Background: Adults with severe and profound hearing loss tend to be long-term, full-time users of amplification who are highly reliant on their hearing aids. As a result of these characteristics, they are often reluctant to update their hearing aids when new features or signal-processing algorithms become available. Due to the electroacoustic constraints of older devices, many severely and profoundly hearing-impaired adults continue to wear hearing aids that provide more low- and mid-frequency gain and less high-frequency gain than would be prescribed by the National Acoustic Laboratories’ revised formula with profound correction factor (NAL-RP). Purpose: To investigate the effect of a gradual change in gain/frequency response on experienced hearing-aid wearers with moderately severe to profound hearing loss. Research Design: Double-blind, randomized controlled trial. Study Sample: Twenty-three experienced adult hearing-aid users with severe and profound hearing loss participated in the study. Participants were selected for inclusion in the study if the gain/frequency response of their own hearing aids differed significantly from their NAL-RP prescription. Participants were assigned either to a control or to an experimental group balanced for aided ear three-frequency pure-tone average (PTA) and age. Intervention: Participants were fitted with Siemens Artis 2 SP behind-the-ear (BTE) hearing aids that were matched to the gain/frequency response of their own hearing aids for a 65 dB SPL input level. The experimental group progressed incrementally to their NAL-RP targets over the course of 15 wk, while the control group maintained their initial settings throughout the study. Data Collection and Analysis: Aided speech discrimination testing, loudness scaling, and structured questionnaires were completed at 3, 6, 9, 12, and 15 wk postfitting. A paired comparison between the old and new gain/frequency responses was completed at 1 and 15 wk postfitting. Statistical analysis was conducted to examine differences between the experimental and control groups and changes in objective performance and subjective perception over time. Results: The results of the study showed that participants in the experimental group were subjectively accepting of the changes to their amplification characteristics, as evidenced by nonsignificant changes in the ratings of device performance over time. Perception of loudness, sound quality, speech intelligibility, and own voice volume did not change significantly throughout the study. Objectively, participants in the experimental group demonstrated poorer speech discrimination performance as the study progressed, although there was no change in objective loudness perception. According to the paired comparison, there was an overall subjective preference for the original gain/frequency response among all participants, although participants in the experimental group did show an increase in preference for the NAL-RP response by the end of the study. Conclusions: Based on the findings of this study, we suggest that undertaking a gradual change to a new gain/frequency response with severely and profoundly hearing-impaired adults is a feasible procedure. However, we recommend that clinicians select transition candidates carefully and initiate the procedure only if there is a clinical reason for doing so. A validated prescriptive formula should be used as a transition target, and speech discrimination performance should be monitored throughout the transition.

AbstractThis article provides a review of the current literature on the topic of frequency lowering hearing aid technology specific to the treatment of severe and profound levels of hearing impairment in child and adult listeners. Factors to consider when assessing listener candidacy for frequency lowering technology are discussed. These include factors related to audiometric assessment, the listener, the type of hearing aid technology, and the verification and validation procedures that can assist in determining candidacy for frequency lowering technology. An individualized candidacy assessment including the use of real-ear verification measures and carefully chosen validation tools are recommended for listeners requiring greater audibility of high-frequency sounds, when compared with amplification via conventional hearing aid technology.

ABSTRACT Purpose: to investigate speech recognition in silence and in noise in subjects with unilateral hearing loss with and without hearing aids, and to analyze the benefit, self-perception of functional performance, satisfaction and the use of hearing aids in these subjects. Methods: eleven adults with unilateral, mixed and sensorineural, mild to severe hearing loss participated in this study. Speech recognition was evaluated by the Brazilian Portuguese sentences lists test; functional performance of the hearing was assessed by using the Speech Spatial and Qualities of Hearing Scale questionnaire; satisfaction was assessed by the Satisfaction with Amplification in Daily Life questionnaire, both in Brazilian Portuguese; and to assess the use of hearing aids, the patient's report was analyzed. Results: the adaptation of hearing aids provided benefits in speech recognition in all positions evaluated, both in silence and in noise. The subjects did not report major limitations in communication activities with the use of hearing aids. They were satisfied with the use of sound amplification. Most of the subjects did not use hearing aids, effectively. The discontinuity of hearing aids use can be justified by the difficulty on perceiving participation’s restriction caused by hearing loss, as well as the benefit of the hearing aid, besides the concern with batteries’ costs and aesthetic aspects. Conclusion: although showing benefits in speech recognition, in silence and in noise, and satisfaction with sound amplification, most subjects with unilateral hearing loss do not effectively use hearing aids.

The present study examined the phenomenon of acclimatization in persons with a severe-to-profound hearing loss. A secondary purpose was to examine the efficacy of a digital nonlinear power hearing aid that has a low compression threshold with expansion for this population. Twenty experienced hearing aid users wore the study hearing aids for three months and their performance with the study hearing aids was evaluated at the initial fitting, one month, and three months after the initial fitting. Performance of their current hearing aids was also evaluated at the initial fitting. Speech recognition testing was conducted at input levels of 50 dB SPL and 65 dB SPL in quiet, and 75 dB SPL in noise at a +10 SNR. Questionnaires were used to measure subjective performance at each evaluation interval. The results showed improvement in speech recognition score at the one-month evaluation over the initial evaluation. No significant improvement was seen at the three-month evaluation from the one-month visit. In addition, subjective and objective performance of the study hearing aids was significantly better than the participants' own hearing aids at all evaluation intervals. These results provided evidence of acclimatization in persons with a severe-to-profound hearing loss and reinforced the precaution that any trial of amplification, especially from linear to nonlinear mode, should consider this phenomenon.

Subjects with profound postlingual hearing loss completed the Hearing Performance Inventory (HPI) during the course of their hearing aid evaluations. Comparisons of responses to the HPI were made for (a) subjects who wore hearing aids versus subjects who did not, and (b)hearing aid users with losses greater than 100 dB versus users with losses between 80-100 dB. The former set of comparisons indicated consistently superior performance for the aided group, and the latter set indicated consistently superior performance for the 80-100 dB group. The HPI may be a valuable tool in hearing aid considerations.

Background: Universal Newborn Hearing Screening (UNHS) was introduced in Kansas in 1999. Prior to UNHS a small percentage of newborns were screened for and identified with hearing loss. Purpose: The purpose of this study was to determine the effects of UNHS on a local early intervention (EI) program for young children with hearing loss. Research Design: This was a retrospective study based on the chart review of children enrolled in the EI program during target years before and after the establishment of UNHS. Study Sample: Charts for 145 children were reviewed. Data Collection and Analysis: The chart review targeted the following aspects of the EI program: caseload size, percentage of caseload identified by UNHS, age of diagnosis, age of enrollment in EI, degree of hearing loss, etiology of hearing loss, late onset of hearing loss, age of hearing aid fit, percentage of children fit with hearing aids by 6 mo, percentage of children with profound hearing loss with cochlear implants, and percentage of children with additional disabilities. Results: Changes in the EI program that occurred after UNHS were increases in caseload size, percentage of caseload identified by UNHS, percentage of children fit with hearing aids by 6 mo of age, and percentage of children with profound hearing loss with cochlear implants. There were decreases in age of diagnosis, age of enrollment in EI, and age of hearing aid fit. Before UNHS, the majority of children had severe and profound hearing loss; after UNHS there were more children with mild and moderate hearing loss. The percentage of known etiology and late-onset hearing loss was approximately the same before and after UNHS, as was the percentage of children with additional disabilities. Conclusion: UNHS had a positive impact on caseload size, age of diagnosis, age of enrollment in EI, and age of hearing aid fit. The percentage of the caseload identified in the newborn period was about 25% before UNHS and over 80% after its implementation. After UNHS, the EI caseload included as many children with mild and moderate hearing loss as with severe and profound loss. By the last reporting year in the study (academic year 2005–2006) all children with profound hearing losses had cochlear implants.

Background and Aim: Parents' evaluation of aural/oral performance of children (PEACH) and teachers' evaluation of aural/oral performance of children (TEACH) questionnaires are used to assess the behaviors of hearing-impaired children in real-life situations. This study aims to compare the scores of PEACH and TEACH in children with severe-to-profound sensorineural hearing loss (SNHL) using hearing aids. Methods: This is a double-blind two-period crossover study on 21 children aged 9-72 months with severe-to-profound SNHL using hearing aids. There were two 6-week periods of fitting Phonak Naida Venture SP hearing aids using the fifth version of the Desired Sensation Level (DSL v5) and the National Acoustics Laborato­ries’ nonlinear fitting procedure (NAL-NL2) pre­scriptions. At the end of each trial, the PEACH and TEACH questioners were completed through an interview with the parents and teachers, res­pectively. Results: There was a strong correlation between the PEACH and TEACH in total and subscale scores. There was no significant difference bet­ween the results of DSL v5 and the NAL-NL2 prescriptions for the total score and subscale scores of PEACH and TEACH. Conclusion: The PEACH score has a strong correlation with the TEACH score. These ques­tionnaires are useful tools for indirectly assess­ment of hearing-impaired children’s communi­cation skills. The DSL v5 and the NAL-NL2 prescriptions make no significant difference in the performance of children with severe-to-profound SNHL. Objective: The objective of this research is to propose an Android-based digital hearing aid signal processing algorithm with following key features:(1) Regenerated audio match the patient-specific pattern of hearing loss, (2) noise reduction, and (3) provide flexibility to the users.Methods: The proposed signal processing algorithm is designed based on the specific hearing loss of the hearing disorder patient using inverse Fouriertransform; besides, noise reduction feature is included in the digital algorithm design as well. Proposed digital algorithm has been implemented intoan Android-based smartphone and its performance has been tested under real-time condition.Results: Simulation results show that the frequency response of the proposed digital hearing aid signal processing algorithm is in agreement withthe initial theoretical design that was carried out based on the hearing impaired patient’s audiogram. The proposed algorithm has been implementedin the Android-based smartphone and tested in real time. Results show that most of the patients are satisfied with the regenerated audio quality.According to patient’s comments, the regenerated audio is clear and the users are allowed to control the volume level. Besides, no obvious hearinglatency can be detected.Conclusion: Audio signals generated by the proposed digital signal processing algorithm show similar audio signal frequency response in boththeoretical design and MATLAB simulation results. The only difference between the design and simulation results is the amplification levels. Theproposed algorithm provides flexibility to the users by allowing them to choose the desired amplification level. In real-time testing, the proposedAndroid-based digital hearing aid is able to reduce noise level from the surrounding and the output processed speech match the patient-specifichearing loss.

This study aimed to determine the low- and high-frequency compression ratios of a fast-acting device that were preferred by people with moderately severe to profound hearing loss. Three compression ratios (1:1, 1.8:1, and 3:1) were combined in the low and high frequencies to produce nine schemes that were evaluated pair-wise for three weeks in the field using an adaptive procedure. The evaluation was performed by 21 experienced hearing aid users with a moderately severe to profound hearing loss. Diaries and an exit interview were used to monitor preferences. Generally, the subjects preferred lower compression ratios than are typically prescribed, especially in the low frequencies. Specifically, 11 subjects preferred linear amplification in the low frequencies, and 14 subjects preferred more compression in the high than in the low frequencies. Preferences could not be predicted from audiometric data, onset of loss, or past experience with amplification. The data suggest that clients with moderately severe to profound hearing loss should be fitted with low-frequency compression ratios in the range 1:1 to 2:1 and that fine-tuning is essential. Este estudio trató de determinar las tasas de compresión de alta y baja frecuencia de un dispositivo de acción rápida, que resultara preferido por personas con hipoacusias moderadamente severas a profundas. Se combinaron tres tasas de compresión (1:1, 1.8:1, y 3:1) en las frecuencias graves y agudas para producir nueve esquemas que fueron evaluados en el campo, en pares, durante tres semanas, utilizando un procedimiento de adaptación. La evaluación fue realizada por 21 usuarios experimentados de audífono con hipoacusias moderadamente severas a profundas. Se usaron diarios y un cuestionario final para monitorear las preferencias. Generalmente, los sujetos prefirieron menores tasas de compresión de lo que típicamente se prescribe, especialmente en las bajas frecuencias. Específicamente, 11 sujetos prefirieron la amplificación lineal en las frecuencias graves y 14 sujetos prefirieron más compresión en las frecuencias altas. Las preferencias no podían predecirse a partir de los datos audiométricos, del inicio de la pérdida, o por experiencias anteriores con amplificación. Los datos sugieren que los clientes con hipoacusias moderadamente severas a profundas, deberían adaptarse con tasas de compresión en las frecuencias graves en el rango de 1:1 a 2:1, y que un ajuste fino es esencial.
