Introduction: Clinical practice guidelines emphasize the importance of accurate blood pressure measurement and recording to diagnose and treat hypertension. Trends in terminal digit preference (typically manifest by a terminal digit of ‘0’) have not been examined nationally. The growing use of automated blood pressure devices may have reduced terminal digit preference and improved accuracy over time. Objective: To evaluate trends in terminal digit preference in office blood pressure measurements among adults with hypertension by patient and provider characteristics. Methods: We used IQVIA National Disease and Therapeutic Index (NDTI) data from January 2014 through June 2019. The NDTI is designed to be nationally-representative of all patient visits to office-based physicians and uses a two-stage stratified sampling design to sample ~4,000 physicians per quarter who report information on all patient visits on 2 random workdays. We included all hypertension treatment visits (~60M/year) among adults aged ≥18. We examined trends in the proportion of hypertension treatment visits with recorded systolic (SBP) and diastolic (DBP) blood pressure measurements with a terminal digit ‘0’. The expected percent of blood pressures with ‘0’ is 10% for automated and 20% for manual readings. Results: There was a decrease in the percent of visits with SBP (43.0% to 37.4%) or DBP (44.3% to 38.1%) recordings ending in zero ( Table ). The decrease in percent of SBPs with a terminal zero was similar by patient and provider characteristics, though the percentage of SBPs with a terminal ‘0’ was consistently higher among patients aged ≥60, when SBP ≥140 mmHg, and among cardiologists. Conclusions: Terminal digit preference is common indicating systematic error in blood pressure measurement and recording, despite some improvement over time. This may lead to under- and overtreatment of patients with hypertension. Improving the quality of blood pressure measurement is central to improving hypertension diagnosis and control in clinical practice.

EMPOWERMENT OF ELDERLY POSYANDU CADRES FOR PREVENTING HYPERTENSION AND ITS COMPLICATION IN PURWOKERTO. The highest prevalence of degenerative diseases in Banyumas regency is hypertension. The prevention of hypertension in elderly could be done by Elderly Posyandu. However, there are many problems which occur in Elderly Posyandu of Soka Nirmala III and Sejahtera II in preventing hypertension and its complication, such as the limitation of materials for blood pressure measurement, lack of knowledge for hypertension management, lack of knowledge about self-treatment for reducing blood pressure. The purposes of this programme were to give kits for hypertension measurement to elderly posyandu, to conduct training of hypertension prevention, training about diet management, training about elderly exercise, training about self-treatment for reducing blood pressure. This community service was done through health education, training, and simulation. This community service could improve the knowledge of cadres and elderly in managing hypertension, improve knowledge in conducting exercise for reducing hypertension, improve knowledge in diet management, and improve the knowledge about self-treatment for reducing hypertension. It is expected that the prevalence of hypertension and its complication could be reduced and the quality of life of elderly could be improved.


Kidney damage can be detected early and simply by urine and blood tests and blood pressure measurements. About one in ten adults has some form of kidney disease. It can develop at any age, and chronic kidney disease is becoming more common with increasing age. The kidneys are an extremely important organ for the health of the whole organism. About 200 liters of blood are purified daily. The kidneys remove waste material that is excreted in the urine. Regardless of age, simple and timely treatment can slow the progression of kidney disease, prevent complications and improve quality of life.

Background: Community pharmacists play an important role in counseling of rational drug use for population. The study aimed to characterize counselling activities of drug use and demand of counselling tools at some community pharmacies at Hue City. Materials and method: A 17-question survey were asked to fill pharmacy staffs of a convenient sample of 100 community pharmacies at Hue City from 2/2017 to 7/2017. Results: There were 58 pharmacies (58.0%) answered this survey. Cough, headache, fever, high blood pressure and diabetes were the most popular symptoms/diseases presented in pharmacies. Patients often need counselling about dose (82.8%) and when to take medicine (79.3%). The main bariers for counselling were a lack of time (53.4%), and of skills/medthods for counseling (31.7%). About 96.6% pharmacies demanded counselling tools and favorite formats were book (32.9%) or pocket handbook (29.3%). Conclusion: Most pharmacies demanded counselling tools. Other studies should be conducted to develop supporting tools for counseling and to assess the quality of counseling in pharmacies.
