In this chapter several topics related to anesthesia for pain management are discussed. Subtopics include anesthetic management of patients with complex regional pain syndrome (CRPS) types 1 and 2, performing a stellate ganglion block, and prevention of local anesthetic toxicity. The chapter begins with a discussion of the mechanisms of pain onset. Next, evaluation and treatment of chronic pain are addressed. Trigger points in myofacial pain are outlined and the mechanism of transcutaneous electrical nerve stimulation (TENS) presented. A case involving a pregnant patient on chronic methadone therapy is presented. Management of cancer pain, post-amputation pain, and lower back pain and sciatica are also addressed. Each scenario is presented as a short, three- to four-question additional topic.

Background: Complex Regional Pain Syndrome (CRPS) is a neuropathic pain syndrome associated with edema, muscle weakness, and hyperhidrosis. It can be precipitated by fracture, surgery, and spinal cord injury and usually involves the ipsilateral and sometimes contralateral extremity. Case Details: A 47-year-old male with CRPS Type-II involving the upper extremity had severe neuropathic pain that was limiting his ADLs despite medical, physical and occupational therapy. This case discusses the use of stellate ganglion block in the treatment of upper extremity CRPS Type-II. Conclusion: Stellate ganglion blockade is an effective adjuvant therapy in the treatment of CRPS Type-II when conservative therapy has failed to provide improvement in pain, highlighting a need for a multimodal therapeutic strategy. Stellate ganglion block is used as an adjunctive therapy method in various painful situations mediated by the sympathetic nervous system, especially complex regional pain syndrome. The process after medical and stellate ganglion block treatment in a 66-year-old female patient diagnosed with hypertension, ischemic heart disease, heart failure, Raynaud phenomenon, who presented to our algology outpatient clinic for complaints of bruising and severe pain at the fingertip, is presented with literature information.
