cause of amblyopia (p < 0.001). One third (29 %) of the subjects had bilateral amblyopia due to high ametropia. Forty percent of eyes had severe amblyopia with visual acuity of 20/120 or worse. About twothirds (59.2 %) of the eyes had astigmatism. Conclusion: The prevalence of amblyopia in the Nepal Eye Hospital is 0.7%. Anisometropia is the most common cause of amblyopia.
