Abstract Background Patients with metastatic cancer to the brain have a poor prognosis. In clinical practice, MRI is used to delineate, diagnose and plan treatment of brain metastases. However, MRI alone is limited in detecting micro-metastases, delineating lesions and discriminating progression from pseudo-progression. Combined PET/MRI utilises superior soft tissue images from MRI and metabolic data from PET to evaluate tumour structure and function. The amino acid PET tracer 18F-FACBC has shown promising results in discriminating high- and low-grade gliomas, but there are currently no reports on its use on brain metastases. This is the first study to evaluate the use of 18F-FACBC on brain metastases. Case presentation A middle-aged female patient with brain metastases was evaluated using hybrid PET/MRI with 18F-FACBC before and after stereotactic radiotherapy, and at suspicion of recurrence. Static/dynamic PET and contrast-enhanced T1 MRI data were acquired and analysed. This case report includes the analysis of four 18F-FACBC PET/MRI examinations, investigating their utility in evaluating functional and structural metastasis properties. Conclusion Analysis showed high tumour-to-background ratios in brain metastases compared to other amino acid PET tracers, including high uptake in a very small cerebellar metastasis, suggesting that 18F-FACBC PET can provide early detection of otherwise overlooked metastases. Further studies to determine a threshold for 18F-FACBC brain tumour boundaries and explore its utility in clinical practice should be performed.

ABSTRACT CONTEXT: Brain metastases are the most common tumors of the central nervous system. Because of their high frequency, they may be associated with rare situations. Among these are tumor-to-tumor metastasis and an even a rarer situation called simultaneous brain tumors, which are more related to primary tumors of the reproductive and endocrine systems. CASE REPORT: A 56-year-old male patient with a history of renal cell carcinoma (which had previously been resected) presented with a ventricular lesion (suggestive of metastatic origin) and simultaneous olfactory groove lesion (probably a meningioma). First, only the ventricular lesion was dealt with, but after a year, the meningothelial lesion increased and an occipital lesion appeared. Therefore, both of these were resected in a single operation. All the procedures were performed by the same neurosurgeon. The patient evolved without neurological deficits during the postoperative period. After these two interventions, the patient remained well and was referred for adjuvant treatment. CONCLUSIONS: This study provides the first description of an association between these two tumors. Brain metastases may be associated with several lesions, and rare presentations such as simultaneity with meningioma should alert neurosurgeons to provide the best oncological treatment.

Synchronous cancers are defined as two or more histologically distinct malignancies arising in the same site, following each other in a sequence of < 2 months. Synchronous primary cancers are rare in general population and account for only 0.5-1.7% of gynaecological malignancies.1,2 It is important to distinguish synchronous primary tumors from metastatic malignancies.3 Case Report:  We report a case of 55 years old female with history of pain in abdomen for the last one year. Surgical intervention was planned after complete work up. Hysterectomy with bilateral salpingo-oophorectomy and omentectomy was done. Histopathological examination reported the presence of synchronous endometrial adenocarcinoma well differentiated and malignant ovarian tumor low grade serous papillary cystadenocarcinoma. Conclusion: Though a rare presentation synchronous primary endometrial and ovarian cancers, usually identified at early stage and have a good prognosis.

Background: Spinal tuberculosis is a common form of extrapulmonary manifestation of tuberculosis. Among them tubercular involvement of the lumbosacral region or isolated involvement of sacram is very rare presentation. Aim of the work: The aim of this case presentation is to describe two rare cases of Pott’s disease which clinically mimicked spinal tumor. Case report: A 60-year-old hypertensive, diabetic male patient admitted with low back pain radiating to both lower limbs for 7 months along with low grade fever, generalized weakness and weight loss for the same duration. Clinically, patient had features of flaccid paraparesis, tenderness at the lumbosacral junction, restricted SLR on both sides. There was no features of autonomic dysfunction. MRI of lumbosacral spine showed infective spondylodiscitis at L4 to S1 level with collapse of L5 vertebrae along with paravertebral and epidural abscess. Diagnosis was confirmed by CT guided FNAC from the lesion. Next case was a 23-year-old male patient presented with back pain and walking difficulty for 5 months with some constitutional symptoms.MRI of the dorsal spine with whole spine screening showed multifocal skipped lesion involving whole spine with presence of huge presacral collection. Percutaneous transpedicular aspiration of the purulent material under C-Arm guidance was done and tuberculous infection confirmed by Gene Xpert. Conclusion: Though sacral tuberculosis is rare presentation of Pott’s disease but it should be considered as one of the differential diagnosis in any lytic lesion involving the sacrum in perspective of our country.
