Abstract Background and Aims Several dialysis patients frequently suffer of different mineral and bone disorders (CKD-MBD) associated with secondary hyperparathyroidism (sHPT). Among CKD-MBD, adynamic bone disease (ABD) is an alteration characterized by reduced osteblasts and osteoclasts, no accumulation of osteoid and low bone turnover. The histologic pattern of ABD is generally associated to low levels of PTH. Etelcalcetide is a novel second-generation calcimimetic given intravenously after each hemodialysis session that has a longer elimination half-life than cinacalcet. Plasmatic concentration remains stable from 24 h to 48 h after injection. One potential risk of calcimimetics, such as etelcalcetide, is the dramatic and sustained PTH lowering, which could lead to the induction of adynamic bone disease (ABD). ABD and elevated serum levels of advanced glycation end products (AGEs) often are found in patients with renal failure caused by diabetic nephropathy since AGEs are involved in the pathogenesis of ABD by inhibiting osteoblastic activity and by parathyroid hormone secretion in response to hypocalcemia. So, diabetic patients treated with etelcalcetide could be considered at increased risk of developing ABD. Aim of our study was to verify the incidence of adynamic bone disease, (defined by low PTH levels) in prevalent diabetic and non-diabetic HD patient of three large community Hospital. Method Data were collected from 3 dialysis units with n = 130 patients on the charge for a period of 1 year from start of the calcimimetic. A total of 40 patients ( 23 male, 17 female) on etelcalcetide were enrolled (21 Diabetic, 19 non diabetic patients). Time points of assessment included 1-3-6-12 months. Patients were 18-years-old or older; they were on stable doses of active vitamin D analogs, phosphorus binders, a supplement of oral calcium, and calcium concentration in dialysate (1.25–1.50 mmol/L) Results Median age was 55,9 years and dialysis vintage was 4.6 year. 59,5% percent of patients switched from cinacalcet to etelcalcetide (90 days from last cinacalcet prescription); the remaining patients were calcimimetic naive. 40% of patients had a history of at least one cardiovascular event 61.5% had a starting etelcalcetide dose of 5 mg and the median weekly dose was 7.5 mg (range: 2.5-15 mg). On Diabetic Group: mean PTHi, Ca2+ and P before calcimimetic start was respectively 844±479,30 pg /mL , 9,95±0,97 mg/dl , 5,02±0,99 mg/dl. In non diabetic patients: mean PTHi, Ca2+ and P before calcimimetic start was: 793,36±534,41 150 pg /mL, 9,33±0,70 mg/dl, 5,9±1,17 mg/dl Conclusion Results of our study show that after 1 year of etelcalcetide treatment, levels of PTHi are lower more in non-diabetic patients, ( M12: 473±340,08 vs 253,13 + 98,87 p:0.042) despite scientific evidence currently supporting hypothesis that osteoblastic activity is reduced by AGES and ABD risk is increased in diabetic nephropathy. Therefore, etelcalcetide could be used safely in diabetic patients and could even protect from the risk of development ABD. However, further studies are required to validate this hypothesis. In the general population, blacks have higher parathyroid gland mass and circulating parathyroid hormone (PTH) levels than whites. This may predispose black patients to more severe parathyroid disease when renal failure develops. Therefore, racial differences in the severity of uremic hyperparathyroidism were examined in a population of patients with endstage renal disease (ESRD). Among ESRD patients receiving hemodialysis or peritoneal dialysis, two or more values of intact PTH (immunoradiometric assay, pg/ml) obtained at least 90 d apart were available in 1270 prevalent cases (61.1% blacks, 51% males, and 31.1% diabetic), including 466 incident cases with onset of ESRD after 1993. Maximum PTH levels were analyzed as a function of race, gender, age, diabetic status, and levels of serum calcium, phosphorus, alkaline phosphatase, and aluminum. Using a stepwise multiple regression model, the determinants of maximum PTH in the order of their importance were black race, serum phosphorus, absence of diabetes, younger age, serum calcium, and female gender. The maximum PTH levels averaged 641.7 in blacks and 346.0 in whites after adjusting for age, gender, diabetic status, serum calcium, and phosphorus (P < 0.0001). In blacks compared with whites, the odds ratio (95% confidence interval) for adynamic bone disease (maximum PTH < 150 pg/ml) was 0.26 (0.17 to 0.41), whereas the odds ratio for hyperparathyroid bone disease (mean PTH > 500 pg/ml) was 4.4 (2.10 to 9.25). Race is a major independent determinant of uremic secondary hyperparathyroidism. Among ESRD patients, blacks may be at an increased risk for hyperparathyroid bone disease and whites for adynamic bone disease.

BACKGROUND Increased levels of urinary biomarkers can be detected in type 2 diabetic patients before the onset of significant albuminuria and may be used as an early marker of renal injury in diabetic nephropathy (DN) which would play a significant role for the effective management and treatment approaches in diabetic care. We wanted to evaluate cystatin C and microalbumin as effective early biomarkers in assessing nephropathy in patients with type 2 diabetes mellitus in this study. METHODS A cross-sectional study was conducted among 180 subjects grouped into healthy controls, clinically proven T2DM without nephropathy and type 2 DM with nephropathy comprising 60 participants in each group. Fasting and postprandial blood samples and urine samples were collected and analysed by standard methods. eGFR was calculated using CKD-EPI 2012 equation. IBM - SPSS version 20 was used for statistical analysis. RESULTS Diabetic nephropathy patients had significantly elevated serum cystatin C and microalbumin (2.43 ± 0.59, 700.5 ± 591.8 mg / L, respectively), compared to T2DM (0.98 ± 0.26, 63.7 ± 102.9 mg / L, respectively), and the control study subjects (0.81 ± 0.16, 11.15 ± 8.9 mg / L, respectively). Serum cystatin C showed AUC of 0.994 (95 % CI, 0.986 - 1.00) whereas microalbumin showed 0.944 (95 % CI, 0.907 - 0.981). Serum cystatin C showed a sensitivity of 96.7 % and a specificity of 91.7 % at a cutoff point of 1.34 mg / L whereas at a cut-off point of 138.5 mg / L for microalbumin, the sensitivity and specificity were 90 % and 83.3 % respectively. CONCLUSIONS Serum cystatin C and microalbumin both could be considered as markers for early detection of nephropathy in T2DM patients. The more prominent rise in serum cystatin C values provide an earlier diagnosis of diabetic nephropathy among T2DM patients.
